1 C - Side Date: 11/18/2017 Time: 21:48 Incident Number: Address: 3330 N Clayton Overview: District 4 report Group response to reported structure fire at 3338 Clayton. E09 on scene reporting fire showing from one apartment in a 1 story row of apartments on the alpha (address) side. E09 laid a supply line from 33rd and Clayton and advanced an 1-3/4 line into 3330 Clayton. E10 on scene went to work at E09's hydrant. TR15 on scene in Command. E09 reported stove fire and fire knocked. At the same time, a report was given that there were people still inside the building. Chief 4 arrived on scene and assumed Command. Conditions on scene at time of Chief 4's arrival were heavy smoke in the entire block with heavy turbulent smoke coming from the roof line of entire structure. Crews reported smoke in all apartments. IC assigned T26 to assist with search and rescue with numerous parties being assisted out of apartments by TR15 and T26. T08 assigned to the roof to assess conditions. T08 began vent ops and immediately encountered a large volume of fire that heavily vented through the roof above the original fire apartment towards the Charlie side of structure. R01 assigned to roof and Roof Division created with T08 as Div Sup. Operations included venting the roof and ensuring that fire spread in the attic was identified and a holding point for fire stop was created. T08 was given an 1-3/4 line to extinguish the fire on roof with caution instructions to not push fire into structure. E03 on scene laying a supply line to the rear and advancing an 1-3/4 line into one of the Delta exposure apartments. It was later determined that there were two levels of ceiling in all apartments and stove fire had extended into both ceiling/attic areas. Due to the difficulty of exposing both ceiling areas and gaining access to hidden fire above, the fire extended into the attic area of four apartments. The fire was contained to these four units and was stopped from extending into all ten units. 1

2 At the height of fire, 4 1-/3/4 lines were in operation and Truck Companies were paired with Engine companies to assist in seeking hidden fire. Based on the amount of fire, command requested an additional Truck and Engine company. These two companies were used to rotate personnel to rehab and keep fire attack in progress. In addition, the initial Dispatch of companies had one extra Truck company assigned. Primary and Secondary searches were completed and negative with one civilian transported for anxiety related to fire. Fire loss was declared stopped by the IC and TR15 and T08 performed a thorough sweep of the affected apartments with thermal imagers to ensure the fire was extinguished. Fire watch was deemed appropriate to ensure that there was not a rekindle fire. E26, E05, and E16 completed fire watch rotating through every 2 hours until following morning. All occupants of the ten affected apartments were displaced due to fire and smoke damage with RTD providing a bus for occupants to keep them warm in the cold. Red Cross worked with occupants to assist them in finding alternative lodging. Board up company and property rep on scene prior to beginning of fire watch. Board up company provided security throughout the night and to begin boarding up the structure the following morning. Arson Investigators determined cause of fire to be from cooking on the stove. Risk vs Benefit: Report of fire in the kitchen area of a single story, ordinary construction, multi-unit apartment building. Risk Fire in kitchen area and extending into the attic with occupants still inside. 4 1-24 ladder on the charlie side Building Size, Type, Number of Stories, and Occupancy: 10 units approx. 600 sf. each, 1 story Type 4 ordinary construction residential Involvement Upon Arrival and Initial Location of Fire: Fire was showing from the alpha side of the building Unit Fire quickly spread into the ceiling and attic area over four units. Communications: TAC 4 Special Challenges, Hazards, and Safety Issues: Communications with the charlie side relaying exactly how many units there were and where crews were located (not all units had a back door) as the view from the rear of the structure differed from the front. Compromised roof, sagging over the initial fire area. T-8A portable was not working properly, used voice communication to IC of compromised roof. Critical information was not transmitted over TAC 4 Fire spread throughout the attic area with a false celling. Companies stated fire was under control and no need to pull more celling. Moments later D02 C side transmitted there was turbulent smoke blowing through an attic vent hole. Picture #1. Command Structure: IC: Chief 4 Ruzycki Safety Officer: Ops 2 Morelli RIT: Tower 9 Captain Schamber Tactical Priorities: Evaluation and Summary Life safety: 4 occupants rescued from 2 separate units by TR-15 and T-26. Vertical ventilation assisted with search and rescue operations as well as extension of fire. Fire suppression: Interior fire attack focused on original fire unit with Engine 3 in exposure unit to stop spread of fire in attic. Property conservation: Aggressive post-control overhaul, exposing celling in 4 exposure apartments to minimize fire spread. Positive Outcomes: 4 persons rescued from 2 separate units and all occupants accounted for. Loss stopped and heavy damage limited to only four apartments in a ten unit building 4

5 Additional Considerations/ Recommendations: Expose drywall until clean wood is seen and fire spread is stopped. Safety considerations: If a transmission is made verbally between the IC and another member this communication should be retransmitted over the air for all members to hear. This is especially true of pertinent safety issues and tactical considerations. Summary: Small kitchen fire which quickly turned into an all hands working event due to extension of fire into the attic. I.C. performed well by coordinating fire attack with search and rescue efforts in the fire unit and exposure units. Crews also did a great job with coordinating fire attack with vertical ventilation to improve conditions on the inside. I.C. noticed the need for additional companies early in the incident and asked for more help with the addition of two companies. All companies, working safely and efficiently, ensured that life safety was a priority while stopping the spread of fire to adjoining units. 5
