1 Laser Lab Inspection Checklist Building/Room: Authorized User: Inspection #: Inspection Detail 1 This inspection is General Questions for Laser Labs 2 Laser Users List Verified by Laser Supervisor? 3 Laser Supervisor and Laser Users have completed online GT Laser Safety Training 4 Laser Supervisor and Laser Users have submitted form LU-1, Laser User Registration 5 All non-laser Users have completed online GT Laser Awareness Training 6 Laser(s) operated only by registered Laser Supervisor or Laser Users 7 Current format ANSI laser warning sign posted at all appropriate locations 8 All entryway laser warning signs include laser class, laser type, wavelength, and LEP OD 9 Presence of invisible wavelength(s) included on all laser warning signs
2 10 EHS emergency contact "Pink Card" is posted at lab entry with current information 11 Viewing aids are used for alignment of invisible beams 12 LEP is available prior to entry into laser controlled area. 13 Are spectators/visitors allowed in the lab during laser operation? 14 If spectators are allowed in the laser lab, provide responses to the following questions. Each much be answered in the affirmative to be satisfactory. 15 LS approved? Hazards explained? Have laser related injuries occurred? Direct supervision? LEP provided? 16 Laser related injuries reported to the LSO 17 Windows within the NHZ are appropriately protected to prevent laser radiation above the MPE exiting the area 18 Laser barriers or curtains are used to prevent laser radiation from exiting the laser controlled area 19 A barrier prevents exposure of an individual entering the laser controlled area 20 Gas cylinders are secured against tipping
3 21 Applicable gas cylinders stored in a dangerous gas cabinet in accordance with the GT Dangerous Gas Program, Table 1 22 Have any lasers been surplused/disposed? Describe the disposal method. 23 Laser(s) surplused? Returned to Vendor? Laser Inventory Verified by Laser Supervisor? Picked up by GT Logistics? Sent to Re-Sale Vendor? 24 Questions for Class 3B and 4 Lasers Laser Supervisor has submitted Form LR-1, Laser Registration to LSO for each laser 25 Written SOP is available for laser(s) in use. 26 Laser Users have received operational training by the Laser Supervisor or designee (indicated by the presence of the Laser User's signature on the 27 Appropriate laser eye protection is available for laser(s) 28 LEP (laser eye protection) is labeled with the protected wavelength and OD 29 Is alignment eyewear used? 30 If alignment eyewear is not already used, is it needed? 31 Laser eye protection is undamaged or has only superficial damage that is not likely to impact the protection provided.
