1 OUTDOOR FIREWORKS DISPLAY THIS APPLICATION MUST BE COMPLETED. IF ALL REQUIRED DOCUMENTS HAVE NOT BEEN SUBMITTED, THERE WILL BE AUTOMATIC DENIAL OF THE PERMIT. ALL SITE PLANS FOR YOUR FIREWORKS DISPLAY MUST BE COMPLETE. IF THE SITE PLANS ARE NOT COMPLETE, THERE WILL BE AN AUTOMATIC DENIAL OF THE PERMIT. THANK YOU FOR YOU COOPERATION. 2 REQUIRED SUBMITTALS FOR FIREWORKS DISPLAY PERMITS 1. Permit Fee 2. Complete fireworks display permit application 3. Copy of the lease agreement or written permission to use the land of the display site for intended purpose 4. Copy of the public liability insurance in the amount of $1,000, Copy of the invoice from the supplier of fireworks to be fired. This must show the number of pounds of products and the size and type of the shells to be fired. 6. Copy of Colorado Division of Fire Safety licenses for all Display and Pyrotechnic operators working at the site. 7. Copy of Pyrotechnics Guild International Certificate that indicates at least one site operator has successfully completed the Display Fireworks Shooters Safety Certification Program. 8. A letter from the company providing information on their personnel involved with the display, including their training and qualifications. 9. A scale drawing of the display site showing the setbacks to property lines and all nearby structures or exposures. This drawing must also include: a. The placement and description of parking barriers to be used around the display site to provide for site security and traffic control. b. The location of any on-site storage. c. The location of fire extinguishers d. The location of the nearest phone or radio that can be used by the display staff to call for emergency assistance during the hours of operation. e. The location of mortars and how they will be placed at the site. 10. The lead pyrotechnist must meet with a fire department inspector at least one week prior to the event, at the display site, to discuss the display and procedures at the launch site, as well as the security and other issues. NOTICE: SELLING ANY ILLEGAL FIREWORKS, FAILING TO COMPLY WITH ANY ORDER OF A FIRE DEPARTMENT OFFICIAL, FAILING TO SUBMIT ANY REQUIRED DOCUMENT, FALSIFYING OR WITHHOLDING ANY INFORMATION ON THIS APPLICATION, OR FAILING TO GET THE REQUIRED DOCUMENTS WEN REQUESTED MAY RESULT IN THE IMMEDIATE REVOCATION OF THIS PERMIT AND CANCELLATION OF THIS DISPLAY. A SUMMONS AND/OR CONFISCATION OF PRODUCTS MAY ALSO RESULT.

3 GENERAL REQUIREMENTS FOR FIREWORKS DISPLAYS 1. All required documents shall be submitted. 2. Security of explosives shall be maintained at all times. 3. Safe on-site procedures shall be strictly adhered to. 4. No smoking or open flames allowed at any time within 25 feet of the launch site or storage area. 5. No alcohol, drugs, or persons under their influence allowed at the display site. 6. All unauthorized personnel shall be kept out of secured areas. 7. Immediate compliance with fire department orders. 8. A valid permit shall not be transferable. 9. A permit shall be valid for the date and approximate time of the fireworks display only (or alternative date). 10. All applications shall be received by the Fire Department at least 14 days prior to the display. 11. A final inspection of the fireworks display site shall be conducted before any fireworks are launched. 12. All fireworks display areas shall have a post-show-search conducted by the pyrotechnist to recover all fireworks fallout and all debris shall be picked up from the launch site. 13. All perimeters shall be measured from launch site. 14. Each launch site shall be provided with two 2A 20B:C multi-purpose dry chemical fire extinguishers readily accessible and in good working order. Each extinguisher shall carry a current annual inspection tag. 15. The applicant shall launch all fireworks. 16. Vegetation within the required setbacks shall be a maximum of two inches above the ground with the exception of tress and shrubs. Weeds, vegetation, etc shall be removed from within 30 feet of the actual launch site. 17. All public fireworks displays and pyrotechnic effects used must be electrically ignited using a system, which requires a key to operate. I have received, read, and understand all information and handouts regarding requirements by the Red, White and Blue Fire Protection District for a Fireworks Display Permit.
