6 PROCESS FLOW A: RESPONSE TO EMERGENCY SIREN STEP EMERGENCY SIREN SOUNDS The PPA have installed emergency sirens (oscillating air raid tones) that when activated indicate a possible anhydrous ammonia release (liquid or gas). Upon hearing the emergency siren all personnel must take all reasonable measures to seek shelter in place and take mitigating measures in accordance with this plan. In the event of a suspected or confirmed anhydrous ammonia release the emergency siren will be activated at one of the following locations: Bulk Liquids Berth Control Room Vessel Traffic Services Centre Safety and Security Building Landside Operations Office Refer to Appendix B for emergency siren locations. STEP ARE YOU A MEMBER OF THE ECO? Members of the Emergency Control Organisation (ECO) have been trained to respond appropriately in the event of an anhydrous ammonia emergency and take all reasonable measures to preserve the health and wellbeing of PPA personnel (staff, visitors and contractors). ECO members are assigned to designated areas / building s within the PPA Landside Restricted Zone (LRZ). If YES refer Process Flow B. Refer to Appendix B for emergency siren locations. STEP ARE YOU IN YOUR DESIGNATED BUILDING? ECO members are assigned to designated buildings within the PPA Landside Restricted Zone (LRZ). If an ECO member is not within their designated building a delegate will graduate into their role (refer Figure 1). If an ECO member is not in their designated building they will access the most appropriate Muster Point location (if safe to do so) and follow the directions of the Area Warden / Deputy Area Warden. If NO refer Step 1.3. STEP 2.3 ECO MEMBERS COLLECT CLIPBOARDS, HATS, TABARDS AND RADIOS Upon hearing the emergency siren all ECO members will convene at the emergency station within their designated building and collect their appointed clipboard (Duty Cards, Emergency Response Checklists, Event Log, Head Count Log, pen, paper), hat, tabard, Hand Held Electronic Access Card Reader (HHCR) and radio. STEP WARDENS DIRECTS PERSONNEL TO SHELTER IN PLACE? Shelter in place and taking mitigating measures is always the first response in an anhydrous ammonia emergency. All personnel who are inside a building (or able to access the building safely) will be directed to remain indoors by the Area Warden / Deputy Area Warden (also called Warden) when the emergency siren sounds. STEP WARDENS COMPLETE HEAD COUNT The Area Warden / Warden will complete a head count of all personnel in their designated building and document the following information on the Head Count Log, this can also be completed electronically by using the HHCR s: Full Name Employer MSIC Number Phone Number Page 8 of 28

9 STEP AREA WARDEN RADIOS CHIEF WARDEN AND REPORTS HEAD COUNT NUMBERS The Area Warden will complete a radio test through to the Chief Warden. Once positive confirmation is received the Area Warden will report the head count and a situation report (casualties, conditions, visual observations, unaccounted personnel, other relevant information) to the Chief Warden (this includes information provided to the Area Warden by the Warden(s). STEP CHIEF WARDEN ADVISES IT IS SAFE TO MUSTER? The Chief Warden will undertake all reasonable efforts to immediately ascertain the actual (or potential) impact of an anhydrous ammonia release (i.e. confirmation of wind direction, visibility of the ammonia gas plume, WAL panel levels). The Chief Warden will establish and maintain contact with the Yara Pilbara Control Room and/or Yara Pilbara Loading Master (located at the Bulk Liquids Berth Control Room) via UHF 17. The Deputy Chief Warden will monitor UHF 16 (Citizen Band) and report accordingly to the Chief Warden. The Chief Warden will advise the Area Wardens when it is appropriate to proceed to the Muster Point (inside the building). If YES refer Process Flow C. Area Wardens will pass on all directions from the Chief Warden to their Warden(s). STEP CHIEF WARDEN ADVISES EMERGENCY KITS BE USED? Emergency Kits have been established to facilitate shelter in place mitigating actions and are located at the: 1. Landside Operations Amenities Building Crib Room 2. Corporate Services Building 3. Administration Building 4. VTS Building 5. Maintenance Building 6. Safety & Security Services Building. Each PPA building has an Emergency Kit stocked. If the Chief Warden determines (based on available information) that it is not safe to proceed to the Muster Point they may direct the Area Warden to take mitigating measures utilizing the Emergency Kits. Area Wardens will pass on all directions from the Chief Warden to the Deputy Area Warden(s). STEP WARDENS COMPLETE SHELTER IN PLACE CHECKLIST The Area Warden and Warden(s) will direct all personnel for the duration of the shelter in place period ensuring the Shelter in Place Checklist (Appendix A) is completed. 10 STEP 2.10 AREA WARDEN AWAITS FURTHER ADVICE FROM CHIEF WARDEN The Area Warden will ensure shelter in place mitigating actions are maintained and await further direction from the Chief Warden. As directed by the Chief Warden the Area Warden will provide all personnel sheltering in place with periodic updates regarding the status of the anhydrous ammonia emergency response. Area Wardens will pass on all directions / updates from the Chief Warden to the Warden(s). Area Wardens and Wardens will also advise the Chief Warden immediately if there is any changes to the scent of the air in their buildings as mitigating action may be required. Page 10 of 28

12 PROCESS FLOW C: MUSTER AND EVACUATION STEP 3.1 WARDENS DIRECT PERSONNEL TO MUSTER POINT The Area Warden and Warden(s) will direct all personnel to proceed to the Muster Point. This will only occur once the Chief Warden has advised that this action is appropriate. Personnel may be issued with Personal Protective Equipment (PPE) by the Area Warden / Warden prior to proceeding to the Muster Point (instruction will be provided by the Wardens regarding how to fit / use PPE). STEP 3.2 AREA WARDEN CONDUCTS MUSTER AND REPORTS TO CHIEF WARDEN The Area Warden (in consultation with Wardens) will account for all personnel once at the Muster Point (inside the building) by checking / compiling all Head Count Logs. The Area Warden will report head count numbers to the Chief Warden. The Area Warden / Warden(s) will take all reasonable measures to account for personnel once at the Muster Point. NOTE: The responsibility to search for, identify and render assistance to missing personnel is the responsibility of the Hazard Management Agency (HMA), not the ECO. STEP 3.3 AREA WARDEN MAINTAINS RADIO CONTACT WITH CHIEF WARDEN The Area Warden will maintain radio contact with the Chief Warden and provide updated situation reports (casualties, conditions, visual observations, unaccounted personnel, and other relevant information) as appropriate. STEP 3.4 CHIEF WARDEN DIRECTS AREA WARDEN TO EVACUATE PERSONNEL FROM SITE? The Chief Warden will direct the Area Warden as to when it is appropriate to commence evacuate from site. STEP 3.5A REMAIN AT MUSTER POINT The Area Warden and Deputy Area Warden(s) will ensure all personnel remain at the Muster Point (within the building) until further advice is received from the Chief Warden. STEP 3.5B CHIEF WARDEN DIRECTS SHELTER IN PLACE If NO refer Step 3.3 If YES refer to Step 2.6 STEP 3.6 AREA WARDENS GROUP IS READY TO EVACUATE? If YES the Area Warden will radio the Chief Warden to confirm that their group is ready to evacuate. Personnel will be directed to evacuate to their residence or alternate location Page 12 of 28

13 dependent on the characteristics of the ammonia gas plume as directed by the Chief Warden. If NO refer Step 3.3 STEP 3.7 AREA WARDEN EVACUATES GROUP FROM SITE Site evacuations will be coordinated in a staged manner so as to avoid traffic congestion. The Area Warden will take all reasonable measures to ensure appropriate vehicular resources are identified prior to evacuating (this will include assigning multiple personnel to vehicles if required). As each building is evacuated the Area Warden will post signs at the entry door(s) to the building indicating that an evacuation has occurred. The Area Warden will direct all personnel to drive in a safe manner and not disrupt traffic flow unnecessarily. The Area Warden will radio the Chief Warden with positive confirmation once their group has evacuated site. Page 13 of 28

14 1. INTRODUCTION This plan provides details of actions to be taken in the event of an unplanned release of anhydrous ammonia (liquid and gas) and potential health impacts. To facilitate effective and efficient emergency response to an unplanned anhydrous ammonia release (liquid or gas) this plan incorporates: Process flowcharts - for the direction of activities Supporting text clarification of roles, accountabilities and background information. 1.1 OBJECTIVES To ensure all PPA leaseholders and licensees are considered in emergency response activities. To ensure that PPA is in a state of readiness for an unplanned release of anhydrous ammonia that has the potential to impact PPA operations or lands. To ensure the timely notification of all personnel, vessels at berth and vessels within Dampier Port that may be impacted by anhydrous ammonia release. To ensure a return to normal operations as soon as possible after the all clear is given. 1.2 SCOPE The scope of this plan includes all personnel working on or visiting the PPA Landside Restricted Zone (LRZ), PPA lands, all vessels and their crews berthed alongside PPA facilities and vessels within the Port of Dampier in an area that may be impacted upon by an anhydrous ammonia release (gas or liquid). 1.3 DEFINITIONS Anhydrous Ammonia (NH 3 ): a colorless water soluble gas liquefied under pressure at a temperature of -33 o C, classified as a hazardous substance. Ammonia Gas: a colorless alkaline gas with a pungent odor and acrid taste that evolves as a result of heating anhydrous ammonia to decomposition (temperatures above -33 o C). PPA Lands: All non-security controlled, common access areas and land outside of the Maritime Security Zone (include access roads in the King Bay Industrial Estate). This includes lease areas in the King Bay Industrial Estate, the Safety & Security Services Building and the main access gates. PPA Landside Restricted Zone: Common areas and land inside of the Maritime Security Zone including the main access road, Dampier Cargo Wharf, Dampier Bulk Liquids Berth, Heavy Load Out Facility, Alternate Load Out Facility, barge ramp and lay down areas, PPA buildings, infrastructure and leased areas. 15 Emergency Planning Response Guideline Level 3 (ERPG-3): The maximum airborne concentration of ammonia below which it is believed that nearly all individuals could be exposed for up to one hour without experiencing or developing life threatening health effects. ERPG-3 equates to an ammonia concentration of 750ppm. Mission Mode: Multi channel urgent communication system utilized by Yara Pilbara to notify external stakeholders that may be impacted by an anhydrous ammonia release event (SMS, voice to mobile, voice to landline, and/or facsimile). Mitigating Measures: Refers to actions taken to minimize the potential health impacts arising from an anhydrous ammonia release (gas or liquid). These actions may take the form of sheltering in place, utilizing personal protective equipment (industrial escape hoods) and/or site evacuation. Parts Per Million (ppm): Concentration unit measure for anhydrous ammonia gas. Shelter in Place: A mitigating measure where shelter is sought in an enclosed structure, building, vessel or vehicle and undertaking all reasonable precautions with a view to minimizing the potential health impacts arising from an anhydrous ammonia release. 1.4 REFERENCE DOCUMENTS Australian Standard 3745 Planning for emergencies in facilities Australian Standard 2022 Anhydrous ammonia storage and handling 1.5 INTEGRATION WITH OTHER S This plan is integrated with and supported by the following documents: PPA Emergency Response Plan Yara Pilbara Fertilisers Plant: Emergency Management Plan Page 15 of 28

Generate a list of all personnel onsite and provide this to the Chief Warden. Unlock all security doors on activation of the emergency siren. Be readily available for the building(s)/designated area(s) allocated to them and respond immediately to any requests for first aid assistance. Provide first aid services in the event of emergency evacuations that require such actions. Follow the directions of the ECO Wardens. Assist visitors and contractors as required. Undertake mitigating measures in accordance with the ship s emergency plan. Undertake mitigating measures in accordance with their own emergency plan. Page 16 of 28

17 2. PIPELINE AND BULK LIQUIDS BERTH Yara Pilbara operates an anhydrous ammonia production plant located on the Burrup Peninsular, North West of Karratha. Yara Pilbara export liquefied anhydrous ammonia from the production facility via a 5.2km pipeline to a ship loading arm on the Bulk Liquids Berth. The pipeline contains anhydrous ammonia at all times. As such appropriate mitigating measures have been established in the event of an unplanned release of anhydrous ammonia (liquid or gas). 3. DETECTING AND COMMUNICATING AN AMMONIA GAS RELEASE 3.1 MOST CREDIBLE SCENARIO Using a risk based approach the mitigating measures described in this plan have been established around the maximum credible event for an ammonia gas release during ship loading / unloading activities on the Bulk Liquids Berth. In accordance with the Lloyds Register Ammonia Release & Dispersion Study (Jun 2010) a 50mm leak and spill on seawater has been used with a maximum plume range for Immediately Dangerous to Life and Health (IDLH) equivalent dose (average exposure of 300ppm for 30 minutes) of: 250m during daytime operations; and 1,050m during night time operations. 3.2 AMMONIA GAS DETECTION When anhydrous ammonia liquid is exposed to the atmosphere ammonia gas evolves as a result of heating and decomposition (temperatures above -33oC). An ammonia gas release may be detected through the following mechanisms: Ammonia gas detectors located at key positions within the Landside Restricted Zone (refer Appendix B Emergency Object Locations). The ammonia gas detectors are set at alarm at 20 parts per million (ppm) and the signals are repeated at the PPA and Yara Pilbara Control Rooms. Visual ammonia gas will form a white cloud on contact with the air due to the condensation of water vapour from the air. Odour ammonia gas is highly irritating with a sharp, pungent odour noticeable at levels ranging from 5 50ppm. 3.3 COMMUNICATION OF AN AMMONIA GAS RELEASE All known or suspected ammonia gas releases must be communicated to a PPA staff member and/or the Port Communications tower on , VHF 11 or UHF 17. Yara Pilbara will provide periodic updates regarding the nature and extent of the ammonia release event via MissionMode to the Vessel Traffic Services Officer. Page 17 of 28

18 Yara Pilbara is responsible for communicating the nature and scale of an ammonia release event to all PPA stakeholders with the potential to be impacted. The Chief Warden or delegate is responsible for communicating with leaseholders and licensee s while the Vessel Traffic Services Officer is responsible for communicating with marine stakeholders as directed by the Harbour Master or delegate. 3.4 ACTIVATION OF THE EMERGENCY SIRENS PPA have installed emergency sirens (oscillating air raid tone) that when activated indicate a possible anhydrous ammonia emergency. In the event of a suspected or confirmed anhydrous ammonia release the emergency siren will be manually activated. All emergency sirens will activate at the same time. Emergency siren can be manually activated at the following locations: Bulk Liquids Berth Control Room Vessel Tracking Services Centre Safety and Security Building Landside Operations and amenities Building Immediately following notification that there is a suspected or confirmed anhydrous ammonia emergency the Chief Warden will undertake all reasonable measures to ascertain the nature and extent of the ammonia gas release and provide advice to the ECO regarding appropriate mitigating measures. The emergency siren and public address system are tested weekly (every Tuesday at 9am). 3.5 PUBLIC ADDRESS NOTIFICATION Immediately following activation of the emergency siren the Chief Warden (or delegate) will task the VTSO or delegate to communicate the following message to all areas on site via the public address systems VHF 11 and UHF 17 two-way radio: This is an emergency, this is an emergency, this is an emergency. There is an ammonia gas release near [location]. Please take appropriate action. There is an ammonia gas release near [location]. Please take appropriate action. This message is repeated three (3) times. Once the emergency response has commenced the Chief Warden (or delegate) will notify and/or maintain contact with relevant emergency stakeholders including: Yara Pilbara Control Room (24 Hour Emergency Line) on Department of Fire and Emergency Services (DFES) on 000 Page 18 of 28

19 4. RESPONDING TO AN EMERGENCY SIREN Upon hearing the emergency siren all personnel must take mitigating measures in accordance with this plan. 4.1 FOR PERSONNEL INSIDE A BUILDING: Stay inside (Process Flow A step 1.8A). If shelter in place is required follow directions of Area Warden / Deputy Area Warden (Process Flow A step 1.8A). If muster is required follow directions of the Area Warden (Process Flow C step 3.1). 4.2 FOR PERSONNEL OUTSIDE: If safe to do so move immediately to the nearest PPA office building and shelter in place. Process Flow A step 1.3. If you cannot access a PPA office building safely shelter in place as practicable. If sheltering in a vehicle or mobile equipment close all windows and vents, turn off the air-conditioning and try to contact a PPA staff member to advise of your location and situation (If possible call the Vessel Traffic Services Officer on , VHF 11 or UHF 17 and advise of your location, number of persons and your contact number(s)), then await further instruction. 4.3 FOR VESSELS WITHIN PORT WATERS: Masters of ships calling at the Port of Dampier are advised that anhydrous ammonia (ammonia in its pure form-without water) is loaded at the Bulk Liquids Berth ( S E). Ammonia (UN number 1005) is classed as a toxic gas. While the likelihood of a release of ammonia gas in the port is very remote, masters of ships are reminded to have a plan to respond to a potential gas release. The port s emergency signal for an ammonia gas release is a siren (oscillating air raid tones). The siren is tested at 0900 every Tuesday. Masters of vessels alongside PPA berths and within port waters are responsible for the health and safety of their crews and should take appropriate action(s) in accordance with their own ship s emergency plan in the event of an anhydrous ammonia emergency. 4.4 MISSING PERSONS Area Wardens / Wardens will conduct roll calls and report to the Chief Warden who will maintain a master list of all personnel believed to be onsite. This list will be handed over to the appointed HMA as soon as practicable. Any responsibility to search for, identify and render assistance to missing persons sits with the HMA. Page 19 of 28

MITIGATING MEASURES In the event of an ammonia gas release there are two mitigating measures: Shelter in place Site evacuation The mitigating measure(s) undertaken will be decided and communicated by the Chief Warden dependent on the location, size, concentration and direction of travel of the ammonia gas plume. 5.1 SHELTER IN PLACE Shelter in place and taking mitigating measures is always the first response in an ammonia gas release event refer Process Flow B. Emergency Kits have been established to facilitate shelter in place actions and are located: Wharf Services Area Amenities Building (Crib Room) Vessel Traffic Services Centre (VTSC) Corporate Services Building Administration Building (Boardroom) Maintenance Building Safety & Security Services Building (First Aid Room). Each occupied PPA building has an Emergency Kit stocked as per the inventory list in Appendix C. The Area Warden / Warden will direct all personnel for the duration of the shelter period ensuring: Communications equipment is working. A head count is conducted (prior to proceeding to site evacuation) Shelter in place is completed in accordance with Appendix A (as directed by the Chief Warden) Updates on the progress of the emergency event are communicated to all personnel. Personnel will continue to shelter in place until notified that it is safe to evacuate the site by the Chief Warden Process Flow C. 5.2 EVACUATION A site evacuation will only commence when instructed by the Chief Warden Process Flow C. Site evacuation will be coordinated in a manner that as far as practicable ensures efficient egress of all personnel from the PPA Landside Restricted Zone Page 20 of 28

21 (LRZ). Area Wardens will take all reasonable measures to account for persons in their area or building (Process Flow C step 1.2) prior to evacuation. Evacuees may be issued with PPE (industrial escape hood or similar) by the Area Warden / Warden prior to evacuating site. The Area Warden will take all reasonable measures to ensure appropriate vehicular resources are identified prior to evacuating site As each area or building is evacuated (Process Flow C step 1.7) the Area Warden will post signs at the entry door(s) to the building indicating that an evacuation has occurred and directing remaining personnel to proceed offsite (if safe to do so) or shelter in place as practicable and call 000. Site evacuations will be coordinated in a staged manner so as to avoid traffic congestion. Personnel will be directed to evacuate to their residence, or alternate location dependent on the characteristics of the ammonia gas emergency as directed by the Chief Warden. 5.3 SECURITY AND TRAFFIC MANAGEMENT Immediately following activation of the emergency siren the Access Control Officer will permanently close the entry boom to the PPA LRZ, permanently open the exit boom and deactivate all security doors. A manifest of all personnel within the PPA LRZ will be generated from the access control system and the visitor sign in logbook will be prepared for handover to the Chief Warden. 6. VESSEL TRAFFIC SERVICES OFFICER S RESPONSE Upon hearing the emergency siren the Vessel Traffic Services Officer (VTSO) will immediately complete the actions outlined in the VTSO Anhydrous Ammonia Release checklist. Make contact with the Chief Warden to report the situation. As soon as practicable, the VTO should then shelter in their building and maintain contact with the ECO wardens. 7. ALL CLEAR When an anhydrous ammonia emergency no longer represents a danger, the Chief Warden or delegate will communicate the all clear status. This is done by switching the emergency siren to one long continuous tone followed by an announcement over the public address system. PPA LRZ lease areas not covered by the public address system will be contacted by phone or radio. 8. PERSONAL PROTECTION MEASURES 8.1 RESPIRATORY PROTECTION (INDUSTRIAL ESCAPE HOODS) Industrial escape hoods (respiratory protection devices) are a form of Personal Protective Equipment (PPE) located in each Emergency Kit refer Appendix C. Industrial escape hoods are an air purifying, full-face respirator equipped with a Page 21 of 28

22 chemical cartridge. The Chief Warden will advise if and when Industrial Escape Hoods are required. All leaseholders / licensees and other contracting groups who regularly visit the PPA LRZ are required to provide industrial escape hoods for their staff, contractors and visitors. The PPA will provide industrial escape hoods however these are intended for visitors and/or other personnel who do not have permanent operations onsite. 8.2 WIND DIRECTION An ammonia gas release may develop into a plume (cloud). The time of day, weather conditions and quantity of anhydrous ammonia will determine how visible a plume of ammonia gas will be. Personnel making their way to the nearest muster point or taking other mitigating measures should (if possible) make note of the wind direction using the wind socks. Taking note of the wind direction may assist in determining the direction that an ammonia gas plume is travelling. Windsocks are located at the Bulk Liquids Berth, Landside Service Area Car Park and the Administration Building Car Park refer Appendix B. Wherever possible, personnel should move across or upwind from the ammonia gas plume. Figure 1 below can be used as a guide to determining the wind direction and strength using a wind sock. Figure 8-1 Guide to Interpreting Wind Socks 8.3 FIRST AID MANAGEMENT Prompt action is essential if there is an ammonia release emergency. Trained First Aid representatives are directed to consult the Anhydrous Ammonia Material Safety Data Sheet for guidance Appendix C. 9. EMERGENCY CONTROL ORGANISATION An Emergency Control Organisation (ECO) has been established refer Figure 2 below to ensure PPA can respond appropriately in the event of an anhydrous ammonia emergency. In accordance with the Occupational Safety and Health Regulations 1996 the primary role of the ECO is to preserve the safety of all PPA personnel (staff, contractors and visitors). Page 22 of 28

23 PPA licensees and leaseholders similarly hold the primary responsibility for their personnel (staff, contractors and visitors) however the ECO will engage with these stakeholders during an event to synergise response activities.
