5 Administrative Controls Medical Surveillance Standard Operating Procedures (SOP) Alignment Procedures Laboratory Practices No jewelry No reflective tools No spectators Laser Hazard Assessment Regular Inspections Laser safety training On-the-job (OTJ) training 9 Medical surveillance Programme eye examination Eye examination application/declination form All SFU personnel who work with Class 3B or Class 4 lasers/laser systems are required to acknowledge the SFU Laser Safety Medical Surveillance Programme through completing either: the Eye Exam Application Form (Medical records will be retained by SFU), or the Eye Exam Declination Form (The Laser User retains entitlement to the Programme.) SFU personnel who work with Class 3B or Class 4 lasers/laser systems are offered cost coverage of qualified eye examinations. 10
