1 5.5 CD/14 (Warning & Advice Notice) General This form can be used to formally notify oil users and/or persons responsible for oil-fired equipment of potential and immediate risks associated with their installations. It should be issued alongside the OFTEC Warning Stickers, which are detailed in sub-section 7.1. CD/14 Form (Warning & Advice Notice) Diagram Edition Section 5 Page 12
2 CD/14 Completion Instructions 1. Insert an optional reference number. This is a number of your choice, which may be a unique customer identification number. 2. Insert the customer/site details, your name and your Technician OFTEC Registration Number. 3. Insert your company details, your Company OFTEC Registration Number and your company telephone number. 4. Insert details of issues identified with the associated oil installation. Next to each issue or group of issues, insert PR or IR in the Risk category box to indicate whether they are Potential Risks or Immediate Risks. For further information on how different risks are categorised, please see sub-section 5.2 and the OFTEC Procedures section of Technical Book Tick to indicate whether warning labels (i.e. OFTEC Warning Stickers) have been affixed to the associated equipment. 6. Identify the status of the person receiving the notice, insert details accordingly and obtain their signature. 7. Insert your signature; along with the date and time that the notice was issued. 3 5.6 TI/133D (Domestic Oil Storage and Fire Risk Assessment) General Protection of the environment and property is vital. Although oil is an intrinsically safe fuel, the TI/133D has been provided to aid a competent person to assess the environmental and fire risks associated with domestic fuel storage facilities. It should be used prior to any installation work commencing. It can also be used to identify and record risks regarding existing installations. This assessment can be used to decide if an oil storage tank needs to be provided with secondary containment and/or fire protection. Provision of secondary containment may be achieved by an integrally bunded tank or the construction of a masonry or concrete bund to CIRIA Report 163, around a single skinned tank. Provision of fire protection may be achieved by the use of third-party approved composite fire rated barriers or by construction of masonry walls. For guidance on approximate fire ratings of domestic buildings and structures, please refer to Section 1, sub-section For domestic dwellings, BS 5410 Part 1 applies to an oil tank installation with a capacity not greater than 3,500 litres. Every installation will have special features, and they must be assessed on each occasion as to their likely effect on both the risk of spillage and risk of spread of fire to the contained fuel. To comply with regional requirements, the undertaking of a risk assessment is usually required for the purpose of determining whether secondary containment can be omitted. However, some regional requirements (e.g. Jersey) prohibit the omission of secondary containment. In those cases part 3 (Fire Hazard) of the TI/133D form can still be utilised (see Regional requirements at the back of this publication). Parts 2 and 3 of the TI/133D form list the potential hazards, which require checking. Regional requirements, such as The Control of Pollution (Oil Storage) (England) Regulations 2001, require that non-domestic properties with storage above 200 litres are to be provided with secondary containment (see Regional requirements at the back of this publication) It is important to provide the top copy of the Risk Assessment to the owner of the oil storage tank, and retain the bottom copy for your records. 5 TI/133D Completion Instructions 1. Record your company name and telephone number. 2. Record the owner of the property s name and signature (if available). 3. Record date of assessment. 4. Record full site address details (this is where the oil storage tank is being installed). 5. Record the full address details of the owner of the property, if different from number 6 above. NOTE 1 on TI/133D Where the owners name and address is different from the site address, it is necessary to record the owners name and address in this box as it is the owner who is the responsible person under law. 6. Environmental hazards that, if ticked yes, will require the oil storage tank to have secondary containment (bunding). NOTE 2 on TI/133D Refer to Section 1, sub-section 1.5 and the Regional Requirements at the back of this publication. 6.1 Identify the capacity of the oil storage tank. 6.2 Identify whether the oil storage tank is located within 10m of controlled water (rivers streams, lakes, canals, coastal waters, estuaries, groundwater. this means all water which is below the surface of the Ground in the saturation zone and in direct contact with the ground or subsoil, and any ditches soakaways, septic tanks and gullies which could pollute groundwater or reach controlled water through groundwater). 6.3 Identify whether the oil storage tank is located in a position where, if there was an oil spill or leak, could the oil storage tanks contents reach open drains of loose fitting manhole covers. (Not sealed or screwed down manhole covers). 6.4 Identify whether the oil storage tank is located within 50m of a borehole or spring (including wells). For information on the location of water extraction sites (which are not visible) contact the local authority and regional environmental authority. 6.5 Identify whether the oil storage tank is to be located/already located over hard ground or hard surfaced ground which could enable spillage run off to reach controlled water. For example where an oil tank is installed over a hard paved, concrete or tarmac drive which slopes down to a road or has its own surface water drainage system. 6.6 Identify whether the vent pipe of the oil storage tank can be seen from the fill point. The fill point is where the hose from the tanker enters the oil storage tank or associated pipework (e.g. extended fill pipes). 6.7 Identify whether the oil storage tank is supplying heating oil to another building other than a single family dwelling (e.g. on site offices) Edition Section 5 Page 16
6 6.8 Identify whether the oil storage tank is located within an England and Wales Source Protection Zone 1. This is a position where the tank is either less than 50m from a borehole, or where an oil spill or leakage could travel to a borehole within 50 days. To view a map of where Source Protection Zones (SPZs) exist in your region, please visit: Identify any other potential hazards that you feel would constitute a pollution risk. NOTE 3 on TI/133D Additional enquiries may be necessary to regional environmental agencies to identify hidden or unseen potential environmental hazards, i.e. where the site is located over a ground water source such as an aquifer. 7. Identify whether the tank is located in flood risk area or is exposed to high wind. 8. Fire hazards that if ticked yes will require fire protection of no less then 30 minutes extending 300mm longer then each side of the oil storage tank and 300mm higher then the highest part of the oil storage tank. NOTE 4 on TI/133D Refer to Section 1, sub-section 1.6 and the Regional Requirements at the back of this publication. 8.1 If it has been identified that the oil storage tank is or has been located closer then 1.8m to a non-fire rated wall, fire protection will be required. 8.2 If it has been identified that the oil storage tank is or has been located closer then 760mm to a non-fire rated boundary, fire protection will be required. 8.3 If it has been identified that the oil storage tank is or has been located closer then 1.8m to non-fire rated eaves, fire protection will be required. 8.4 If it has been identified that the oil storage tank is or has been located closer then 1.8m to any windows or doors, fire protection will be required. 8.5 If it has been identified that the oil storage tank is or has been located closer then 1.8m to a flue terminal (in any direction), fire protection will be required. 8.6 If it has been identified that the oil storage tank is or has been located on a base which does not extend 300mm around the perimeter of the tank. The oil storage tank base will require extending to 300mm around the perimeter of the tank (see note 6). NOTE 6 on TI/133D Except where the side and/or end of the base abuts a fire rated structure, such as a building, boundary or barrier. In these circumstances the base need only extend 100mm from the tank to the fire rated structure. Any exposed sides or ends should still be extended by 300mm. For further information refer to Section 1, sub-section 1.6 of this publication. 9. Record name of the OFTEC Registered Technician completing the Risk Assessment and their OFTEC Registration number (this begins with a letter and then 5 digits and then OFTEC qualifications). The OFTEC Registered Technician should then sign and date the Risk Assessment. Page 17 Section Edition
7 5.7 TI/133ND (Non-Domestic Oil Storage Risk Assessment) General This form can be used to determine whether a non-domestic oil storage tank requires fire protection measures to be implemented in order to comply with the requirements of BS For guidance on approximate fire ratings of buildings materials and structures, please refer to Section 3 of this publication. If YES is selected action 1 or 2 is required. If NO is selected then there are no hazards identified. 7.2 Identify whether the oil storage tank is located within 2m of a non-fire rated wall of a building where the fire rated integrity is less than 60 minutes. If YES is selected then action 1 is required. If NO is selected then there are no hazards identified. 8. Identify the capacity of the oil storage tank. 8.1 Identify whether the oil storage tank is located within 6m of a non-fire rated boundary where the fire rated integrity is less than 120 minutes. If YES is selected action 3 or 4 is required. If NO is selected then there are no hazards identified. 8.2 Identify whether the oil storage tank is located within 6m of a non-fire rated wall of a building where the fire rated integrity is less than 120 minutes. If YES is selected then action 3 is required. If NO is selected then there are no hazards identified. 9. Identify whether the tank base conforms to industry recommendations and whether the tank has been provided secondary containment in accordance with regional requirements. Also, record any comments or observations regarding the installation/condition of the tank. 10. Record name of the OFTEC Registered Technician completing the Risk Assessment and their OFTEC Registration number (this begins with a letter and then 5 digits and then OFTEC qualifications). The OFTEC Registered Technician should then sign and date the Risk Assessment Edition Section 5 Page 20
10 5.8 Work Notification England and Wales General Changes to the Building Regulations in England and Wales mean that since April 1 st 2005, Local Authority Building Control must be notified of any oil fired appliance, oil storage and supply system installation and commissioning works undertaken and self-certificated by OFTEC Registered Businesses and Technicians. Furthermore, building owners must receive a certificate of any works undertaken in their home/premises, and that they comply with the Building regulations in force on the date the works were completed. OFTEC registered businesses can notify their works to OFTEC, and OFTEC will inform the relevant Local Authority and provide a compliance certificate to the householder on the businesses behalf. For work notification in other regions, refer to the Regional requirements at the back of this publication Work Notification OFTEC s Works Notification Scheme not only meets legal requirements, but is an added benefit that OFTEC Registered Technicians can offer their customers. The only other alternative means of compliance is to arrange a Local Authority Building Control Inspection for any works subject to Building Regulations, which can be costly and time consuming. OFTEC offer two means of notifying work, either, registering work online (see sub-section 5.7.3) or by using the Building Regulations Work Notification Fax form (see sub-section 5.7.4). Please contact OFTEC s Registration department for the latest options of performing Work Notification How to Register your Works Online When notifying your works via the internet you will need to log into the Registered Technician s area of the OFTEC website ( Use your password to access Work Notifications. You can then: Add a new Work Notification. View Notification history. Change your password. To add a new Notification you will need to input the following: Job completion date (once entered this cannot be amended). The address where the works were carried out (there is an option of providing different dispatch address, if different to the works address. For new build OFTEC will require details of the relevant Local Authority). At this point you can also add your own unique job reference number. Once you have entered the site details, you will see a summary screen, click on the Add Work Details button to add work activity details. Page 21 Section Edition
11 To add the job details you will need to complete the following. Tick the box next to the name of each technician that worked on the job. Choose the work activity from the drop down list of notifiable jobs. Select the location (e.g. Boiler Room). Select the quantity of the type of work. Click save to save details of work and return to the summary screen. If you experience any problems during the online notification process simply return to the summary screen and mark the record as invalid. This will prevent the job being notified to Local Authority Building Control and there will not be a charge. Job details can be amended at any time up to the point where the Local Authority is notified. However, the date cannot be amended, so if this is incorrect mark the notification as invalid to prevent it being reported and start another record with the correct details How to Register your Works by Fax An updated version of the Building Regulations Work Notification Fax is available from the OFTEC website ( or can be obtained from OFTEC on Once you have a master copy, this can be photocopied as many times as needed. The Building Regulations Work Notification Fax is spilt up into sections (1-4) which consist of a series of boxes. You should only complete the sections which correspond with the works you have completed and for which you hold the relevant class of OFTEC Registration Edition Section 5 Page 22

Record a telephone number where you can be contacted if a query with the Building Regulations Work Notification arises. 3. List the individual OFTEC registration numbers of the registered technicians that are employed by the Registered Business concerned (see item 4) and who undertook the notifiable work they are going to report. 4. Record your OFTEC Business Registration number (this begins with a C ). Page 23 Section Edition
13 5. Record a contact name of the person in your company responsible for the Building Regulations Works Notification. 6. Record the postal address as listed with the Royal Mail where the works were carried out. Please ensure you provide the postcode, a contact name and telephone number for the person responsible for the property. If property is new build, tick box and record the name of the Local Authority concerned. 7. Compliance Certificates produced for oil, heating, hot water and electrical work selfcertified under Schedule 3 of The Building Regulations 2010 are automatically sent to the property where the work is carried out. If you require a duplicate certificate, please call OFTEC Registration Services on Tel to order following the notification being made. You will need your OFTEC Company number and address/postcode of the property ready when calling. 8. The date when the works were fully completed. 9. If you have your own unique reference/invoice number for the works, you can enter it in this section so notification is easily recognisable for you. This is optional. 10. Section 4 lists the oil work types. You are required to put the quantity (if any) next to each item of work you have completed. E.g. If you have installed a new boiler and heating system, you are required to notify both the boiler and the system which can be done on the same submission Registered Technicians holding the OFT 105E qualification can notify the following works: Install an open flue pressure jet appliance or conversion. Install a room sealed balanced flue pressure jet appliance. Install an open flue vaporising appliance or conversion. Install a fan assisted vaporising appliance. Install/alter/commission wet central heating system and associated controls. Install/alter/commission dry central heating system and associated controls. Install and commission a flue liner/chimney system. Install and commission a vented hot water storage system Registered Technicians holding the OFT 600 qualification can notify the following works: Install an oil storage tank. Install oil supply pipework Registered Technicians holding the OFT 101 qualification can notify the following works: Commission a pressure jet appliance installation 70kW. 14 10.4 Registered Technicians holding the OFT 102 qualification can notify the following works: Commission a vaporising appliance installation (Dry). Commission a vaporising appliance installation (Wet) Registered Technicians holding the OFT 107 qualification can notify the following works: Install and commission unvented hot water storage system Registered Technicians holding the OFT 201 qualification can notify the following works: Commission a pressure jet appliance installation 70kW. Commission a pressure jet appliance installation > 70kW. 11. If electrical installation work is to be notified, the name and registration number of the appropriately qualified technician (i.e. holding OFT 103 Full Scope Part P registration) should be entered here. 12. Section 3a lists the electrical work types associated with oil fired equipment. You are required to put the quantity (if any) next to each item of work you have completed Technicians holding the OFT 103 qualification can notify the following works via OFTEC: Replace a consumer unit. Partial rewire. Installation of a generator. Upgrade or alteration to means of earthing. Circuit alteration or addition in a kitchen or special location. One or more circuits. New Installation (new build). Lighting/Power outdoors. Rewire of all circuits. Control wiring including that of fire, security, heating, cooling or ventilation systems. ELV Lighting within the Building. Electric Floor or Ceiling Heating System. Reference should be made to the Regional requirements at the back of this book for further guidance on which electrical works do and do not require notification. 13. You need to sign the form to declare that the works you are notifying have been carried out in accordance with Local Building Regulations. Without a signature, the notification will not be processed. 14. The completed form is to be faxed back to OFTEC on Check before sending that the form has been completed fully.
