Master Gardener Volunteer Mentors The Master Gardener Community Garden Mentor Program is designed to match an experienced Ohio State University Extension Master Gardener Volunteer (MGV) with a community garden. MGV s are to provide growing advice to gardens throughout the growing season. It can be in the form of site visits, phone calls, , etc. The MGV mentor can help you solve growing problems but should not be expected to do labor on site. We have a limited number of MGV s, so we will match the gardens that have the greatest need (usually the least experienced) first. 4 Suburban Community Gardening Agreement 2017 The Suburban Community Gardening Program will: Provide community garden based education and training through Ohio State University Extension (OSUE). Provide registered community vegetable garden groups the opportunity to purchase a fire hydrant permit at the City of Cleveland Water Department. Visit community vegetable gardens during the season to provide education and technical assistance, as needed or as requested. Facilitate opportunities for learning and collaboration among community gardens. Provide resources to support garden s sustainability when funding is available. The garden leadership agrees to: Fully complete and return the Suburban Community Gardening Program (SCGP) Application and yearly renewal form, Agreement, and Waiver by its due date. Have at least five registered participants in the garden. All gardeners cannot be from the same family. Provide accurate and timely gardening information to all of the registered gardeners. Distribute any supplies and services made available by the program equitably among the registered gardeners, including the OSUE e-newsletter and workshop schedule. Cooperate with reasonable requests for garden-based information from the Suburban Community Gardening Program. Work with the registered gardeners to see that gardens are planted, maintained, and harvested. Coordinate maintenance of the garden throughout the growing season. Provide OSUE event and workshop information to gardeners. Cooperate to the best of your ability with OSU Extension, registered gardeners, and neighbors. Have or obtain written permission from the landowner to use the land as a community vegetable garden. (If your garden does not have a written agreement and would like help getting one, please contact the Suburban Community Gardening Program.) 5 2017 WAIVER FORM RELEASE OF CLAIMS I am duly aware of the risks and hazards that may arise through participation in Ohio State University Extension's Community Gardening Program and assume any expenses and liabilities I incur in the event of an accident, illness, or other incapacity. If I have had any questions about the Community Gardening Program, its nature, risks, or hazards, I have contacted the Extension Educator and/or Program Coordinator and discussed those questions with him or her to my satisfaction. In consideration of being granted the opportunity to participate in the Community Gardening Program, I, for myself, my executors, administrators, agents, and assigns, do hereby release and forever discharge the Ohio State University, and its Board of Trustees, administrators, employees, servants, agents, assigns, and officers and the cooperating landowner for all claims and damages, demands, and any actions whatsoever, including those based on negligence, in any manner arising out of my participation in this activity. I understand that this release means that, among other things, I am giving up my right to sue Ohio State University and cooperating landowner for any such losses, damages, injury, or costs that I may incur.
