1 Annex Laundry and Shower Alterations ADDENDUM #1 March 7, 2019 ADDENDUM #1: MODIFICATIONS AND ADDITIONS TO CONTRACT DOCUMENTS A. Invitation to Bid - Call for Bids has been modified as follows: 1. Notice is hereby given that St. Vincent de Paul of Lane County will receive sealed bids Tuesday March 5, 2019 until Friday March 15, 2019 until the Bid Closing time of 2:00 p.m. Bids may NOT be submitted electronically. to the Project Manager, Drew Sizemore at drew.sizemore@svdp.us. Bids and attached bid security must be delivered to the Reception Desk at St. Vincent de Paul Headquarters located at 2890 Chad Drive Eugene, OR by the bid closing time. The bid opening will be at 2:05 p.m. on Friday, March 15, 2019, at which time those bids will be opened and read. B. Bid Form 1. A mandatory Bid Form has been issued and is part of this addendum. All bids must be signed and provided on this form. C. Architecture Project Manual 1. Specification Section Summary: a. Revise 1.1.A Prevailing Wage Requirements as follows: This project is subject to payment of Residential Davis Bacon prevailing rate of wages Oregon Prevailing Wage Rates, per BOLI ORS 279C.800 to 279C.870. No bid will be received or considered unless the bid contains a statement by the bidder that the provisions of ORS 279C.800 to 279C.870 pertaining to prevailing wage rates are complied with. 2. Specification section : a. 3 Annex Laundry & Shower Room Alterations Bid Opening Time/Date: 2:00pm, March 15, 2019 I, the undersigned, and authorized representative of (Company Name) CCB No. : Acknowledgement of terms and conditions and specifications 1. The bidder represents that bidder is properly licensed and adequately experienced, equipped, organized and financed to furnish and perform services required. 2. The bidder has reviewed the Contract Documents (Plans and Specifications dated 2/25/19) and prepared a complete bid that includes the entire scope of work as described. 3. The bidder acknowledges receipt of the following Addenda No(s) and agrees that all addenda issued are part of the Contract Documents and have been considered in preparing this bid. (Bidder insert the number of each addendum receipted; if no addenda were received, write None in the space) 4. The bidder agrees that if this bid is accepted it will, within five days after having received the Notice of Award, execute and deliver to the Owner the Performance Bond, Payment Bond and Evidence of Insurance. Completion / Delivery Date The bidder agrees, if awarded the contract to fully complete the work by June 15, 2019 Oregon Prevailing Wage This project is subject to payment of prevailing rate of wages, per BOLI & ORS 279C.800 to 279C.870. The contractor and every subcontractor on the project shall pay at a minimum the Oregon prevailing wage rate as specified for this project. The contractor certifies compliance with the Oregon PWR concerning the payment and certification of payment of prevailing wage as provided in ORS 297C.800 to 279C.870. BASE BID The undersigned proposes to furnish all material, equipment, and labor required for the complete project, and to perform all work in strict accordance with the Contract Documents for the lump sum prices indicated below. Residency Bidder is is not an Oregon Resident Bidder. An Oregon Resident Bidder, as defined by ORS 279A.120, means a bidder that has paid unemployment taxes or income taxes in this state during the 12 calendar months immediately preceding submission of the bid, and has a business address in this state. If bidder is not an Oregon Resident Bidder, in the field provided indicate the bidder s (Business) State of Residence. DESCRIPTION BID AMOUNT Provide all materials and labor as outlined in the project manual and drawings for all work. Base Bid amount not to include details on the drawings designated as an Alternate Total Base Bid $

4 Performance Bond Cost A performance bond in an amount equal to the full contract price conditioned on the faithful performance of the contract in accordance with the plans, specifications and conditions of the contract. (to be added to base bid) $ Payment Bond Cost - A payment bond in an amount equal to the full contract price (to be added to base bid) $ Bid Security A security of ten percent of the base bid. To be returned to all unsuccessful bidders and for the successful bidder upon execution of the contract and delivery of a good and sufficient performance bond, a good and sufficient payment bond and required proof of insurance. (to be added to base bid) $ ATTACHMENTS Bidder s may include with bid form any attachments to the extent necessary to clarify Bidder s scope of work. 5 Section Fire Extinguishers Page 1 SECTION FIRE EXTINGUISHERS and ACCESSORIES PART 1 - GENERAL 1.1 SUMMARY A. Section includes: 1. Portable, hand-carried fire extinguishers 2. Semi-recessed cabinets 1.2 SUBMITTALS A. Product Data: For each type of product indicated. 1.3 QUALITY ASSURANCE A. NFPA Compliance: Fabricate and label fire extinguishers to comply with NFPA 10, "Portable Fire Extinguishers." B. Fire Extinguishers: Listed and labeled for type, rating, and classification by an independent testing agency acceptable to authorities having jurisdiction. C. Coordinate type and capacity of fire extinguishers with fire protection cabinets to ensure fit and function. D. Coordinate size of fire protection cabinets to ensure that type and capacity of fire extinguishers indicated are accommodated. E. Coordinate sizes and locations of fire protection cabinets with wall depths. F. Where located in rated wall construction, wrap opening with 5/8 in, Type X Gyp. Bd. prior to installing non-rated cabinet. Coordinate rough opening sizes with added layers of Gyp. Bd. 1.4 WARRANTY A. Special Warranty: Manufacturer's standard form in which manufacturer agrees to repair or replace fire extinguishers that fail in materials or workmanship within specified warranty period. 1. Failures include, but are not limited to, the following: a. Failure of hydrostatic test according to NFPA 10. b. Faulty operation of valves or release levers. 2. Warranty Period: Six years from date of Substantial Completion Annex

6 Section Fire Extinguishers Page 2 PART 2 - PRODUCTS 2.1 PORTABLE, HAND-CARRIED FIRE EXTINGUISHERS A. Fire Extinguishers: Type, size, and capacity for each fire protection cabinet. 1. Basis-of-Design Product: Subject to compliance with requirements, provide MP 5 by Larsen s or comparable product by one of the following: a. J. L. Industries, Inc.; a division of Activar Construction Products Group. b. Kidde Residential and Commercial Division; Subsidiary of Kidde plc. 2. Instruction Labels: Include pictorial marking system complying with NFPA 10, Appendix B. B. Multipurpose Dry-Chemical Type: UL-rated with monoammonium phosphate-based dry chemical in manufacturer's standard enameled container. 1. See Drawing Sheets for locations 2. FE-1: 3A-10B:C 5 lb (with cabinet) C. Locations: 1. Laundry Room 2. And as required by local Fire Marshal. 2.2 CABINETS A. Cabinet Type: Suitable for fire extinguisher. 1. Products: Subject to compliance with requirements, provide one of the following: a. J. L. Industries, Inc., a division of Activar Construction Products Group; b. Kidde Residential and Commercial Division, Subsidiary of Kidde plc. c. Larsen's Manufacturing Company B. Cabinet Construction: 1. Fire-Rated Cabinets: Construct fire-rated cabinets with double walls fabricated from inchthick, cold-rolled steel sheet lined with minimum 5/8-inch thick, fire-barrier material. Provide factory-drilled mounting holes. A. Cabinet Material: 1. Cold-Rolled Steel Sheet: ASTM A 1008/A 1008M, Commercial Steel (CS), Type B. 2. Transparent Acrylic Sheet: ASTM D 4802, Category A-1 (cell-cast sheet), 3 mm thick, with. B. Semi-recessed Cabinet: Cabinet box partially recessed in walls of sufficient depth to suit style of trim indicated; with one-piece combination trim and perimeter door frame overlapping surrounding wall surface with exposed trim face and wall return at outer edge (backbend). Provide where walls are of insufficient depth for recessed cabinets but are of sufficient depth to accommodate semi-recessed cabinet installation. Rolled-Edge Trim: 2-1/2-inch backbend depth. C. Cabinet Trim Material: Same material and finish as door. 1. Door Material: Steel sheet. 2. Door Style: Fully glazed panel with frame. 3. Door Glazing: Clear Acrylic sheet Annex

7 Section Fire Extinguishers Page 3 D. Door Hardware: Manufacturer's standard door-operating hardware of proper type for cabinet type, trim style, and door material and style indicated. 1. Door Lock: Cam lock that allows door to be opened during emergency by pulling sharply on door handle. E. Accessories: 1. Mounting Bracket: Manufacturer's standard steel, designed to secure fire extinguisher to fire protection cabinet, of sizes required for types and capacities of fire extinguishers indicated, with plated or baked-enamel finish. 2. Break-Glass Strike: Manufacturer's standard metal strike, complete with chain and mounting clip, secured to cabinet. 3. Identification: Lettering complying with authorities having jurisdiction for letter style, size, spacing, and location. Locate as indicated. a. Identify fire extinguisher in fire protection cabinet with the words "FIRE EXTINGUISHER." 1) Location: Applied to cabinet door. 2) Application Process: Pressure-sensitive vinyl letters. 3) Lettering Color: Red. 4) Orientation: Vertical. 4. Finishes: Manufacturer's standard baked-enamel paint. PART 3 - EXECUTION 3.1 INSTALLATION A. Examine fire extinguishers for proper charging and tagging. Remove and replace damaged, defective, or undercharged fire extinguishers. B. Install fire extinguishers in locations indicated and in compliance with requirements of authorities having jurisdiction.
