1 Sheetz Inc. Fire Protection Service Guidelines Overview The following are guidelines for Sheetz Fire Protection Services for the 2016/2018 contract year. Each key area is listed below with the appropriate details. Upon arrival at the site, Provider MUST check in with Store manager on duty. The manager will assign the work to be done. Upon completion of work, the manager on duty will inspect the work, and, if satisfied, provide a receipt showing the work performed. The receipt is the Provider s proof of service and all payment will be made based solely on POS receivings. Payment will be issued weekly at 2%, 15 day terms and will be deposited directly to Provider s account. For example, services provided in the first week of a month will be paid (less a 2% discount) on the second Thursday following the first week. NO PAPER INVOICES WILL BE ACCEPTED FROM PROVIDER. Provider must furnish their Bank information for Direct Deposit of payments. Fire alarm panel testing scope of work (NFPA-72) (Monitored fire alarm panels) Quarterly requirements Visual inspection of all fire sprinkler devices connected to the alarm. This includes water flow switches and valve tamper switches. Test of fire pump monitored points (loss of phase, phase reversal, pump running etc.) Semi-annual requirements Test of sprinkler waterflow switches (flow water or manually trip flapper) Test of sprinkler valve tamper switches (operate valves) Visual inspection lead-acid battery (swelling of loose connections) Test batteries-load voltage Annual requirements Test and visual inspection of panel functionality, LED s, fuses, etc. Test panel battery charger Battery discharge test Test & visual inspection of horns, strobes, chimes, & bells, etc. Test & visual inspection of smoke detectors Test & visual inspection of heat detectors Test & visual inspection of duct smoke detectors Test & visual inspection of electromechanical releasing devices (solenoid) Test & visual inspection of voice evacuation equipment Other Replace sealed leac-acid batteries every 5 years or as test results require Smoke detector cleaning: as test results require. Many newer fire alarm panels are capable of issuing maintenance alerts and adjusting sensitivity as the detector becomes dirtier. Both of these features reduce false alarms. Smoke & heat detector replacement recommended after 10 years of service Any and all parts used may NOT be priced separately, but must be included in the bid price Any deficiencies found must be reported to the store manager on duty immediately

Please refer to NFPA-72 for the complete list of devices, test frequencies, and procedures. Kitchen fire suppression inspection and test scope of work (NFPA 17A, 2009 Edition, Chapter 7 and in accordance with the manufacturer s listed installation and maintenance manual) Inspections shall be performed twice per year. The initial inspection must be completed no sooner than 5 months, and no later than 6 months from the date of last inspection, which will be provided by Sheetz corp. office. Inspections may not be performed during the hours of 11am to 1pm. Contractor must check in with store management prior to initiating service. All fusible links shall be replaced during every inspection.

3 Contractor shall: test all remote pulls test automatic (trip test) and manual release of system verify mechanical operation of system. verify gas shutoff (where applicable). verify electrical shutoff. check system components for cleanliness. test grease build up levels using the International Kitchen Exhaust Cleaning Association (IKECA) grease depth gauge comb (see pictures below). Any system found to be at critical or at the maximum depth must be reported to Sheetz corporate office. Contractor will provide photo validation of depth test. restore system to normal operation. reset system. install new tamper seals. remove cylinder, verify agent weight, and hydrostatic test dates. verify piping to manufacturers specifications (piping/bracing). verify that nozzle caps are intact (for metal caps only). Inspect nozzles to assure that they are properly adjusted for cooking surface coverage and free of obstructions and debris so that they will perform correctly. Install record tag. Verify that owner s manual is on site. Any and all parts used may NOT be priced separately, but included in the bid price. This includes, nozzle caps, fusible links, Co2 cartridge, inspection tag, tamper seal(s), etc. Portable Fire Extinguisher Inspection and Test Scope of Work (NFPA 10, 2010 edition, Chapter 7) Inspection shall occur annually Extinguisher inspections shall be performed at the same time as one of the Kitchen Inspection listed above. Contractor shall: Ensure that facility meets current NFPA guidelines. Remove extinguisher from hanger. Check gauge pressure. Check gauge condition and compatibility. Check weight. Visually inspect for external damage. Check date of manufacture. Check last hydro test date (12 years). Check last 6 year maintenance inspection. Check valve and shell for damage or corrosion Remove nozzle. Check nozzle condition for cracks or splits. Check nozzle threads. Check conditions of horn (if applicable).

4 Check for obstructions. Break seal and remove pin. Check upper and lower handles. Check valve opening for powder or any foreign matter. Replace locking pin and replace with new tamper seal. Clean shell with spray. Replace nozzle. Check band assembly. Tag extinguisher with new annual certification tag. View hazard. Verify that the units correspond with the hazard. Verify that the unit is properly hung with the manufacturer s hanger. Ensure that the unit is properly placed with the normal path of travel. Ensure the unit use is properly defined with class decal. Verify that the unit operation instructions are clear and visible. Ensure that the unit is visible and unobstructed. Replace on hanger. Any and all parts used may NOT be priced separately, but included in the bid price. This includes pull pins, tags, tamper seals, etc. Portable Fire Extinguisher Service and Repair Service calls for portable fire extinguishers and components (cabinets, covers, hangers, etc) will be scheduled by Sheetz corporate office. Contractor must check in with store management prior to initiating service. Each service must be priced separately. Travel charges must be priced separately. Contact Numbers: Contractor must have the capability to provide twenty-four hour emergency service. Contractor must furnish Sheetz with a list of contact telephone and cell phone numbers. Damages: Should the contractor be unwilling or unable to repair or restore damages, contractor shall be invoiced for, and obligated to pay, all actual costs associated with use of an alternative repair contractor. All charges will be invoiced to the contractor responsible for damages. Pricing: All prices must be listed in dollar amounts and be in effect through August 31, Service Expectations and Procedures: Upon arrival at store, store management will assign services that are needed. After the work is completed to the satisfaction of the manager on duty, these services will be entered in to the store POS and a receipt will be given to contractor. Receipt will list services and time of service and will serve as proof that service was provided. No work orders are to be signed by store personnel. Payment: Sheetz will pay contractor based solely on services received at store POS. Payment will be issued weekly at 2%, 15 day terms and will be deposited directly to contractor s account. For

Insurance: At all times during the term, the Contractor shall maintain comprehensive general liability insurance written on a comprehensive basis to provide all major divisions of coverage. The insurance shall be in the minimum amount of $2,000,000 general aggregate limit; $2,000,000 products/completed operations aggregate limit; $1,000,000 personal injury and advertising injury limit; $1,000,000 per occurrence and $1,000,000 fire damage per fire. Worker s compensation insurance in compliance with all applicable statutory requirements, including employer s liability in the minimum amount of $1,000,000 each accident, $1,000,000 disease-policy limit and $1,000,000 disease-each employee. All-risk property insurance to cover full replacement value of Contractor s personal property, improvements and betterments and Sheetz property in Contractor s care, custody, or control. Automobile liability insurance covering owned, non-owned, and hired vehicles in a minimum amount of $1,000,000 for each accident-combined single limit for bodily injury and property damage. All policies shall name Sheetz Inc. and its subsidiaries as additional insured. Contractor shall be obligated to furnish a properly executed original Certificate of Insurance prior to execution of the agreement. No work shall be undertaken by the contractor until a contract has been properly executed.
