4 1.0 Purpose The purpose of this procedure is to provide the necessary information for what is required to perform Fire Watch at the 4M telescope. 2.0 Applicable To 4M Mayall telescope and can also be used as a guide for fire watch at other telescopes at Kitt Peak. 3.0 Scope Provide the necessary steps to prevent fires inside the 4M Mayall telescope. 4.0 Definitions 1. Fast Attack truck a small water truck that is used to put out small fires. 2. Fire Blankets flame retardant or flame proof blankets that are placed over an area(s) to prevent it from burning. 3. Fire Watch a person(s) that observe a specific location where fire could ignite while using a cutting torch, grinders, welding equipment or other potential sources for ignition. 4. Hot Work - is any work that involves burning, welding, using fire- or spark-producing tools, or that produces a source of ignition. 5. Incipient Fire- fire which is in the initial or beginning stage and which can be controlled or extinguished by portable fire extinguishers, without the need for protective clothing or breathing apparatus. 6. Portable Fire Extinguisher- Portable fire extinguishers apply an extinguishing agent that will either cool burning fuel, displace or remove oxygen, or stop the chemical reaction so a fire cannot continue to burn. 5.0 Training Any employee that has the potential to stand fire watch over any Hot Work activities in the 4M telescope must have the following: 1. Be trained to detect fires that could occur in the areas exposed to Hot Work. 2. Be trained in Fire Extinguisher use. 3. Know how to sound alarm in the event of a fire. 6.0 Safety 1. JHA Form 2. Daily Briefing Form 7.0 Personnel, Equipment and Material Required 1. At a minimum, three (3) persons should be standing fire watch 2. One fire extinguisher per person standing watch. 3. 5 4. Fast Attack truck parked at 4M while work in progress. 5. Local Emergency Medical Technician on site notified of Hot Work. 8.0 Procedure The procedures for performing fire watch are as follows: 1. Every person performing the fire watch position should be advised of the potential dangers and be able to identify them while observing the Hot Work that is taking place at the 4M Mayall telescope. The Mountain Supervisor or designated representative is responsible to train the Fire Watch personnel on how to detect potential fires that could occur in the areas exposed to hot work. 2. No other duties are assigned while fire watch is in progress. The sole job is to make sure that no fires are ignited in the work area. 3. The fire watch must have a clear view and immediate access to all areas included in the fire watch. 4. The fire watch must be able to communicate with workers exposed to the hot work of any concerns, such as smoke and/or sparks igniting material. 5. The fire watch is authorized to stop work if necessary and restore to safe conditions within the hot work area. 6. At least one, of the three, fire watch must remain in the hot work area for a minimum of 1 hour after completion of the hot work to detect and extinguish any possible smoldering fires. 8. The fire watch should attempt to extinguish any incipient stages of fire in the hot work areas that are within the capability of available equipment. 9. The fire watch must alert employees of any fire beyond incipient stage. 10. If unable to extinguish fire in the areas exposed to the hot work, activate an alarm by radio/phone. 9.0 Procedure Review This procedure must be reviewed by employees under the following terms: 1. Initially 2. When there is a change in the job process(es) 3. When there is a change in a worker s status, or job title 4. When governing authorities (such as OSHA) make changes 5. Every three years, at a minimum 10.0 Records The records that must be kept by the supervisor are listed below. Training records must be kept on file for a minimum of five years. 1.
