3 About SIRIM QAS INTERNATIONAL Established in 1996 as a wholly-owned subsidiary of SIRIM Berhad SIRIM BERHAD is a wholly-owned company of the Malaysian Government under the Ministry of Finance Incorporated Have been providing conformity assessment services since the 1970s when these services were provided as a division of SIRIM Malaysian based, international accreditation and recognition for Quality Management System, Product Certification and Testing Services. Local and international clients in countries such as P.R. China,Thailand, Singapore, Brunei, Indonesia, Vietnam, Myanmar, Korea, Philippines, Taiwan, Japan, India, Sri Lanka, Australia, New Zealand, Germany, UAE and Spain.

5 Personal Protective Equipment (PPE) The personal protective equipment (PPE) is any equipment which is intended to be worn or held by a person at work which protects him/her against one or more risks to health or safety and any additional accessory designed to meet that objective. The principle of PPE in prevention of injuries is to avoid contact between the hazards and the external or internal parts of the body that is to be protected. PPE can be categorized according to the risk and parts of the body to be protected.

13 Standards for PPE Standard for each product can be found in Appendix 1 and Appendix 2. However, importer/manufacturer is allowed to use other suitable standards as long as the requirements meet the intended purpose to ensure safety of the user. The standard shall be accepted by DOSH

24 Route to DOSH-SIRIM PPE Approval FACTORY AUDIT AND SAMPLING Inspection conducted to examine product details, QC plan adequacy, test equipment adequacy and calibration, quality system and record-keeping system Samples selection and testing INITIAL TYPE TESTING APPROVAL PROCESS & LICENCE DOSH APPROVAL Type Testing in SIRIM QAS International lab (Type Testing to be witnessed by DOSH officer) OR Type Testing at factory lab or 3rd party lab or an accredited lab to ISO/IEC supervised by SIRIM Tester and to be witnessed by DOSH Officer Any 3 rd part lab or an accredited lab shall AGREE to allow Type Testing to be supervised by SIRIM Tester and to be witnessed by DOSH Officer Certification Report preparation and recommendation to Certification Panel Approval by Certification Panel The license issued by SIRIM QAS International will be used by the Manufacturers / Importers for approval from DOSH Apply for DOSH Approval (Refer to Route to DOSH Approval) Addition of new model similar to Initial Type Test (for DOSH Approval: Test Report No. to be reflected in the SIRIM Product Certification Licence)

25 Route to DOSH-SIRIM PPE Approval SURVEILLANCE & RE-EVALUATION TESTING Planned inspections and re-tests conducted to monitor continuing compliance Re-evaluation test for models in license every 5 years APPLICATION for SIRIM LABEL SIRIM Label will be approved for 3 months production quantity RENEWAL Approval for renewal after satisfactory surveillance audit, re-evaluation test and payment of fees.

26 Route to DOSH-SIRIM PPE Approval APPLY FOR DOSH APPROVAL (Applicant) LIST UNDER DOSH APPROVED PPE (DOSH) Document to be submitted; 1) Full Type Test Report 2) Product Certification License Approval for each model Validity = 5 years (according to validity of test report = 5 years) SIRIM will notify DOSH if there is any withdrawal or termination of license PRODUCT ON THE MARKET (Licensee/ Importer) Products shall bear SIRIM Label on the product/packaging (Point of Sale) RENEWAL OF DOSH APPROVAL (Licensee/ Importer) Upon expiry of DOSH approval and Valid Type Test Report/Product Certification Licensee

27 Requirement of PPE Approval for existing SIRIM licensee under the DOSH-SIRIM PPE APPROVAL Scheme EXISTING SIRIM LICENSEE WITH VALID TYPE TEST REPORT (WITHIN 5 YEARS) THE TYPE TEST REPORT WILL BE VALIDATED BY DOSH THROUGH VERIFICATION OF TEST DURING SURVEILLANCE PROGGRAMME VALIDITY OF APPROVAL IS BASED ON THE REMAINING YEARS FROM THE PPE APPROVAL APPLICATION DATE TO THE DATE OF TYPE TEST REPORT

Products bearing the Product Certification/Safety Mark enhances the reputation of manufacturer and thus, extends market and facilitates acceptance locally and internationally. 35
