Officer determined the dog belongs to a home owner on West Hawthorne Court and the animal was returned to the owner. Officer cleared. Alarm Business 03/26/ :37: NORTH SHORE DR Narrative: Officer responded to the 900 block of North Shore Drive per the alarm company viewing someone in a pick up truck on the main grounds. Officer arrived on scene and checked the exterior of the business. Security also arrived on scnene. No irregularities were observed on officers cleared to resume patrol duties. Animal Complaint 03/26/ :51: SHERIDAN RD Narrative: Officer responded to the 900 block of Sheridan Rd. for a animal complaint involving a lamb that had left the farm and was traveling N/B on Sheridan Rd. Officer searched the area yielding negative results. Officer met with the key holder of the property who advised that there was no issue and that police services were no longer needed. Assist Fire 03/26/ :24: E SCRANTON AVE Narrative: Officer responded to the 200 block of E. Scranton Ave for a lockout to residence. Lake Bluff Fire Department responded and were unable to make entry into the home. The resident was provided contact information for Locksmith Services in Lake Forest. When no other police services were needed officers cleared Ambulance Assist 03/26/ :28: S WAUKEGAN RD Narrative: Officer responded to the 200 block of Waukegan Rd to assist the ambulance with a medical issue. Knollwood Fire responded and Lake Bluff EMT's responded. Lake Forest Ambulance 421 also responded and transported the patient to Lake Forest Hospital. Check Well Being 03/26/ :11: HANCOCK AVE Narrative: Officers responded to the Lake Bluff Public Safety Building and met with the caller who was concerned for her daughter. The caller requested officers perform a check for well being due to her concerns. Officers then responded to Hancock and met with the caller's daughter and found no issues. Alarm Residential 03/27/ :20: MEADOW CIR Narrative: Officers were dispatched to the block of Meadow Cir for an activated burglar alarm. Prior to Officers arrival, the alarm company stated they received proper codes and attempted to cancel. The alarm company stated they were unsure if anyone was on scene. Officer went to verify and there was no answer at the door. Officer observed no irregularities and cleared.

2 Alarm Residential 03/27/ :30: GREEN BAY RD Narrative: Officers responded to the 1600 block of Green Bay Road for an activated burglar alarm. Officers, while en route, were advised there was a person on location with wrong access code. Upon arriving, Officers were advised that the alarm was being cancelled. Officers verified with an employee on location and then cleared. Theft Under $300 03/27/ :32: ROCKLAND RD Narrative: Officer took a station report of a theft that took place in the 900 block of Rockland Road. The complainant reported that $1, USC was taken from her purse. Check Well Being 03/27/ :56: E SHERIDAN PL Narrative: Officer responded to the Lake Bluff Middle school for two children who were on their way to the park and got separated from their uncle. The two children were able to go to the middle school where school employees let them into the building. Assist Fire 03/27/ :44: LINCOLN AVE Narrative: Officers responded to the 400 Blk of Lincoln Ave for a smell of natural gas. Lake Bluff Fire Department Engine 520 responded and contacted the North Shore Gas Company after being unable to locate the source of the odor. When no police services were needed officers cleared. Check Well Being 03/27/ :58: E WOODLAND RD Narrative: Officers responded to the block of Woodland Rd. for a well being check. Upon arrival officers assisted the subject in getting to Lake Forest Hospital. Alarm Business 03/27/ :06: S WAUKEGAN RD Narrative: Officers responded to the 200 block of S. Waukegan, for an alarm, north door entry activation. Officers found the exterior secure, with an audible alarm. No irregularities observed, no keyholder responding and Officers cleared. Suspicious Person 03/28/ :31: E SCRANTON AVE Narrative: Officers responded to the block of E. Scranton for a report of suspicious persons selling candy and asking for money. Officers attempted to locate the subjects involved which yielded negative results. Assist Fire 03/28/ :46: SUNRISE AVE Narrative: Officers responded to the 400 block of Sunrise Ave for a smoke detector activation. Officers met with the keyholder who explaned the activation was done so in error and that there was no fire. Lake Bluff Fire Department arrived on scene noted no irregularities. When no other police services were needed officers cleared.

3 Ordinance Violation 03/28/ :32: W BLODGETT AVE / LINCOLN AVE Narrative: Officer responded to the area of W. Blodgett Ave and Lincoln Ave for a solicitor complaint. The complainant advised that a male subject was asking for money for environmental services. Officer located the subject and determined the organization that he was working with did have a valid village soliciting permit, however his name was not included. Officer advised the subject and his supervisor who was listed on the permit that all subjects names must be included on the permit and that they should carry a copy of the permit with each individual to be presented upon request. The organization stated that they understood and would correct the misunderstanding before continueing to ask for donations. Officer cleared. Ordinance Violation 03/28/ :08: N ENVIRON CIR Narrative: Officers responded to the block of for a solicitor complaint. Officers met with the solicitor and explained a permit would be necessary. The solicitor was unable to produce a permit and was told to cease soliciting. Child Seat 03/28/ :23: E CENTER AVE 0 Narrative: Subject came into the Lake Bluff Public Safety Building to have two (2) child safety seats checked. An Operation Kids Child Safety Seat Checklist was signed and the seats were installed. Ambulance Assist 03/28/ :31: PHILLIP CT Narrative: Officers responded to the 100 block of Phillips Ct for a medical issue. Officers met with the subject who was transported to Lake Forest Hospital by Lake Forest Ambulance A-421. When no other police services were needed officers cleared. Lost/Mislaid Property 03/29/ :43: FOREST COVE RD Narrative: Officer responded to the PSB to meet with a resident who noticed on 3/23/17 at O hare Airport that his front plate was missing. Resident was unsure on how or why the plate was missing and already replaced both license plates with new ones at the Secretary of State office. The office then recommended he file a police report. Citizen Assist 03/29/ :04: SKOKIE HWY Narrative: Officer responded to assist a car dealership with a VIN inspection on a 2015 BMW that was being sold to a customer and will be registered in the state of Missouri. Animal Complaint 03/29/ :48: SHERIDAN RD Narrative: Officers responded to the area of Crab Tree Farms for an animal complaint involving sheep in the roadway. Officers searched the area and were unable to locate any sheep. A keyholder was notified of the incident. When no other police services were needed officers cleared. Alarm Business 03/29/ :16: S WAUKEGAN RD Narrative: Officer responded to the 200 block of S Waukegan Rd for an activated burglar alarm coming from the north entry door. Officer found the building secure and observed no irregularities. S

4 Assist Fire 03/29/ :16: LAKELAND DR Narrative: Officer responded to the 500 block of Lakeland Drive for report of smoke in the house. Upon arrival homeowner located paper on a light fixture that was burning. The item was removed and smoke extinguished. LBFD responded to verify the fire was out. Officer cleared. Alarm Residential 03/30/ :24: SHERIDAN RD Narrative: Officer responded to the 1000 block of Sheridan Rd for an activated burglar alarm, caretaker called back to verify that it was a false alarm. Officer arrived on scene to verify with the caretaker and observed no irregularities. Officer cleared to resume patrol duties. Alarm Business 03/30/ :59: SHERWOOD DR Narrative: Officer responded to the 900 block of Sherwood Dr for an activated burglar alarm. Officer met with employees on scene who stated they set the alarm off in error and there were no problems. Accident Property Damage 03/30/ :34: ROCKLAND RD / S WAUKEGAN RD Narrative: Officer responded to the Shell parking lot for a two vehicle accident that occurred on Rockland Road at Waukegan Road. Officer completed a formal traffic crash report and issued citation for failure to reduce speed to avoid an accident to the at fault driver. Officer explained the forms to the drivers and cleared. Citizen Assist 03/30/ :51: SKOKIE HWY Narrative: Officer responded to a dealership for a vin verification. Officer completed all documentation and ran the VIN for the vehicle which revealed negative results. Check Well Being 03/30/ :24: S WAUKEGAN RD Narrative: Officers recieved a call for a check well being for a male who the caller believed to be the subject of a missing persons case in a parking lot. Prior to officers arrival, the caller advised that the subject left in a unknown direction and that police service would no longer be necessary. Accident Property Damage 03/30/ :33: ROCKLAND RD / S WAUKEGAN RD Narrative: Officers responded to the area of Waukegan Rd and Rockland Rd. for a possible 3 vehicle accident. Officers responded to the area and were able to locate the accident west of the intersection in Lake County jurisdiction. Officers confirmed Lake County Sheriff's Office was on scene and did not require any further assistance. When no other police services were needed officers cleared. Child Seat 03/30/ :20: E CENTER AVE Narrative: Subject came into the Lake Bluff Public Safety Building for a child safety seat install. An Operation Kids Child Safety Seat Checklist was signed and seat was installed forward facing.

5 Found Property 03/30/ :21: SHERIDAN RD Narrative: Officers responded to the 600 block of Sheridan Rd for a report of a set of keys laying in the parking lot. Officers were able to locate the keys and transported them to the PSB. The owner of the keys was contacted and advised that he would collect the set of keys when he returns later in the night. The keys were entered into found property. Accident-Hit and Run PDO 03/30/ :18: E SCRANTON AVE Narrative: Officers responded to the block of E. Scranton for a delayed hit and run accident property damage only involving a parked vehicle. Officers met with the victim and completed a Illinois Traffic Crash Report after which a copy was distributed to the victim. Accident PI 03/30/ :40: N WAUKEGAN RD / ROCKLAND RD Narrative: Officers responded to the intersection of Rockland Rd. and Waukegan Rd. for a 2 vehicle accident with injuries. Lake Forest Ambulance 421, 422, and 423 transported the injured in Unit #1 to the Lake Forest Hospital and the injured from Unit #2 to Condell Hospital. Both Lake Bluff and Knollwood Fire Departments responded and rendered aid. Both vehicles were removed from the scene by Towing. All pertinent information was collected and a Illinois Traffic Crash Report was completed. Alarm Business 03/30/ :13: S WAUKEGAN RD Narrative: Officers responded to the 200 block of S Waukegan Rd for an activated burglar alarm. Officers checked the building and found it to be secure. No keyholder was responding and Officer cleared. Animal Complaint 03/31/ :42: RAVINE AVE Narrative: Officer responded to the 700 block of Ravine Avenue for a report of a sick or injured fox in the back yard. Officer arrived on scene and observed a baby fox in the bushes. Officer was able to move the fox into the ravine area along the east side of the property. Officer gave the homeowner a number for Lake County Animal Control if the baby fox stays in the area. Lost/Mislaid Property 03/31/ :46: RAVINE AVE Narrative: Officer took a lobby report for a lost/ mislaid property case involving an Identification Card. Officer met with the complainant who stated that she thinks she had lost the id card about a year and a half ago. Assist Fire 03/31/ :52: NORTH SHORE DR Narrative: Officers responded to the 900 block of North Shore Dr to assist the fire department with a broken gas line. Officer arrived on scene with Lake Bluff, Knollwood, and Lake Forest Fire Departments. Officer stood by until no further assistance was needed and cleared. Lost/Mislaid Property 03/31/ :17: CRAB TREE LN Narrative: Officer responded to the lobby of the Lake Bluff Police Department for a walk in complaint for a lost/missing/stolen report. The complainant stated the temporary license plate fell off her vehicle somewhere between Lake Bluff and Chicago and could not locate it.

6 Driving Complaint 03/31/ :17: ROCKLAND RD / GREEN BAY RD Narrative: Officer responded to the area of the 900 block of Rockland Rd for a driving complaint. The complainant advised that a gray in color Jeep SUV was all over the road and was last seen parking. Officer located the vehicle unoccupied in the lot. The complainant did not wish to sign complaints and officer observed no violations and cleared. Alarm Business 04/01/ :44: S WAUKEGAN RD Narrative: Officers responded to the 200 block of S Waukegan Rd for an activated burglar alarm. Officers found the exterior to be secure and observed no irregularities. No keyholder responding. Ambulance Assist 04/01/ :08: WARRINGTON DR Narrative: Officer responded to the block of Warrington Dr for a female having a medical issue. The Lake Bluff and Knollwood Fire Departments responded and Lake Forest Ambulance A421 responded to the scene as well. The subject was transported to Lake Forest ER. Assist Fire 04/01/ :36: WARRINGTON DR Narrative: Officers responded to block of Warrington Dr for a residential lock out. The Lake Bluff Fire Department and Knollwood Fire Department were able gain entry into the house. All fire and police personnel cleared. Alarm Business 04/01/ :16: ROCKLAND RD Narrative: Officer responded to the 300 block of Rockland Rd for an activated burglar alarm. Officer met with employees on scene who stated they are working on a trailer and must have set an alarm off. Officer observed no irregularities and cleared. Citizen Assist 04/01/ :29: E SHERIDAN RD Narrative: Officer responded to the Public Safety Building to take a report of deceptive practice. Formal report is pending. Citizen Assist 04/01/ :57: SKOKIE HWY Narrative: Officer responded to a dealership for a VIN verification. Accident Property Damage 04/01/ :10: ROCKLAND RD Narrative: Officer responded to the area of Skokie Valley Rd at Route 176 for a property damage vehicle accident. Officer arrived on scene and determined the accident occurred in Lake County Sheriff Jurisdiction. Lake County Sheriffs Department was notified and officer cleared. Alarm Residential 04/01/ :44: E SHERIDAN RD Narrative: Officer responded to the 200 block of E. Sheridan Rd for an activated burglar alarm. Officer arrived on scene and checked the exterior noting no irregularities and all doors secured. Officer cleared.
