1 HEOA Institutional Disclosure Requirements Emergency Response and Evacuation Procedures Under the direction of the Safety Committee, the following emergency response plan provides a framework by which Trinity International University (TIU) will respond in emergency situations. The goal of this plan is to allow for rapid and efficient deployment of resources to deal with an emergency situation and allow the TIU community to adapt to the situation. It will be the responsibility of individual departments to develop their own emergency plan for their area of responsibility. These plans shall be held in each respective department, with a copy Campus Safety & Security Services and Facility Services. 1. Procedure for immediate notification of the campus community a. Speaker intercom phones are installed on some staff and faculty offices for 4 or 10 digit dialing from either on or off campus, respectively. b. Speaker intercom phones are installed in all Deerfield campus classrooms. c. Two outdoor speakers are installed for notification to outdoor areas on the Deerfield campus. i) West side of Campus: One outdoor speaker equipped with 4 or 10 digit dialing from either on or off campus, respectively; ii) East side of Campus: One outdoor speaker is provided by Bannockburn Police Department, activated at a call box on the pole. d. broadcast system. e. A popup message on the computer monitor for users logged into the TIU Novell system. f. An official TIU Twitter account has been established. g. Trinity College Residence Life staff keeps an inventory of all resident students mobile phone numbers. In the event of an emergency each Resident Assistant would text the residents of their floor. Group texting will be used where available for quicker communication. 2. Future additions a. Installation of speaker intercom phones in all faculty offices. b. Installation of speaker intercom phones in all resident apartments. c. Mass Notification Software to communicate by text message, pre- recorded phone call, voice mail message, etc.

3 Level 4 - Major Impact Examples: Departments Impacted: Significant portion of campus (1 or more zones) evacuated due to severe weather, chemical spill, riot, bomb threat Facility Services, Information Technology, Safety & Security Services, Residence Life, Academics, Business Office, Communications & Marketing, Human Resources and Civil Authorities 5 days Level 5 - Maximum Impact Example: Department Impacted: Complete campus evacuation Entire University and Civil Authorities 7 days or more

4 Declaration of Emergency Situation Following the order of command designated by the Emergency Response Flow Chart, the President, the Executive Vice President, the Senior Vice President of Business & Finance, the Director of Facility Services or the Civil Authorities Liaison is authorized to declare a Level 3, 4, or 5 emergency when it is necessary to put into effect emergency procedures for a portion or all of the campus. This individual may act on behalf of the University to activate the Unified Command and Operations Center for the coordination of emergency procedures. Emergency Response An emergency, or the potential for an emergency, should be reported immediately to TIU s Campus Safety & Security Services department by dialing x6401 from an on- campus phone or dialing If there is a fire, or emergency medical personnel are necessary, one should first dial from an on- campus phone and then have a companion call x6401. Campus Safety & Security Services will respond according to normal operating procedures by: 1. Directing emergency personnel to the scene. 2. Dispatching an officer to investigate and coordinate activities. 3. Contacting appropriate University administration to determine appropriate level of response for the emergency situation. It may be necessary to deviate from standard operating procedures in some situations but in every event, communication between Campus Safety & Security Services and University administration will be maintained. The Bannockburn Police Department will be in charge of disseminating emergency information to the larger Bannockburn/Deerfield and Lake County community Response Levels 1 and 2 Campus Safety & Security Services notifies Facility Services of weather conditions by calling x7135 during business hours or texting/calling Facility Services staffs who are on call after hours. Depending upon the extent of the weather, Campus Safety & Security Services will also involve Academics, Residence Life and University Administration.

5 Level 3, 4 and 5 If an emergency situation is occurring or appears to be imminent, the Emergency Coordinator shall contact the President or designate to initiate emergency procedures. The Emergency Coordinator shall also notify the civil authorities. The Emergency Coordinator and the Executive Council along with the Civil Authorities hereby known as the Unified Command shall lead the Operations Center. Appropriate personnel from the departments represented in the Operations Center shall deploy their resources to make the Operations Center fully operational. The Civil Authorities shall also ask for assistance and be granted the reassignment of personnel to assist in an emergency situation. Testing Procedures The institution conducts three drills annually to test the emergency response and evacuation procedures across the campus. They are Fire Drill, Shelter in Place Drill, and Severe Weather Drill. Every two or three years a Tabletop Scenario Emergency Response is conducted by the Unified Command.
