Vogl Document: Issue/ Start of Next August 2014 See Document Control Matrix Revision Date: Review Period: Revision #: 1 No. of Pages: 5 This procedure has been developed as to provide an overview of the requirements of the Ontario Fire Code, O.Reg.213/07 as it relates to the Fire Safety Plans developed for facilities owned by the Corporation of the Town of Grimsby. The purpose of Fire Safety Plans is to provide facility users with instructions on how to safely and efficiently evacuate the building during a fire and also to provide municipal supervisory staff with documentation that clearly defines duties and responsibilities during a fire as well as their day-to-day duties with respect to fire safety. Fire Safety Plans are also a useful tool for municipal Fire Departments during a fire emergency and therefore are required to be readily available for their use. 2.0 SCOPE This procedure applies to all facilities owned by the Corporation of the Town of Grimsby. 3.0 DEFINITIONS Chief Fire Official Event Organizers Owner Supervisory Staff Refers to the Fire Chief or a member (or members) of the Grimsby Fire Department designated by the Fire Chief to act in this capacity. Event Organizers are any individual, group or corporation that rents or uses space in the building and is not considered a User Group (i.e. shows, festivals, dances, concerts, banquets, weddings, meetings, etc.). Refer to Section 5.4 of this procedure. The Ontario Fire Code, O.Reg.213/07 defines Supervisory Staff as, those occupants of a building who have some delegated responsibility for the fire safety of other occupants under the fire safety plan. For the purpose of Fire Safety Plans created for Town facilities, Supervisory Staff have been divided into two categories: Supervisory Staff (Corporate) - are Town of Grimsby staff who have a high level responsibility over the Town s corporate program for fire safety, such as ensuring Fire Safety Plans are created and implemented for all Town owned facilities and resources are provided (staffing, budgets, etc.) to ensure legislative requirements can be met, etc. Page 1 of 5

7 CORPORATION OF THE TOWN OF GRIMSBY Fire Code Requirements - General CO-GEN-FSP-PRO User Groups Worksite Manager 4.0 RESPONSIBILITIES Council (Owner) Town Manager (Owner Representative) EHS Compliance Manager Fire Chief (Chief Fire Official) Senior Manager/ Department Head Worksite Manager Supervisory Staff (Facility Specific) - are staff who are responsible for the day-to-day administrative duties for fire safety at a specific facility and also have specific responsibilities in responding to fire-related emergencies. Includes Town of Grimsby, User Group or Event Organizer staff. User Groups are any group that leases, rents or uses space on a regular basis within any of the Town facilities. A member of management designated by the employer to be responsible for supervision/control of a worksite(s), including the provisions for Fire Safety. Refer to the Town s OHS program document called Worksite Manager List (CO-GEN-OHS-LM ) for a list of current Worksite Managers. Ensure that Fire Safety Plans are created, endorsed by the Town Manager and/or Council, submitted to the Fire Department for approval and fully implemented for all Town facilities. Carry out responsibilities as identified in this procedure and the Responsibilities for Supervisory Staff procedures (specific to each facility). Ensure that Fire Safety Plans are created, endorsed by the Town Manager and/or Council, submitted to the Fire Department for approval and fully implemented for all Town facilities. Carry out responsibilities as identified in this procedure and the Responsibilities for Supervisory Staff procedures (specific to each facility). Provide administration over all Fire Safety Plan related documents and assist in the overall coordination of the Town s corporate fire safety program. Carry out responsibilities as identified in this procedure and the Responsibilities for Supervisory Staff procedures (specific to each facility). Review and approve (per legislated authority) Fire Safety Plans and applicable revisions created for all Town facilities. Ensure that Fire Safety Plans are created, reviewed, endorsed by the Town Manager and/or Council, submitted to the Fire Department for approval and fully implemented for all Town facilities under his/her respective authority. Carry out responsibilities as identified in this procedure and the Responsibilities for Supervisory Staff procedures (specific to each facility). Ensure that Fire Safety Plans for all Town facilities under his/her respective authority are fully implemented and reviewed at least annually. Carry out responsibilities as identified in this procedure and the Responsibilities for Supervisory Staff procedures (specific to each facility). All Staff Follow all procedures as outline in the Fire Safety Plan documentation for the respective site location. User Groups and Event Organizers Follow all procedures as outline in the Fire Safety Plan documentation for the respective site location. Carry out responsibilities as identified in this procedure and the Responsibilities for Supervisory Staff procedures (specific to each facility). 5.0 PROCEDURE 5.1 Implementation of Fire Safety Plans Fire Safety Plans have been developed for all municipal facilities owned by the Town of Grimsby. These Fire Safety Plans have been developed to satisfy the requirements of Section 2.8 of the Ontario Fire Code, O.Reg.213/07. The subsections under Section 2.8 require the Fire Safety Plans to be fully implemented (i.e. put into practice) as per the approved Fire Safety Plan documented for each respective facility. This includes training Supervisory Staff (refer to Section 3.0 for definition) conducting fire drills, completing maintenance requirements for the listed life safety systems, etc. 5.2 Creation, Review and Approval of Fire Safety Plans The process for the creation, review and approval of Fire Safety Plan documents is described in the FSP Control of Documents (CO-GEN-FSP-PRO ) procedure. The protocols established in this procedure have been developed to comply with the requirements of the Ontario Fire Code, O.Reg.213/ Fire Safety Plans are approved both by the Owner 1 (as the Corporation of Town of Grimsby) and the Chief Fire Official 2 (as the approval authority as defined under the code). Note: the Chief Fire Official s approval does not relieve the Owner of any of their responsibilities defined in the Ontario Fire Code, O.Reg.213/07 or set out in the each Fire Safety Plan Worksite Managers 3 are required to review the applicable Fire Safety Plan for their respective facilities and communicate any changes to the EHS Compliance Manager. These reviews are required to be completed as often as necessary, but at intervals not greater than 12 months The EHS Compliance Manager is responsible for making the appropriate changes to the document, obtaining the appropriate approvals and distributing the documents in accordance with the FSP Document Control Matrix (CO-GEN-FSP-LM ). 1 Refer to Section 5.4 for definition. 2 Refer to Section 3.0 for definition. 3 Ibid. 9 CORPORATION OF THE TOWN OF GRIMSBY Fire Code Requirements - General CO-GEN-FSP-PRO Minor changes to any Fire Safety Plan documents will be endorsed by the Town Manager on behalf of the Owner; however major changes must be submitted directly to the Owner (Town Council) for endorsement (via Council resolution). All changes, whether minor or major, must be submitted to the Chief Fire Official for approval. 5.3 Storage of the Fire Safety Plan The location of all hard-copy (printed) Fire Safety Plans is described in the FSP Document Control Matrix (CO-GEN-FSP-LM ). As a minimum, a hard-copy of the Fire Safety Plan for each facility is required to be kept at the approved location within each respective facility, readily available for the municipal fire department, supervisory staff and other personnel. 5.4 Definition of Owner Ultimately the building Owner is responsible for carrying out the provisions of the Ontario Fire Code, O.Reg.213/07. Under the code, the Owner is defined as any person, firm or corporation having control over any portion of the building or property under consideration and includes the persons in the building or property Generally the Town of Grimsby is considered the Owner of all facilities; however any User Group or Event Organizer under contract with the Town having control over a portion of a facility or the facility as a whole, will be deemed to be the Owner of the particular area of the facility, during the time that the facility is under their control. As the Owner, the User Group or Event Organizer is responsible to adhere to the provisions of the Ontario Fire Code, O.Reg.213/07 and the requirements of the Fire Safety Plan for the applicable facility. 5.5 Penalties for Non-Compliance Part VII, Section 28 of the Fire Protection and Prevention Act, S.O.1997, C.4 defines the penalties associated with not complying with the Ontario Fire Code, O.Reg.213/07. Penalties are as follows: A corporation is liable to a fine of not more than $100,000; A director or officer of a corporation is liable to a fine of not more than $50,000 or to imprisonment for a term of not more than one year or to both; An individual is liable to a fine of not more than $50,000 or to imprisonment for a term of not more than one year or to both The Grimsby Fire Department (Chief Fire Official) is responsible for enforcing the provisions in the Fire Protection and Prevention Act, S.O.1997, C.4 and the Ontario Fire Code, O.Reg.213/07. Vogl Document: Issue/ Start of Next August 2012 See Document Control Matrix Revision Date: Review Period: Revision #: 0 No. of Pages: 5 The purpose of this procedure is to identify persons requiring assistance to both staff and Managers in order to provide a level of assistance to these persons to ensure they are able to safety evacuate the building in the event of an emergency. 2.0 SCOPE This procedure applies to all facilities owned by the Corporation of the Town of Grimsby. 3.0 DEFINITIONS Emergency Coordinator Event Organizers The Emergency Coordinator is the person designated as the main contact during an emergency evacuation who will meet with the municipal Fire Department to provide access to the building and provide information regarding the status of the evacuation. An Event Organizer is any individual, group or corporation that rents or uses space in the building and is not considered a User Group (i.e. shows, festivals, dances, concerts, banquets, weddings, meetings, etc.). Event Organizer personnel are divided into three categories: Event Organizer Administrator - is a person affiliated with the organization coordinating the special event (i.e. president, board member, etc.) and is responsible for ensuring that the requirements of the fire safety plan are being met by the Event Organizer while occupying a Town facility. Event Organizer Representative(s) - is a person/persons appointed by the Event Organizer who is responsible for leading the evacuation of the area of a facility occupied by the Event Organizer during a special event. Event Organizer Assistants - are persons appointed by the Event Organizer who are responsible for assisting in the evacuation of the area of a facility occupied by the Event Organizer during a special event. Event Organizer Assistants could be security, sound/lighting technicians, wait staff, bartenders or any other persons responsible for event participants or occupants. Page 1 of 5

12 CORPORATION OF THE TOWN OF GRIMSBY Evacuation of Persons Requiring Assistance - General CO-GEN-FSP-PRO User Groups A User Group is any group that rents or uses space on a regular basis within any of the Town facilities (i.e. Grimsby Minor Hockey Association, Grimsby Junior C Hockey team, Grimsby Skating Club, Lincoln Lacrosse Association, etc.). User Group personnel are divided into three categories: 4.0 RESPONSIBILITIES Worksite Manager All Staff Evacuation Assistants & Evacuation Assistant Alternates User Group Representative & User Group Assistants Event Organizer Representatives & Event Organizer Assistants User Group Administrator - is a person who is responsible for the overall administration of the User Group (i.e. president, board member, etc.) and is responsible for ensuring that the User Group is meeting the requirements of the fire safety plan while occupying a Town facility. User Group Representative(s) - is a person/persons appointed by the User Group Administrator who is responsible for leading the evacuation of the area of a facility occupied by the User Group. User Group Assistants - are persons appointed by the User Group who are responsible for assisting in the evacuation of the area of a facility occupied by the User Group. User Group Assistants could be officials, referees, coaches or any other persons responsible for User Group participants or occupants. Ensure that provisions for the evacuation of persons requiring assistance are addressed prior to an emergency in accordance with this procedure. Be aware of persons requiring assistance who access the site during operating hours and their general location within the building. Carryout duties/responsibilities as indentified in this procedure. Optional: Inform the Worksite Manager if they will require assistance in the event of an emergency evacuation. Carryout duties/responsibilities as indentified in this procedure. Be aware of persons requiring assistance who access the site for the purpose of attending User Group activities including their general location within the building. Carryout duties/responsibilities as indentified in this procedure. Be aware of persons requiring assistance who access the site for the purpose of attending the Event Organizer s activities including their general location within the building. 13 CORPORATION OF THE TOWN OF GRIMSBY Evacuation of Persons Requiring Assistance - General CO-GEN-FSP-PRO PROCEDURE 5.1 Disabilities Affecting Occupant Evacuation There are many types of disabilities that may affect an occupant s ability to safely evacuate the building, including those who: Require the use of wheelchairs, walkers, crutches or canes. Have temporary conditions (i.e. recovery after surgery breaks or sprains, etc.). Have vision and/or hearing disabilities. Are pregnant. Have breathing disabilities (i.e. asthma, etc.). Have arthritis. Have hidden disabilities (i.e. epilepsy, etc.). 5.2 Identification of Persons Requiring Assistance Employees Employees with disabilities who may require assistance during an emergency evacuation of a building/facility, need to be identified in advance of an emergency occurring to ensure that an appropriate level of assistance is made available for their safe evacuation. It is also important to know what type of assistance may be required to assist these employees in safely evacuating their respective building/facility With the understanding that each employee s personal health, including any disabilities they may have, is private information, it is not mandatory for employees to divulge personal disabilities to their employer. However, employees who would like the assistance from another employee during an emergency evacuation are encouraged to volunteer information concerning their particular needs to their Manager/Supervisor so that the appropriate assistance can be provided. The Manager/Supervisor is responsible for communicating the employee s needs to the appropriate Worksite Manager. Employees will be made aware of the provisions of this procedure through the appropriate Fire Safety Plan training Employees who have requested assistance for emergency evacuations will be assigned a minimum of one Evacuation Assistant and one designated Alternate Evacuation Assistant who work in close proximity to them Employees who require assistance as well as there Evacuation Assistant(s) and Alternate Evacuation Assistant(s) must be identified on the appropriate FSP Persons Requiring Assistance List (specific to each facility 1 ). Worksite Managers are responsible for ensuring that FSP Persons Requiring Assistance Lists are kept up to date and kept with the appropriate Fire Safety Plan for their respective facility/facilities. 1 Refer to FSP Document Control Matrix (CO-GEN-FSP-LM ) for the applicable document number for each respective facility. 14 CORPORATION OF THE TOWN OF GRIMSBY Evacuation of Persons Requiring Assistance - General CO-GEN-FSP-PRO General Public/Visitors Persons requiring assistance who are members of the public or visitors may not be readily visible to employees, User Group Representatives, User Group Assistants, Event Organizer Representatives or Event Organizer Assistants All employees, User Groups Representatives, User Group Assistants, Event Organizer Representatives and Event Organizer Assistants should be aware of persons from the general public with readily visible disabilities (e.g. visual disabilities, require use of a wheelchair, etc.) that may require assistance in the event of an emergency. 5.3 Evacuation of Persons Requiring Assistance During a fire emergency evacuation (or upon hearing the fire alarm signal), previously assigned Evacuation Assistants (or Alternate Evacuation Assistants) should assist the persons requiring assistance as described in the following subsections Evacuation Assistants, Alternate Evacuation Assistants, employees, User Group Representatives, User Group Assistants, Event Organizer Representatives and Event Organizer Assistants helping persons requiring assistance, should only assist persons requiring assistance if safe to do so, taking into account their own personal safety Employees Previously assigned Evacuation Assistants (or Alternate Evacuation Assistants) should assist persons requiring assistance to the exterior of the building General Public/Visitors Employees, User Group Representatives, User Group Assistants, Event Organizer Representatives and Event Organizer Assistants should offer assistance to any person requiring who require assistance in evacuating the building Evacuation Difficulties If a person requiring assistance cannot be evacuated from the building, the Evacuation Assistants, Alternate Evacuation Assistants assigned to that person or the User Group Representative, User Group Assistant, Event Organizer Representative or Event Organizer Assistant responsible for the area the person is occupying, must notify the Emergency Coordinator immediately upon evacuating the building. Most times, fire occurs due to human error or carelessness. It is important that everyone in the workplace take responsibility for fire prevention by reporting any hazards to their Supervisor. 2.0 SCOPE This procedure applies to all facilities owned by the Corporation of the Town of Grimsby. 3.0 DEFINITIONS Worksite Manager 4.0 RESPONSIBILITIES Worksite Manager Managers/ Supervisors All Staff A member of management designated by the employer to be responsible for supervision/control of a worksite(s), including the provisions for Fire Safety. Avoid plugging in more than one extension cord into an outlet Installation or repairs to permanent wiring should be completed by a certified electrician Do not store combustible materials in service rooms or within 1 m (3 ft.) of electrical panels not located in service rooms Electrical outlets must remain clear of storage Temporary wiring is not permitted where it presents a fire hazard. 5.3 Storage Hazards Combustible storage is not permitted in service rooms (i.e. electrical rooms, etc.), service spaces, elevator shafts, ventilation shafts, stairwells, concealed spaces, (i.e. ceiling spaces, etc.), means of egress or on the roof Do not store flammable and combustible liquids or hazardous materials in storage rooms containing combustible materials Dumpsters storing combustible materials are required to be located so that they do not create a fire hazard to the building. 5.4 General Hazards Smoking is not permitted at any Town facility. The Worksite Manager is responsible for ensuring that the appropriate signage is posted Fire and stairwell doors are to remain closed at all times All exits and access to exits (i.e. hallways, corridors, aisles, etc.) are required to remain unobstructed Prevent the accumulation of lint in laundry equipment Abandoned optical fibre cables and electrical wires and cables with combustible insulation, jackets or sheaths and non-metallic raceways are required to be removed from a plenum, except where such items are permanently enclosed, removal would disturb the structure or finish of the building or where removal could affect the performance of the cables in use Do not exceed permitted occupant load. A minimum clearance of 457 mm (18 in) is required Do not hang anything from a sprinkler head Clear access to the sprinkler control valves is required at all times. 5.6 Flammable and Combustible Liquids and Hazardous Materials Do not store flammable or combustible liquids in or adjacent to exits or ignition sources Flammable and combustible liquids are required to be stored separate from dangerous goods (i.e. explosives, compressed gases, oxidizing substances, radioactive materials, etc.) Ensure that flammable and combustible liquids are distinctly marked/labelled Persons handling flammable and combustible liquids must be WHMIS trained and have access to Material Safety Data Sheets (MSDS) Remove spills immediately using an absorbent material that does not increase the hazard and dispose of such products in a safe manner. 5.7 Propane Industrial Truck Hazards Industrial trucks are required to be kept clean and reasonably free of lint, excess oil and grease Non-combustible agents are required to be used for cleaning trucks During the refuelling of the truck, the engine is required to be stopped and the operator is not permitted to be inside the truck The service valve of the fuel container is required to be closed whenever the vehicle is parked overnight or stored indoors for a protracted time. 5.8 Welding and Cutting Welding and cutting is required to be carried out in areas that are free of combustible and flammable contents and have walls, ceilings and floors comprised of non-combustible construction (or lined with non-combustible materials). Where it is not practical, combustible materials shall be kept at least 11 m from the work area or protected against ignition by sheet metal or other non-combustible material. Page 3 of 4

The production of toxic fumes in buildings makes fire fighting potentially dangerous, particularly if a large amount of smoke is being generated. 5.2 Extinguishment of a Fire Only after ensuring everyone has evacuated the area, the fire alarm system has been activated (where applicable) and the Fire Department has been notified, should a person that has been properly trained on how to use a fire extinguisher attempt to extinguish a small fire. This is a voluntary act. Never attempt to fight a fire alone. If it cannot be easily extinguished with the use of one (1) portable fire extinguisher, leave the area and confine the fire by closing the door. A member of management designated by the employer to be responsible for supervision/control of a worksite(s), including the provisions for Fire Safety. When shutting down a portion of, or the entire fire alarm, sprinkler or standpipe systems Personnel 2. When hot work is being performed at a facility under the approval of a Hot Work Permit as defined in Hot Work Procedures - General (CO-GEN-FSP-PRO ). Works which are exempt from obtaining a Hot Work Permit do not require a Fire Watch to be completed A fire watch is to consist of a minimum of one (1) supervisory building personnel (i.e. maintenance personnel, etc.) patrolling the appropriate areas for the frequency and duration referred to below Person(s) completing the fire watch should be familiar with all fire safety features in the building including information and procedures contained within the Fire Safety Plan for that particular facility Communication The person completing the fire watch must carry a form of communication (e.g. bull horn, portable two-way radios, available telephone, etc.) to inform building occupants and to contact (directly or indirectly) the St. Catharines Fire Dispatch Refer to the appropriate FSP Emergency Contact List (specific to each facility) for the appropriate contact information Records The Fire Watch Report (CO-GEN-FSP-FRM ) will be used to record all occasions where a fire watch was implemented (i.e. fire alarm, sprinkler or standpipe system shutdowns) or during cooking extinguishing system shut-downs, where emergency exits need to be blocked, where fire extinguishers are removed and where hot work is being performed Completed reports must be provided to the Worksite Manager for record keeping. 29 CORPORATION OF THE TOWN OF GRIMSBY Fire Watch Procedures - General CO-GEN-FSP-PRO Fire Watch Specifics - System Shut-Downs Frequency and Duration Patrols must be completed at least once per hour for the entire period that the system(s) is shut-down or otherwise affected Fire Watch Areas During regular business hours, patrols should concentrate on unoccupied rooms (i.e. electrical, mechanical, storage and computer rooms). After business hours, the patrol should encompass all areas affected by the shut-down. In addition, all patrols should include a patrol of the means of egress to ensure that they are kept clear. 5.3 Fire Watch Specifics - Hot Work Frequency and Duration Patrols are required to be completed for the duration of the hot work, including any break activities and for a further 60 minutes following completion of the work. During this period all affected areas need to be patrolled on a continuous basis After the 60 minute period has expired, additional patrols need to be completed for an additional 3 hours. During this period all affected areas must be inspected at least once every 30 minutes Fire Watch Areas The fire watch area should consist of the area where the hot work procedures are being performed including the surrounding 11 m (35 feet). Ensuring that all employees performing hot work are trained (including hot work operations, fire watch, etc.). Ensure staff and contractors follow the requirements of this procedure when completing hot work. Retain a copy of the hot work permits. Follow the requirements identified in this procedure for completing hot work. 5.0 PROCEDURE 5.1 Overview This procedure describes hot work procedures that are required to be followed at all Town of Grimsby facilities (both within the buildings and on the exterior) Alternate work practices should be utilized whenever possible to avoid/eliminate the use of hot work Hot work is not permitted to be completed where a facilities fire protection systems (i.e. fire alarm, sprinkler or standpipe system) are impaired. The only two exceptions permitted are: 1. During maintenance or repair of a facilities fire protection systems (i.e. fire alarm, sprinkler or standpipe system), proving that non-hot work means/practices for completing the maintenance/repairs are not practical. 2. When bypassing a component of a facility s fire alarm system (i.e. individual device or system zone) in order to ensure hot work does cause the system to activate inadvertently (i.e. nuisance alarms). Bypassing of devices should be limited to the immediate area where the hot work is being conducted. 5.2 Hot Work Permits Requirements A Hot Work Permit (CO-GEN-FSP-FRM ) must be obtained when any form of hot work is being completed at any Town facility, with the exception of the exemptions noted below Exemptions The following conditions are exempt from the requirement to obtain a Hot Work Permit (CO- GEN-FSP-FRM ): 1. The hot work is be being performed outside of a Town facility on an item not affixed to any structure, and the work is being completed more than 11 m (35 feet) away from any structure. Page 2 of 6

33 CORPORATION OF THE TOWN OF GRIMSBY Hot Work Procedures - General CO-GEN-FSP-PRO In the areas of facilities which are specifically designed to allow for hot work to be completed as part of the facilities normal operations. In order to be considered exempt the area must be: Of non-combustible and fire-resistant construction; Free of combustibles and flammables or containing combustibles or flammables providing they are appropriately protected against ignition (i.e. stored in closed fire cabinets); Appropriately segregated from adjacent areas; Equipped with appropriate fire safety equipment (e.g. fire extinguishers, fire blankets, etc.); Equipped with adequate ventilation. 3. Presently the exempted areas are: Operations Centre shop; Peach King Centre shop; Fire Station 1 apparatus bay Permit Process The following process must be followed when a hot work permit is required: 1. The on-site person who will be responsible for the hot work is required to obtain a permit from the appropriate Worksite Manager. 2. Each permit is required to be fully complete prior to being approved. Each Worksite Manager is responsible for reviewing and approving hot work permits. 3. The on-site person is responsible for posting the hot work permit in a conspicuous location where the hot work is being completed for the duration of the work. 4. The permit is valid for the length of time identified on the permit. 5. The on-site person is responsible for ensuring that the hot work procedures are followed and a fire watch is completed. 6. The on-site person is responsible for ceasing hot work operations if unsafe conditions develop and will notify the Worksite Manager. 5.3 Prohibited Hot Work Hot work is prohibited in the following conditions: 1. On pipes or other materials that are in contact with a combustible partition, wall, ceiling, roof or other combustible material if the work is close enough that the heat could travel and cause ignition (i.e. through conduction). 2. On walls, partitions, ceilings, roofs or ductwork which contain combustible materials, coverings or insulation. Page 3 of 6

On containers in which flammable liquids, solids or vapours could be present. 5.4 Alternate Measures (Prior to Hot Work) Prior to beginning hot work, alternate measures must be taken in accordance with the appropriate Alternate Measures procedure (specific to each facility). 5.5 Hot Work Precautions (Prior to Hot Work) General Precautions The following general precautions must be taken prior to beginning any hot work: 1. The work area must be inspected to ensure it is safe for hot work. 2. All cutting and welding equipment (e.g. torches, valves, hoses, etc.) must be inspected to ensure that it is in good operating condition and all cylinders must be secured to prevent them from being knocked over. 3. Adequate local exhaust and/or general ventilation are provided. 4. Any natural gas pipelines within 1 m (3 feet) of the hot work must be covered with wet non-combustible insulating materials at least 6 mm ( 1 / 4 inch) thick. 5. Appropriate fire safety equipment is available and the on-site person responsible is trained in its use and the appropriate emergency response procedures. 6. For those facilities equipped with a fire alarm system: a. If the completion of hot work could activate an initiating device (i.e. smoke detector, heat detector, etc.) and cause the fire alarm system to activate, the fire alarm system should be bypassed for the device(s) or the zone or individual devices may be bagged in order to prevent contamination and nuisance alarms. b. Where a zone of the fire alarm system is bypassed, a fire watch of the zone area should be completed in addition to the fire watch for the hot work procedures. c. If bags are installed to protect a device, the bag is required to be removed upon completion of the work or upon completion of the shift. d. The on-site person is responsible for notifying the Worksite Manager if bypassing of the fire alarm system is required. 7. If hot work is conducted within 1 m (3 feet) of a sprinkler head, precautions must be taken to prevent heat from the hot work activating the sprinkler head: a. Sprinkler heads may be protected using noncombustible sheet material or damp cloth guards. Page 4 of 6

Protective materials are required to be removed upon completion of the work or upon completion of the shift Precautions within 11 m (35 feet) The following precautions are to be taken within an 11 m (35 feet) radius of the hot work area, prior to beginning any hot work: 1. Floors must be swept clean and free of combustible materials. 2. Combustible floors (e.g. carpets, hardwood, etc.) must be protected by wetting the floor down, covering the floor with damp sand or covering the floor with non-combustible or fire-retardant shields. If wetting is being used, ensure that appropriate precautions are taken to protected against electric shock. 3. All combustible materials (e.g. flammable liquids, cardboard, etc.) must be removed or appropriately shielded against ignition with metal or fire-retardant guards or curtains. 4. Any openings or cracks in walls, floors, under locked doors, ducts or drains (e.g. floor drains) must be tightly covered with a fire-retardant or non-combustible material to prevent the passage of sparks to adjacent areas Precautions when Working On/Near Walls, Partitions and Ceilings The following precautions are to be taken prior to beginning any hot work on/near walls, partitions and ceilings: 1. Hot work must be not be completed if any of the restrictions listed in Section 5.3 are present. 2. Non-combustible or fire-retardant shields must be used where hot work is being completed near a wall, partition, ceiling or roof of comprised of combustible materials. 3. Includes Town of Grimsby, User Group or Event Organizer staff. A User Group is any group that rents or uses space on a regular basis within in the Elizabeth Street Pumphouse. Administrating the overall control of Fire Safety Plan related documentation for all facilities owned by the Town of Grimsby, including: - Authoring changes to documentation as communicated by the respective Worksite Manager for each facility. - Coordinating endorsement and approval of changes to documentation through the Town Manager, Council and the Fire Department. - Maintaining electronic copies of all documentation. - Distributing hard-copies (printed) of the documentation to the appropriate locations as identified in FSP Document Control Matrix (CO-GEN-FSP-LM ). Assisting in the overall coordination of the Town s corporate program for Fire Safety including: - Assisting in the communication of documentation to employees, Supervisory Staff User Groups and Event Organizers. - Assisting in the coordination of training for employees and Supervisory Staff. - Assisting in the coordination of fire drills. Ensuring that all contracted company personnel are familiar with the procedures to be taken during an emergency. Ensuring that all maintenance requirements for life safety systems including checks, tests and inspections are completed as prescribed within the Ontario Fire Code, O.Reg.213/07 and as identified in the Maintenance of Fire and Life Safety Systems - Elizabeth Street Pumphouse (CO-GEN-FSP-PRO ) procedure. Ensure that facility maintenance records are documented as identified in Facility Maintenance Records - Elizabeth Street Pumphouse (CO-GEN-FSP-MAN ). Ensuring that all Maintenance personnel are capable of fulfilling the maintenance requirements (checks, tests and inspections) as prescribed in the Ontario Fire Code, O.Reg.213/07 and as identified in the Maintenance of Fire and Life Safety Systems - Elizabeth Street Pumphouse (CO-GEN-FSP-PRO ) procedure. Ensuring that all signage as required throughout the building is posted and maintained in accordance with the Requirements for Posted Instructions - Elizabeth Street Pumphouse (CO-GEN-FSP-PRO ) procedure including designated fire routes, emergency posted instructions, identification of room names and fire protection equipment, operation of commercial cooking extinguishing systems, etc. Ensuring a fire drill is conducted (at least annually) with Supervisory Staff in accordance with the Fire Drill Requirements - Elizabeth Street Pumphouse (CO-GEN-FSP-PRO ) procedure. Ensuring results of fire drills are recorded on the Fire Drill Report - Facility Without Fire Alarm (CO-GEN-FSP-FRM ) form and maintained for a minimum of twelve (12) months. Ensuring that the evacuation of persons requiring assistance is addressed prior to an emergency in accordance with the Evacuation of Persons Requiring Assistance - General (CO-GEN-FSP-PRO ) procedure. Ensuring that the following information is maintained for the Elizabeth Street Pumphouse, the information is reviewed annually and required changes are communicated to the EHS Compliance Manager: - A list of Supervisory Staff for the building is documented on the FSP Supervisory Staff List - Elizabeth Street Pumphouse (CO-GEN-FSP-LM ). 49 CORPORATION OF THE TOWN OF GRIMSBY Responsibilities of Supervisory Staff - Elizabeth Street Pumphouse CO-GEN-FSP-PRO An inventory of WHMIS controlled products (i.e. hazardous materials, flammable liquids, combustible liquids, etc.) stored or used within the building is kept electronically in the MSDSonline application. The inventory is exported to the Compiled SDS Inventory Matrix (CO-GEN-OHS-DBS ) which is printed and used as an index for the SDS Binder at each facility. User Groups, Event Organizers and/or contractors are required to provide a list of all WHMIS controlled products and their corresponding SDS to the Worksite Manager. The Worksite Manager will use his/her discretion in determining which products brought in by these groups will be added to MSDSonline (e.g. products brought onsite for temporary/short durations may not be included). Regardless of whether the product is included in the MSDSonline, a hard copy of the SDS must be kept onsite for each product which is present. - A list of emergency contacts for the building is documented on the FSP Emergency Contact List - Elizabeth Street Pumphouse (CO-GEN-FSP-LM ). Practicing fire prevention in accordance with the Fire Prevention - General (CO-GEN- FSP-PRO ) procedure. Reviewing and approving where appropriate: - Any WHMIS controlled products to be brought into the building by outside parties (i.e. User Groups, Event Organizers, Contractors, etc.). - Any heat or flame producing equipment that will be brought into the building by outside parties (i.e. User Groups, Event Organizers, Contractors, etc.). - Event layout plans for any special events to be held that are outside the normal operations of the facility. - Information concerning the intended occupant loads for special events. Ensuring that all issues regarding fire hazards, unsafe equipment or equipment deficiencies reported are addressed immediately. 50 CORPORATION OF THE TOWN OF GRIMSBY Responsibilities of Supervisory Staff - Elizabeth Street Pumphouse CO-GEN-FSP-PRO Recreation Program Coordinator (Facility Rental Representative) The Recreation Program Coordinator is responsible for: Ensuring that the appropriate Fire Safety Plan documents are available to User Groups and Event Organizers so they in turn can ensure that their representatives are familiar with their respective duties and responsibilities in accordance with the FSP Competencies Matrix - Elizabeth Street Pumphouse (CO-GEN-FSP-LM ) document. Ensuring that all Event Organizers and User Groups sign the FSP Facility Rental Agreement - General (CO-GEN-FSP-FRM ) which outlines their responsibilities for fire safety while using the facility. Ensuring that Event Organizers and User Groups provide appropriate emergency contact information and list of designated Event Organizer/User Group Representatives as maybe required. The level of information requested will be at the discretion of the Recreation Program Coordinator based on the size/scale of the event and indented use of the facility. Ensuring that all User Groups and Event Organizers have a Representative assigned while on site and are instructed on the emergency evacuation procedures. Ensuring that User Groups who intend to hold any special events which are outside the normal use of the facility follow the appropriate Fire Safety Plan requirements as an Event Organizer in these circumstances. Ensuring that Event Organizers provide the appropriate information for any events intended to be held which are outside the normal use of the facility (e.g. trade shows, concerts, etc.) and ensuring this information is communicated to the Manager Parks and Facilities and the Grimsby Fire Department for their approval. Information will include: - An event layout plan showing the general layout of the facility during the event (e.g. table configurations for banquets, stall locations for trades shows, stage and seating configurations for concerts, etc.) - Information concerning the intended occupant loads for each event. Ensuring that when the User Group is on site at least one (1) person is designated as the User Group Representative. Ensuring that the User Group Representative(s) and all User Group Assistants understand their duties and responsibilities for fire safety in accordance with the FSP Competencies Matrix - Elizabeth Street Pumphouse (CO-GEN-FSP-LM ). Ensuring that the User Group Representative(s) and all User Group Assistants are instructed on evacuation procedures for the building in accordance with the Fire Related Emergency Procedures - Elizabeth Street Pumphouse (CO-GEN-FSP-PRO ) document. Ensuring that the User Group Representatives and all User Group Assistants are fulfilling their responsibilities with respect to Fire Safety. Notifying the Recreation Program Coordinator if the User Group intends to hold any special events which are outside the normal use of the facility and following the Fire Safety Plan requirements as an Event Organizer in these circumstances. In addition, this person will restrict access to the building. An Event Organizer is any individual, group or corporation that rents or uses space in the Elizabeth Street Pumphouse and is not considered a User Group (i.e. dances, banquets, weddings, meetings, etc.). Event Organizer personnel are divided into three categories: Event Organizer Administrator - is a person affiliated with the organization coordinating the special event (i.e. president, board member, etc.) and is responsible for ensuring that the requirements of the fire safety plan are being met by the Event Organizer while occupying a Town facility. Event Organizer Representative(s) - is a person/persons appointed by the Event Organizer who is responsible for leading the evacuation of the area of a facility occupied by the Event Organizer during a special event. Event Organizer Assistants - are persons appointed by the Event Organizer who are responsible for assisting in the evacuation of the area of a facility occupied by the Event Organizer during a special event. Event Organizer Assistants could be security, sound/lighting technicians, wait staff, bartenders or any other persons responsible for event participants or occupants. Page 1 of 8

Includes Town of Grimsby, User Group or Event Organizer staff. A User Group is any group that rents or uses space on a regular basis within in the Elizabeth Street Pumphouse. User Group personnel are divided into three categories: 4.0 RESPONSIBILITIES Manager Parks and Facilities (Worksite Manager) Managers/ Supervisors All Staff User Group Representatives & User Group Assistants Event Organizer Representatives & Event Organizer Assistants Contractors User Group Administrator - is a person who is responsible for the overall administration of the User Group (i.e. president, board member, etc.) and is responsible for ensuring that the User Group is meeting the requirements of the fire safety plan while occupying a Town facility. User Group Representative(s) - is a person/persons appointed by the User Group Administrator who is responsible for leading the evacuation of the area of a facility occupied by the User Group. User Group Assistants - are persons appointed by the User Group who are responsible for assisting in the evacuation of the area of a facility occupied by the User Group. User Group Assistants could be officials, referees, coaches or any other persons responsible for User Group participants or occupants. Immediately leave the fire area, notifying any persons in the immediate area. 2. Ensure that doors to the fire area are closed. 3. If safe to do so, perform your emergency duties as defined in the Emergency Procedures section of this plan. 4. Use the nearest exterior exit to evacuate the building. If you encounter smoke or fire, use an alternate exit. 5. Call the Fire Department (dial 911) from a safe location. Advise them of building address and location of the fire (i.e. Elizabeth Street Pumphouse, 447 Elizabeth Street). 6. Remain calm. 7. Proceed to the designated assembly location. 8. Do not return to the building until the Fire Department has declared the all clear Upon Being Notified of a Fire Emergency The following procedures should be followed upon being notified of a fire emergency: 1. Fulfil your emergency duties (if safe to do so). 2. Leave the building by the nearest exterior exit. 3. Close all doors behind you. 4. Proceed to the designated assembly area. 5. Remain calm. 6. Before opening any door, test the door for heat using the back of your hand against the door. Look for smoke around the edges of the door. 2. If the door is hot or smoke is seen around the edges of the door, leave the door closed and unlocked. a. If you are in a room or office with no other exit, remain where you are and call the Fire Department at 911, identifying your location. b. Otherwise use an alternate exit to evacuate the area. 3. If the door is not hot, brace yourself against the door, opening slightly. If you feel a hot draft or air pressure, close the door immediately, leaving it unlocked. 4. If the door is not hot and you did not feel a hot draft or air pressure, proceed to the nearest exit and evacuate the building Defending in Place The following procedure should be followed in the event of fire, where no additional exit is available, you are unable to use an alternate exit or you are trapped: 1. Return to a safe location and close the door, leaving it unlocked. 2. Call the Fire Department at 911 and inform them of your location. 3. Seal off all openings which may admit smoke (i.e. door edges, vents, etc.). 4. If smoke enters the rooms, crouch low to the floor. 5. Wait for Fire Department arrival and listen for instructions. 6. 61 CORPORATION OF THE TOWN OF GRIMSBY Fire Related Emergency Procedures - Elizabeth Street Pumphouse CO-GEN-FSP-PRO Responsibility Specific Emergency Procedures - Supervisory Staff The following subsections describe the specific responsibilities for various Supervisory Staff, upon them being made aware of fire emergency Emergency Coordinator (User Group or Event Organizer Representative) In the event of being notified of a fire emergency, the Emergency Coordinator (providing it is safe to do so) will: 1. Ensure that the Fire Department has been contacted at 911 and that they will attend the fire department response point. 2. Lead the evacuation of the area occupied by the User Group/Event Organizer by immediately ceasing all User Group/Event Organizer activities and requesting participants (i.e. general public, etc.) in the User Group/Event Organizer area to evacuate the building. 3. Request help from User Group/Event Organizer Assistants to assist in the evacuation process (including checking areas for occupants, helping persons requiring assistance, directing occupants to exit the building, etc.). 4. Attend the main entrance to the building. 5. Meet the Fire Department at the primary response point, providing information regarding the fire emergency. 6. Coordinate the evacuation of occupants. 7. Receive reports from User Group Assistant(s) or the Event Organizer Assistant(s). Status reports should include the following information: a. If the observed area was clear; b. If any persons were left in the building; c. If any persons were injured; or d. The location of where fire and/or smoke was seen. 1. Ensure that access to the building is restricted so that no persons can re-enter the building until the Fire Department has declared the all clear. 2. Ensure that occupants are located away from the building and are not blocking access routes and equipment. 3. Relay all information regarding the emergency to the Fire Department including information provided by User Representatives or Event Organizers and occupants such as the location of injured persons, location of fire and smoke, etc. 4. Notify the Worksite Manager for the facility. Page 5 of 8

Permit occupants to re-enter the building (when advised safe to do so). 2. Investigate the cause of the fire emergency complete the Fire Incident Report - General (CO-GEN-FSP-FRM ), once the Chief Fire Official has completed their investigation. 3. Coordinate the clean-up, taking any photographs of the damage prior to clean-up. 4. Follow instructions from the Fire Department User Group Assistants (Persons Responsible for User Group Participants or Occupants) In the event of being notified of a fire emergency, providing it is safe to do so, User Group Assistants will: 1. Assist participants in the area occupied by the User Group with emergency evacuation by immediately ceasing all User Group activities and requesting participants (i.e. general public, etc.) to immediately evacuate the area. 2. Check the area occupied by the User Group to ensure that the participants and occupants are evacuating the building (including washrooms, etc.). 3. Offer evacuation help to any persons requiring assistance who are in the area occupied by the User Group. If persons requiring assistance cannot be evacuated from the building, report this information directly to the Emergency Coordinator after exiting. 4. Report to the Emergency Coordinator (located at the main entrance) if your area was clear of occupants or: a. If anyone was left inside the building and their approximate location. b. If any persons were injured. c. If fire and/or smoke was seen (i.e. identify the location). 5. Evacuate the building using the nearest exterior exit. 6. Attend the designated assembly area. 7. Assist participants in the area occupied by the Event Organizer with emergency evacuation by immediately ceasing all Event activities and requesting participants to immediately evacuate the area. Any sound/lighting systems should be turned off and the building lighting system should be returned to normal lighting. 2. Check the area occupied by the Event Organizer to ensure that participants or occupants are evacuating the building (including washrooms, etc.). 3. Offer evacuation assistance to any persons requiring assistance who are in the area occupied by the Event Organizer. If persons requiring assistance cannot be evacuated from the building, report this information directly to the Emergency Coordinator after exiting. 4. Evacuate the building using the nearest exterior exit. 5. Attend the designated assembly area. 6. Follow instructions from the Fire Department. 7. Do not return to the building until the Fire Department has given the all clear User Group or Event Organizers Involving Children Programs In the event of being notified of a fire emergency, Children Program Leaders (providing it is safe to do so) will: 1. Immediately begin to evacuate all children in the program area. The children should form a line with one Leader at the front to lead children from the building and one Leader at the rear of the line to ensure that all children follow. Children and Program Leaders will not stop to obtain jackets, shoes, extra clothing, etc. during the evacuation. 2. Ensure that a search is conducted of the program area to ensure that no children have been left behind. 3. Bring the attendance records with you to the designated assembly location. 4. Conduct a head count of children in the program using the attendance records. 5. Report to the Emergency Coordinator (located at the main entrance of the building) that all children have been accounted for or if any children are missing/not accounted for. 6. Any operation involving open flames or producing heat or sparks on the site location for the facility. Hot work includes (but not limited to) welding, brazing, cutting, grinding, soldering, thawing pipe, torch-applied roofing, etc. For the purpose of this procedure, Supervisory Staff are any staff that are responsible for the day-to-day administrative duties for fire safety at a specific facility and also have specific responsibilities in responding to fire-related emergencies. Interruption of Other Life and Fire Safety Systems: When other life and fire safety systems (i.e. fire extinguishers or emergency exits) at/within a building are being temporarily compromised, the requirements of Section 5.2 need to be followed. 2. Hot Work: When hot work is being performed at/within a building, the requirements of Section 5.3 need to be followed. 5.2 Interruption of Other Life and Fire Safety Systems Fire Extinguishers If a fire extinguisher is removed for servicing, it must be replaced with a temporary fire extinguisher meeting the same type and rating requirement Exits If an exit is blocked or deemed unavailable for use (i.e. during construction, maintenance, etc.) existing exit signs are to be covered so as not to be confused as an available exit. Also, temporary signage must be installed clearly identifying the nearest exits Temporary emergency procedures should be prepared and provided to Supervisory Staff and building occupants Any changes to exits shall be approved by the Fire Chief (Chief Fire Official) or designate in advance. 5.3 Hot Work Hot Work must be completed in accordance with the Hot Work Procedures - General (CO- GEN-FSP-PRO ) document A hot work permit is must be obtained before any hot work is completed at a Town facility. Occupant Load Event Organizer Assistants - are persons appointed by the Event Organizer who are responsible for assisting in the evacuation of the area of a facility occupied by the Event Organizer during a special event. Event Organizer Assistants could be security, sound/lighting technicians, wait staff, bartenders or any other persons responsible for event participants or occupants. B of the Ontario Fire Code, O.Reg.213/07, the Fire Drill procedures must be prepared in consultation with the Chief Fire Official Supervisory Staff and Occupant Participation Supervisory staff are required to participate in all fire drills Occupants should be encouraged to participate in fire drills to in order for them to become familiar with their primary and secondary emergency evacuation routes, become more familiar with the emergency procedures and to assess their preparedness and response Fire Incidents Fire Officials may consider a fire incident to be a fire drill. 89 FIRE SAFETY PLAN FACILITY RENTAL AGREEMENT Definitions: Event Organizer: Any individuals, groups or corporations that rent or use space within a Town of Grimsby facility. User Group: Any individuals, groups or corporations that rent or use space within a Town of Grimsby facility on a regular basis. General Responsibilities: All Event Organizers/User Groups are required to comply with fire safety responsibilities and evacuation procedures identified in this agreement and the approved Fire Safety Plan for their respective facility. Event Organizer/User Group Administrators are responsible for signing this agreement (see page 2) on behalf of their respective organization. Event Organizer/User Group Administrators are responsible for reviewing the appropriate sections of the Fire Safety Plan for the respective Town of Grimsby facility. A copy of each Fire Safety Plan is available on the Town of Grimsby s website at Fire Safety Responsibilities: Assigning an Event Organizer/User Group Administrator who is responsible for the overall administration of the Event Organizer/User Group and is responsible for ensuring that the Event Organizer/User Group is meeting the requirements of the Fire Safety Plan while occupying a Town of Grimsby facility. Ensuring that when on site one (1) person is designated as the Event Organizer/User Group Representative to be responsible for leading the emergency evacuation of the area occupied by the Event Organizer/User Group and ensuring the Representative is instructed on the applicable emergency evacuation procedures for the facility. Ensuring that all Event Organizer/User Group Assistants are instructed on evacuation procedures for the facility. Notifying the Rental Representative for the Town of Grimsby when the Event Organizer/User Group intends to use a portion of or the facility for an event outside of the normal use for the facility. When requested by the Rental Representative, providing an event layout plan which meets the requirements of the Grimsby Fire Department and ensuring that the event layout is implemented in accordance with the approved event layout plan. Ensuring that an appropriate staff/volunteer-to-attendee ratio is maintained so that persons can safely execute their duties. For those events where the number of attendees is unknown prior to the event (i.e. trade shows, large dances, etc.), ensure that the occupants load is not exceeded by maintaining a count of persons entering and exiting the event area. Providing a list of any WHMIS controlled products which will be brought into the building (i.e. hazardous materials, flammable liquids, combustible liquids, etc.), to the Worksite Manager for his/her approval. Providing the appropriate MSDS for any WHMIS controlled products and ensuring they are stored onsite for the duration that the product is present. Providing a list of any heat or flame producing equipment which will be brought into the building, to the Worksite Manager and the Grimsby Fire Department for their approval. Practicing fire prevention and ensuring that all issues regarding fire hazards, unsafe equipment or equipment deficiencies are reported immediately. Knowing your duties and responsibilities with respect to fire safety. Knowing the following: - The sound of the fire alarm signal or how to notify occupants of a fire emergency. - The location of exit routes. - The location of the designated assembly area. 90 Evacuation Procedures: In the event of a fire emergency, the designated Event Organizer/User Group Representative is responsible for leading the evacuation of the area occupied by the Event Organizer/User Group. Event Organizer/User Group Assistants are responsible for assisting participants in the area occupied by the Event Organizer/User Group with emergency evacuation. The following steps will be followed, where it is safe to do so: Immediately cease all activities and request all participants or occupants (i.e. vendors, spectators, visitors, etc.) in your area to evacuate the building. Any sound/lighting systems should be turned off and the building s lighting system returned to normal. All food preparation and serving should stop and all cooking equipment should be turned off. Building occupants should not be permitted to go to dressing rooms, coat check-in, etc. to obtain personal items. Check the area, including dressing rooms, washrooms, etc. to ensure that all attendees and occupants in your area are evacuating the building. Provide assistance to any persons in your area who require assistance to evacuate the building. If persons requiring assistance cannot be evacuated from the building, report this information directly to the Emergency Coordinator (located at the main entrance) after exiting. Evacuate the building using the nearest exit. Call 911 from a safe location. The designated Event Organizer/User Group Representative will report to the Emergency Coordinator (located at the main entrance) if your area was clear of occupants or: a. If anyone was left inside the building and their approximate location. b. If any persons were injured. c. If fire and/or smoke was seen and where. Instruct all vacated building occupants to move away from the building to the designated assembly area while the fire alarm is being investigated. For large events (i.e. trade shows, dances, flea markets, festivals, etc.), the Event Organizer/User Group should designate one (1) additional person to restrict vehicle access to the fire route other than emergency responders. Attend the designated assembly area. Do not return to the building until the Fire Department has given the all clear. By signing below, the Event Organizer/User Group agrees to implement the requirements identified in the applicable Fire Safety Plan and as identified in this agreement.
