4 BOARD OF COUNTY COMMISSIONERS 1. List of all approved products to be sold, max gross weight and quantity. 2. Copy of the State fire Marshal s certificate allowing for the sales of sparklers must be kept onsite. 3. Identify the total max. gross weight of sparklers and pyrotechnic composition that will be on site at any one time. 4. The maximum temporary stand size is 800 Square feet. NFPA (4) 5. Tent(s) must be flame resistant. Copy of certification is required and must be kept on site. 6. Provide a Site plan which meets the criteria found in the attached Fireworks and Sparklers Sales in tents Checklist 7. Consumer fireworks that are damaged shall be removed and not offered for sale. 8. Minimum of two (2) fire extinguishers with a minimum 4A rating, at least one of which shall be pressured water. Portable fire extinguishers for temporary CFRS facilities shall be installed and located so that the maximum distance of travel required to reach an extinguisher from any point does not exceed 35 ft (10.6 m) Each side of tent shall have signs with letters at minimum four (4) inches in height, stating "FIREWORKS: NO SMOKING" and "NO FIREWORKS DISCHARGE WITHIN 300 FT".

5 BOARD OF COUNTY COMMISSIONERS All applications shall be reviewed under FFPC 6 TH Edition (FSS 791, FAC 69A-50, NFPA 1, 101, 102, 1124) Please complete "Tent Permit for Seasonal Sales of Sparklers/Fireworks in Tents" The layout drawings shall be drawn to an indicated scale and include the following items when submitted: Tents Not Occupied By the Public 1. Retail sales areas within temporary CFRS stands shall have a minimum of two exits. 2. Travel distance shall not exceed 35 feet to an exit, NFPA ( ) 3. Aisles shall not be less than 28 inches wide, 4. Exit openings shall be clear width of 28 inches, 5. Display shelves shall not exceed 8 feet in height, 6. Designate arrangements of sales tables 7. ALL LIFESAFETY features must be identified. Tents Occupied By the Public 1. Minimum of two (2) exits are required, 2. Travel distance shall not exceed 35 feet, 3. Minimum aisle width is 48 inches wide, 4. Exit opening minimum 32 inches clear width, 5. Display shelves shall not exceed 6 feet in height, 6. All electrical wiring will require a building permit (electrical), and must be in compliance with article 305 of NFPA Edition on temporary wiring, 7. Portable Generators must be 20 feet away from the tent and are limited to a 5 gallon fuel tank for diesels and a 2 gallon fuel tank for gasoline, 8. If fuel capacity exceeds the above, the generator and fuel must be be located at least 50 feet from the fireworks tents, 9. Position of sales tables. 10. ALL LIFESAFETY features must be identified. Requirements for All Tents Selling Consumer Fireworks 1. No cooking equipment allowed within 20 feet of tents,

6 2. No open flame within 50 feet of tent, BOARD OF COUNTY COMMISSIONERS 3. Minimum of two (2) fire extinguisher within a minimum 4A rating, at least one of which shall be pressurized water. Maximum travel distance to an extinguisher shall not exceed 35 feet 4. Exit signs and emergency lights are required if business is conducted after dusk. Exits signs must be continuously externally or internally illuminated by a reliable source. 5. Tents must be within 150 feet of public way and Fire Department access. 6. Tent must be at least 30 feet away from combustibles, including dry grass and brush. 7. Parked motor vehicles or trailers used for storage shall be at least 10 feet away from the tent except for deliveries. 8. Each side of tents shall have signs with letters of a minimum of four (4) inches in height stating; "FIREWORKS: NO SMOKING" and "NO FIREWORKS DISCHARGED WITHIN 300 FEET". 9. Tents must be at least 20 feet from any building, 50 feet from any fuel and propane dispensers, flammable/ combustible above ground storage tanks (300 feet from bulk storage areas).

7 BOARD OF COUNTY COMMISSIONERS SPECIAL EVENT/CARNIVALS/FAIRS FIRE INSPECTION INFORMATION AND PLANS SUBMITTAL REQUIREMENTS A Safety Plan is required for review by the Office for any Special Event/Carnival/Fair. The Safety Plan must include but not limited to all of the information listed below. If any portion does not apply, a note must be provided on the plan stating that it is not applicable. If any portion of the requested information is not provided or clarified, the plan will be returned for that provision or clarification to be made and a re-examination fee may apply. THE SAFETY PLAN MUST INCLUDE BUT NOT LIMITED TO ALL OF THE FOLLOWING: Applicable State of Florida codes: NFPA 1- Uniform Fire Code (Florida specific 2015 Edition) Chapter Special Outdoor Events, Carnivals and Fairs Chapter 11.1 Electrical Fire Safety Chapter 18 Fire Department Access and Water Supply Chapter 25 Grandstands and Bleachers, Folding and Telescoping Seating, Tents and Membrane Structures NFPA 96 (2011 Edition) Standard for Ventilation Control and Fire Protection of Commercial Cooking Operation The 6 th Edition Florida fire Prevention Code (2017 Edition) 1. A Scaled Plan with the scale indicated (Use ¼ or 1/8 Scale) 2. A North directional Indicator 3. A detailed Scope of Work/Event Plan/Brief Narrative describing the event. 4. Dates and times of the event. 5. Location of the event 6. The estimated number of attendees 7. Will any tents, Canopies, air supported structures, rides, booths, games, stages and types be utilized and type(s) size(s) and Sizes of all fire extinguishers. 8. Demonstrate/Depict Fire Department Access routes on plan (NFPA1-18) 9. Demonstrate/Depict Fire Department water supply location and type or Fire Hydrants on plan

8 BOARD OF COUNTY COMMISSIONERS The AHJ shall be permitted to regulate all outdoor events such as carnivals and fairs as it pertains to access for emergency vehicles; access to fire protection equipment; placement of stands, concession booths, and exhibits; and the control of hazardous conditions dangerous to life and property. NFPA Tents One of the following shall serve as evidence that the tent fabric materials have the required flame propagation performance: (1) The AHJ shall require a certificate or other evidence of acceptance by an organization acceptable to the AHJ. (2) The AHJ shall require a report of tests made by other inspection authorities There shall be a minimum of 10 ft (3050 mm) between stake lines. [101: ] Adjacent tents shall be spaced to provide an area to be used as a means of emergency egress. Where 10 ft (3050 mm) between stake lines does not meet the requirements for means of egress, the distance necessary for means of egress shall govern. [101: ] Tents not occupied by the public and not used for the storage of combustible material shall be permitted to be erected less than 10 ft (3050 mm) from other structures where the AHJ deems such close spacing to be safe from hazard to the public. [101: ] Tents, each not exceeding 1200 ft2 (112m2) in ground area and located in fairgrounds or similar open spaces, shall not be required to be separated from each other, provided that safety precautions meet the approval of the AHJ. [101: ] Smoking Smoking shall not be permitted in any tent, unless approved by the AHJ. [101: ] In rooms or areas where smoking is prohibited, plainly visible signs shall be posted that read as follows: NO SMOKING Fire Hazards The finished ground level enclosed by any tent, and the finished ground level for a reasonable distance, but for not less than 10 ft (3050 mm) outside of such a tent, shall be cleared of all flammable or combustible material or vegetation that is not used for necessary support equipment. The clearing work shall be accomplished to the satisfaction of the AHJ prior to the erection of such a tent. The premises shall be kept free from such flammable or combustible materials during the period for which the premises are used by the public. [101: ]

9 BOARD OF COUNTY COMMISSIONERS Fire-Extinguishing Equipment. Portable fire extinguishing equipment of approved types shall be furnished and maintained in tents in such quantity and in such locations as directed by the AHJ. [101: ] ELECTRICAL Extension Cords Extension cords shall be plugged directly into an approved receptacle, power tap, or multiplug adapter and shall, except for approved multiplug extension cords, serve only one portable appliance * The ampacity of the extension cords shall not be less than the rated capacity of the portable appliance supplied by the cord The extension cords shall be maintained in good condition without splices, deterioration, or damage Extension cords shall be grounded when servicing grounded portable appliances. Internal Combustion Power Sources Fueling. Fuel tanks shall be of adequate capacity to permit uninterrupted operation during normal operating hours Refueling. Refueling shall be conducted only when not in use Protection. Internal combustion power sources shall be isolated from contact with the public by either physical guards, fencing, or an enclosure Fire Extinguishers. A minimum of one portable fire extinguisher with a rating of not less than 2-A:10-B:C shall be provided. COOKING REQUIREMENTS Concession stands utilized for cooking shall have a minimum of 10 ft (3 m) of clearance on two sides and shall not be located within 10 ft (3 m) of amusement rides or devices. Portable Fire Extinguishers. A minimum of one portable fire extinguisher shall be provided for each concession stand where required by the AHJ in accordance with Section Class K fire extinguishers shall be provided for hazards where there is a potential for fires involving combustible cooking media (vegetable or animal oils and fats). [10:6.6.1] Certified fire extinguishers required per code (must be from a Florida certifying contractor.) Cooking equipment used in fixed, mobile or temporary concessions, such as trailers, pavilions, tents or any form of roofed enclosure, shall comply with NFPA 96 Standard for Ventilation Control and Fire Protection of Commercial Cooking Operation and must have a current inspection and be tagged by a Florida licensed contractor. NFPA

10 BOARD OF COUNTY COMMISSIONERS Open flame cooking must be located a minimum of 10 feet from any structure Cooking areas must be separated from the general public by fencing, enclosure or other approved means Misc. Compressed gas tanks such as helium tanks used for filling balloons need to be secured in such a manner as to prevent them from falling over. LP tanks used for appliances are required to be tested and certified by a licensed person or company permitted to work on LP systems. The equipment is to be checked in place on the day of the event and paperwork provided to the Fire Department, Fire Marshal s Office when completed. LP tanks are to be placed in a crate or stable container, so the LP tank does not have the possibility of falling over. LP tanks are to be located 10 ft. from the open flame cooking devices. Commercial cooking appliances being utilized such as a self- contained cooking unit that does not require the LP tank to be located as noted above from the appliance; documentation to be submitted to the Fire Department, Office for review. Appliances set up on grass surfaces or uneven terrain need to be placed so the equipment is stable from falling over. A fire inspection is required prior to opening to make sure all requirements have been met. NOTE: Special Events including all carnivals, festivals, concerts, and similar assemblies, may require a fire watch based on the safety/site plan review.
