4 The new standard The new standard Thorough cleaning is an essential prerequisite for processing medical instruments. Cleaning and disinfection processes for instruments have to be validated in accordance with health and safety regulations. Washer disinfectors will be expected to comply with a totally new set of standards in the future as specified by the Norm pren ISO Miele has already implemented technical solutions in the latest generation of washer disinfectors which meet the requirements of this standard. They are highly reliable and efficient and suitable for use in doctors' surgeries as well as in central sterilising centres in clinics where they are particularly suited to processing delicate instruments. The single compartment system offered by Miele washer disinfectors, in which each machine cleans, disinfects and dries, has proved itself as particularly flexible and economical. The automatic mobile unit recognition on the G 7835 CD, further ensures that instruments are cleaned in the correct programme. Separate sensors control and monitor the process to ensure that the machine is able to maintain cleaning and disinfecting parameters with even greater precision. Easy validation and revalidation thanks to the controllability of the temperature in the wash cabinet and on the items being processed via the many temperature sensors. The door is securely locked for the whole programme duration. It can only be opened at the end of the programme if all programme parameters have been successfully achieved. 6

7 Safer More economical Better results Miele's new washer disinfectors are economical to run The steam condenser with heat exchanger ensures significantly reduced water consumption on machines without a drying unit. Thanks to the new Profi Monobloc water softener reactivation during a cleaning programme has led to a reduction in salt consumption of about 50% (in hardness range 5 15 dh / mmol per litre). A separate reactivation programme is not necessary. If you update your machine to a newer model you can still use your existing baskets. Service friendly and easy to recycle construction. 18 Mobile units for anaesthetic instruments Cleaning and disinfecting anaesthetic instruments Miele's E 435/2 and E 436/2 universal mobile injector units set new standards in processing anaesthetic instruments and equipment. These new developments are in response to the requirements for optimum standardisation of cleaning, drying and handling. Breathing tubes up to 150 cm long are attached to an adapter and fitted on a spiral rack. During cleaning and disinfection a vertical flow of water ensures optimum processing of anaesthetic tubes. Following the cleaning and disinfection stages, the wash liquor is quickly and completely drained out of the tubes. These are then dried thoroughly in the subsequent drying phase. Instruments can also be thoroughly processed in the E 435/2 and E 436/2 universal mobile injector units using a chemo-thermal disinfecting programme. The upper level of the E 450 can be fitted with up to forty different connections, including injector jets, injection sleeves and adapters for processing MIS instruments. Instruments can be arranged in a variety of ways to suit individual applications. The lower level can be used for mesh trays E 457, E 460 and E 451. The E 451 mesh tray, suitable for small instruments such as plunger valves, screw fittings, seals etc. is included as standard. On machines with MCU (microprocessor) controls the DESIN vario TD programme can be used for processing Hopkins fibre optics (manufactured by Storz) and Lumina fibre optics (manufactured by Wolf) in an E 450 mobile injector unit. Connections The E 450 has a total of forty connections for cleaning and rinsing hollow instruments used in keyhole surgery. 20 instruments/outer sleeves can be arranged in a slanting position in two rows, each with 10 connections. As these are plug-in connections, the arrangement can easily be varied to suit individual applications. On both outer edges of the upper level there are two further rows with 20 screw connectors each (arranged vertically for injector jets/adapters/injection sleeves). In order to maintain spray pressure, all unused connections must be fitted with a blanking screw. The standard fittings supplied with the E 450 of injector jets, injection sleeves, adapters will not be suitable for all applications. The basic range of fittings can be complemented by accessories designed to suit individual requirements. The E 474/2 has been designed by Miele to provide a flexible system solution for use with the G 7736 CD, a machine which has proved itself to be particularly successful in all aspects of handling and personal protection. The E 474/2 is designed as a basic unit to take three insert modules. The insert modules are equipped with one or two feeder tubes with connections for adapters, injector sleeves and jets. Modules can be specifically set up for surgical instrument sets or converted very quickly to take surgical instruments by simply changing the jets. Depending on the surgical instrument set (e.g. laparoscopes) you can use either two E 905 inserts for short instruments and one E 906 insert for long instruments or one E 905 insert for short instruments and two E 906 inserts for long instruments. For urology applications there is the E 903 modular insert which is designed to take TUR instrument sets. Instruments are fitted onto the jets and adapters on the insert modules at a workbench. They are then fitted into the mobile unit and connected. Because of the many different ways in which the E 474/2 can be equipped to suit specific applications it is supplied without inserts or other modules. This way users can specify their specific requirements and have a system that provides troublefree processing of MIS instruments. 22

The upper level of the E 440/1 has 36 connectors for hollow instruments. On the left hand side there are two rows with 10 connections each for Luer lock apapters or injector jets for cannulae. The right hand side has 16 horizontal, flexible connectors for tubes with Luer lock adapters for cannulae, hand pieces, rinsing and suction hand pieces etc. The lower and middle levels of the E 440/1 are designed to take micro instruments such as scissors, needle containers, forceps, hooks etc. in an E 441/1 mesh tray. The E 440/1 is designed to break the force of water from the lower spray arm. This way micro instruments are cleaned almost exclusively from above by the E 440/1 spray arm. The water jets will not dislodge the micro instruments, but rather push them even firmer into their holders. Instruments cannot slip or jump out 24 of their connections. Disinfection is achieved by maintaining a temperature and holding it for a period of time, e.g. 90 C for 5 minutes. To achieve optimum cleaning results whilst maintaining these standards it is advisable, so long as infection control permits, to use a pre-wash, i.e. use the standard DESIN vario TD programme. Carrying out the final rinse with fully demineralised water (AD) is particularly important for protecting instruments from any residues in the water. Miele is the first to develop a system for these. The system has been tested by the company Aesculap and approved for use with their COMFORT Motorline and ACCULAN II Series motors. Machine processing of these motor systems in Miele washer disinfectors is patent pending. Solutions for other systems are being developed. The process requires the Desin Vario TD programme together with special holders for the motor components. The following conditions must be maintained: 1. Preparation The Desin vario TD programme must be used for cleaning and disinfection together with a neutral/enzyme cleaning agent at 50 C with the final rinse carried out in fully demineralised water and thermal disinfection. 2. 34 Container system for baby bottles 1. Soiled, empty baby bottles are placed in the E 135 insert with the bottle neck facing upwards. 2. The E 135 is locked with the lid (latticed). 3. The containers are turned over 180 (the bottle neck is now facing downwards) and placed in the washer disinfector. Advantage: Simple handling of 19 baby bottles in a single container. 4. Following cleaning and disinfection the lid is removed and the bottles can be refilled whilst remaining in the container. 5. Refilled bottles are sealed and stored in the refrigerator until required. Advantage: No need to handle each bottle separately, saves time and minimises the risk of breakages. The number of E 135 inserts required depends on the size of the post-natal unit. Example: A unit with 60 babies being fed 5 times a day: 5 x 60 = 300 bottles with an additional 2 bottles a day for water etc.: 2 x 60 = 120 bottles per day. A total of approx. 420 baby bottles are therefore required every day. Handling 420 baby bottles will require 22 x E 135 inserts and 2 3 x E 364 inserts for teats. 34

The resin contains sodium ions. When hard water flows over the resin in one cartridge, the resin attracts calcium and magnesium ions and at the same time discharges its sodium load. There is no flow of water through the second cartridge at this time. When the supply in the first cartridge is depleted, a switch to the second cartridge is activated by the hardness range selector which is set to the local water hardness level by the service technician when commissioning the machine. At the same time reactivation of the spent resin in the first cartridge begins. Brine (NaCl) is taken from the salt reservoir. The salt reservoir must be regularly filled with coarse salt pellets. The reservoir takes approx. 25 kg of salt. This ensures that the system operates continuously. 39
