A declaration of CE conformity from the manufacturer. 2. An electrical safety test (with report) on the unit On-site acceptance test procedures & results. 4. Two copies of the service manual. 5. Two copies of the user s manual. 1 Note that the electrical safety test must be compliant to IEC/EN or later. The test s report must be accompanied by the calibration certificate of the corresponding Electrical Safety Tester. The calibration period on the certificate must cover the date when the test was performed on the unit in question. Note also that any repairs carried out on the unit while it is covered by warranty or service contract must also be accompanied by an electrical safety test.

6 If the Electrical Safety Tester is re-calibrated, a copy of the new certificate must be provided with the next test report supplied. Note also that service training for a Saint James In-house Engineer must be included in the offer. Further Instructions to Suppliers: Suppliers are kindly requested to include the following information in their quote: Full name of the company Address of the company Full name of the contact person Telephone/Fax number and if possible, Mobile number address VAT number Quotation date Quotation reference number Estimated time of delivery Literature / Brochures, including the full technical data and specifications Please note that quotations should ideally be in the Euro Currency. Customs duty, Insurance, and VAT are to be quoted separately. Quotations must be valid for a minimum of 90 days. Suppliers must conform to CE standards and any applicable local legislation and thus, shall be bound to submit the declaration of conformity upon request. Suppliers may seek clarifications on any of the above until one working day prior to the deadline for submission of the quotation. Quotes may be either: Sent by on purchasing@stjameshospital.com. Sent by FAX on FAX number Brought personally to the Purchasing Officer from Monday to Friday between 09:00 to 14:00. The deadline for submission is Thursday 3 rd February Any offers submitted after this date will be rejected. Note that the Saint James Hospital Group management reserves the right to decline all offers even if an offer proves to be the most advantageous from the ones submitted.
