Students should not operate at a national laboratory due to variety of tasks we do here. It is difficult to track the student activities. 6

7 Equipment & Controls Available equipment and laser facility configuration: these are good and are effective for reducing risk of a laser injury incident. 94% Strongly Agree or Agree, 6% Neutral, < 1% Disagree or Strongly Disagree Engineering Controls (such as interlocks, safety shutters, barriers and Class 1 enclosures): these are well done and are effective for reducing risk of a laser injury incident. 95% Strongly Agree or Agree, 4% Neutral, < 1% Disagree or Strongly Disagree Correct LPE for different modes was mentioned by several people as being a concern. 7

8 SOPs Standard Operating Procedures (and other related laser safety documents) are used to document laser system hazards and controls. SOPs (and related laser safety documents) accomplish these goals well; no significant improvements are needed to reduce risk of a laser injury incident. 90% Strongly Agree or Agree, 7% Neutral, < 2% Disagree or Strongly Disagree SOPs (and related laser safety documents) are easy to understand and use. 82% Strongly Agree or Agree, 15% Neutral, 3% Disagree or Strongly Disagree Laser workers perform their tasks safely, adhering to safe practices described in the site-specific training(ojt) and the SOP document 92% Strongly Agree or Agree, 7% Neutral, 1% Disagree or Strongly Disagree Several responses that SOPs could be shorter. 8

9 Laser Eyewear Laser eyewear requirements and available laser eyewear. These are implemented well and are effective, and it is easy to comply with the laser eyewear requirements; no significant improvements are needed to reduce risk of a laser injury incident. 91% Strongly Agree or Agree, 7% Neutral, 2% Disagree or Strongly Disagree There are situations, particularly with multi-wavelength laser systems, where completely safe eyewear can impede effective alignment work. There could be improvements for areas that operate in multiple modes and require a variety of eyewear. There is the potential for mistakes is someone is not paying attention and grabs the wrong eyewear. 9

10 Laser Eyewear Laser Eyewear Practices: Laser eyewear requirements are always adhered to. During the last 12 months, there were no cases of laser workers intentionally or mistakenly violating a laser eyewear requirement. 88% Strongly Agree or Agree, 10% Neutral, 2% Disagree or Strongly Disagree Eyewear requirements are generally emphasized by supervisors at the individual PI level. However, especially at a user facility, this is not always followed by postdocs, students, and users (although postdocs have gotten much better in recent years). Accidental cases that violated administrative practices, no real hazards present. Laser Eyewear Practices: If a laser eyewear requirement was observed to be violated in the last 12 months, whether intentional or by mistake, estimate the potential for a hazardous exposure of the most severe violation observed. 85% No violation observed, 7% No plausible injury, 6% Very low possibility, 1% Possibility >2% 10

11 Peeking Laser Eyewear peeking: Peeking occurs when eyewear is required, but the worker looks over the top or under the bottom of the eyewear to perform a task. What are reasons an individual might want to peek? 65% Eyewear is too dark to see through 46% Individual believes to know where all the laser beam paths are 64% Detailed vision is required 17% Have seen others do this 15% Other reason 11

12 Peeking Laser Eyewear peeking: Have you witnessed someone peek at your DOE site within the past 12 months? 12% Yes, 81% No, 6% Unsure No, not at higher powers Looking at computer screen located in area that required goggles, but not in the vicinity of the experimental setup. A new employee It is sometimes also important to see color when aligning OPA's which is hard with some of our goggles. Never in a situation that was at all dangerous Laser Eyewear peeking: Have you peeked within the last year? 10% Yes, 90% No I had a complete knowledge of the beam path, and also the laser beam was of low power. At location that is away from laser but still in the eyewear zone. Just to pick up tools. 12

Lab policy not consistent on this actually. 14

15 Laser Safety Perceptions For the laser facilities in which you work, you perceive laser safety more as an integral part of laser operations than as an added burden. 61% Strongly Agree, 30% Agree, 7% Neutral, < 2% Disagree or Strongly Disagree Oversight and assistance by the Laser Safety Officer are effective and well done for helping reduce risk of a laser injury incident. 88% Strongly Agree or Agree, 9% Neutral, 2% Disagree or Strongly Disagree 15

17 Near Misses/Lessons Learned A Near Miss occurs when safety is compromised such that only one or no barriers are in place to prevent a laser eye injury. 18 Undue Burdens/Unnecessary Controls Laser safety requirements are appropriate and do not UNDULY impede meeting goals and schedule. 89% Strongly Agree or Agree, 7% Neutral, 3% Disagree, <1% Strongly Disagree The safety rules are fine. But the tracking, reporting, training, paperwork, etc. are overly confusing and burdensome. The laser safety requirements themselves do not unduly impede meeting goals and schedule, but the enforced implementation of those requirements may unduly impede meeting goals and schedules. 19 Undue Burdens/Unnecessary Controls Provide comments about any controls required by your DOE Facility's laser safety program that you think are unnecessary. Excessive redundancy of engineering controls. Overly strict LSO The problem is not the controls, but how quickly line management and DOE amplifies an incident. It shifts the focus away from mitigation and to managing the potential hysteria. The laser-key requirement is silly. No one has ever in my career attempted to start up a laser that they were not permitted to start up, to be thwarted by the absence of a laser key. Provide any additional comments about risk to meeting program objectives or schedule from the laser safety program requirements. I know of a specific lab that took more than a year to get approval to operate The laser safety program requirements REDUCE the risk to meeting program objectives by reducing the risk of an accident that would truly put the program objectives at risk. Our folks seem very willing to provide timely inputs and responses such that addressing safety isn t assumed to adversely impact objectives and schedule (unless truly warranted). There seems to be a focus on balanced response and responsibility here, and in my areas the safety folks are regarded as partners and team members essential to meeting our goals. 19
