1 Environmental Works, LLC 2634 SE Steele St. Portland, OR Phone: Fax: Comprehensive Short-Term Radon Survey Report Test Performed At: Trillium Charter School 5420 N Interstate Ave Portland, OR Environmental Works is a licensed, bonded, and insured general contractor (CCB# ; WA# ENVIRWN906CS) and is certified by NEHA-NRPP (National Environmental Health Association- National Radon Proficiency Program #106186RT) as a Radon measurement and mitigation contractor. The following scope of work is to determine Radon levels in the above referenced property and offer a mitigation technique if needed. This survey does not address radon levels generate from out-gassing from well water or building materials. Test Methods: All radon testing methods presented in this agreement are based upon the American Association of Radon Scientists and Technologists (AARST), Protocol for Conducting Radon and Radon Decay Product Measurements in Schools and Large Buildings(ANSI-AARST MAMF-2012) Client was provided with an Attention Notice prior to conducting the radon survey. This notice described the Closed Building conditions required prior to and for the duration of the radon survey. Client guarantees that these conditions were maintained during this radon survey. This survey was conducted and tests were deployed by Sebastian Busby (NEHA-NRPP # RT) on 10/24/2016 and retrieved by Sebastian Busby on 10/27/16, for 72 hours of continuous radon measurement. 1 Short-Term activated charcoal kit was deployed in each selected room, totaling 22 test locations. 2 locations were deployed with a side-by-side duplicate test. 1 Blank was deployed to detect for batch contamination. 1 Spike was conducted to test for laboratory analysis accuracy. 1 Continuous Radon Monitor (CRM) was deployed to track hourly variations of radon gas and compare results to a side-by-side charcoal test kit. Charcoal test kits were provided and analyzed by AirChek Inc. (NEHA Lab ID: AL) Charcoal test kits were sent to the AirChek lab via UPS Second Day Air service. 1

2 Summary: Quality control measures indicate no error in laboratory analysis or batch contamination. All rooms tested below the EPA s action level of 4.0 Pci/l. No action is required. Retest units every 5 years or after changes to building structure or HVAC system. Statement of Test Limitations: There is some uncertainly with any measurement result due to statistical variations and other factors such as daily and seasonal variations in radon concentrations. Variations may be due to changes in the weather, operation of the dwelling, or possible interference with the necessary test conditions. 3 Table 2: Quality Control Measures Blanks involve test kits that are immediately sealed after opening and blindly tested with real kits to determine batch contamination. Results should be less than 0.5 PcI/l. 10 Radon Risk Information Radon is the second leading cause of lung cancer, after smoking. The US EPA and Surgeon General strongly recommend taking further action when a homes radon test results are 4.0 or greater. The concentration of radon in the home is measured in picocuries per liter of air (). Radon levels less than 4.0 still pose some risk and in many cases may be reduced. If the radon level in the home is between 2.0 and 4.0, the EPA still recommends that you consider fixing the home. The average indoor radon level is estimated to be about 1.3 ; roughly 0.4 of radon is normally found in the outside air. The higher the home radon level, the greater the health risk. Even homes with very high radon levels can be reduced to below 4.0 and many homes can be reduced to 2.0 or less.
