1 30-DAY NOTICE OF INTENT TO VACATE You are hereby given notice that I (we) will be vacating my home which is a bedroom home located at (Street Address) on (). My reason for terminating my lease is: The date given above is a DEFINITE VACATING DATE, and you are hereby authorized to pre-lease the premises to a prospective resident. I (we) understand that if the premises is not vacated as described above, the applicable provisions of the Lease will apply and I (we) may be responsible for additional rent as well as any loss or damages suffered by you or by a new resident deprived of possession. ** YES NO I (we) grant authorization to share my contact information with the privatized housing partner at my next duty station for leasing purposes. 2 RECEIPT OF CLEANING STANDARDS I have received a copy of the Balfour Beatty Communities cleaning guidelines and procedures for clearing my home at the end of my stay at F.E. Warren AFB Homes. I have been provided a copy of both the Pay & Go Cleaning Agreement and Inspection Standards of Conditions at Move-Out for a self-clean. Resident Signature Resident Specialist

3 FINAL INSPECTION CLEANING STANDARDS In order to avoid any delays in clearing of your housing and to prevent you from incurring charges, the following guidelines should be followed prior to the final inspection of your home: Walls and Ceilings Remove all nails, picture holders, molly bolts, etc. Spackling of holes is not required. Please leave anchors intact in wall. Remove all self-adhesive hooks. Spot painting is discouraged, as different paint batches do not match, thus creating an unsightly condition. Remove all scuffmarks, wall paper, borders, decals, and restore any wall damage. Repaint rooms painted with non-standard colors back to the original interior color. Windows, Doors, Medicine Cabinets and Wall Cabinets Be sure that all window coverings that were provided at move-in are not damaged and are affixed properly to windows. All storm windows, doors and screens must be in place. Remove all contact paper and shelf paper. Be sure to remove all items from medicine cabinet, closets, kitchen cabinets and closets. Floors Carpets shall be vacuumed and free of dirt and stains. Resident with pets will be required to have the carpets cleaned professionally. Tile and wood floors must be swept and damp-mopped. Appliances Ensure all the shelves are in place in refrigerator. Ensure all broiler pans and oven racks are in place in range. Ranges and refrigerators must be in kitchen, not basement or stored elsewhere. Be sure that all items are removed from refrigerator and freezer, and that it is wiped clean and left on the lowest setting. Be sure that dishwasher is empty and wiped clean. Be sure that oven and range top, including drip pans are thoroughly cleaned. DO NOT USE OVEN CLEANER. Exterior Remove mud, chalk, crayon, oil, grease or other substances off sidewalks, patio slabs, driveways, fences and walls. All trash cans (regular and recyclables) must be empty and rinsed clean and placed in the garage upon move out. Remove all nails and anchors, including cable anchors that are no longer in use, from exterior of unit. Pull all weeds from front and back lawns, flowerbeds, tree circles and driveway/sidewalk cracks. Remove all trash, debris and animal waste. Repair yard damage, including filling in holes, and ensure grass is growing in areas destroyed by pets or children. Garages/Carports (If Applicable) Remove all personal property and thoroughly sweep out area. Remove all nails and hooks. Remove dirt, spilled paint and grease stains from shelves and floors. Miscellaneous Home must be free of all offensive odors. Residents must turn in their house, garage and their cluster mailbox keys to the Resident Specialist. All personal property (such as carpets, wallpaper, yard fences, swings, etc.) must be removed. ALL CHARGES ASSESSED ARE DUE IMMEDIATELY AT THE FINAL INSPECTION

4 PAY & GO CLEANING STANDARDS 1. Remove all trash, food, and personal belongings. 2. Clean front and back yards of extraneous materials: rubbish, garbage, trash, unwanted personal items and animal feces. If the back yard was fenced in, then back yard must be mowed to the same standards as the rest of military housing. All holes in the yard must be filled. Pull all weeds from front and back lawns, flowerbeds, tree circles and driveway/sidewalk cracks. 3. Fences must be taken down and fence holes filled prior to final inspection. 4. Remove all trash from the sidewalk and curb. 5. Clean your shed and garage including trash containers and recycling bins. They must be free of odor and excess debris and placed in the garage upon move out. 6. Clean exterior doorways, porches, patios, garages and windows of dirt, cobwebs, trash and debris. 7. Satellite dishes and mountings must be removed. 8. There must be working bulbs for all light fixtures. 9. Remove all nails from the walls. Nail holes do not need to be filled. Leave anchor bolts in the wall. 10. This service does NOT include carpet cleaning. ALL CHARGES ASSESSED ARE DUE IMMEDIATELY AT THE FINAL INSPECTION

5 PAY & GO CLEANING AGREEMENT This agreement is drawn between (hereinafter referred to as the resident) and Balfour Beatty Communities (hereinafter referred to as contractor). The resident living at address # has agreed to pay the contractor for the cleaning services to be provided upon termination of the above mentioned housing unit. The payment must be received in full by money order, cashier s check, VISA or Master Card in the amount of $ (a pre-determined cost) due to size as noted below prior to final inspection. Once the agreement has been made and all monies have been paid, the resident s final inspection will reflect approval in the cleaning category. It is still the resident s responsibility to repair any damages caused by negligence or misuse and return the exact number of keys issued for the home. All repairs must be completed prior to final inspection or charges for repair may be assesses to your account. Any damages must be repaired by resident. Also, a superficial clean (per attached memo) is required. If the unit is carpeted, the resident is responsible for cleaning the carpet. The resident is responsible for any damages to the carpet, including pet damages and pet odors. The resident is also responsible for pet stains on the carpet and would be required to have the carpet cleaning company do a pet treatment on those stains. If all items on the Pay & Go Cleaning Agreement have been fulfilled, the resident will be cleared from housing on the date of final inspection and the contractor will bring in the cleaning subcontractor to clean the home after it is vacated.
