3 Introduction Program applicants are required to demonstrate procurement transparency to be eligible for funding. This template is a tool for eligible retirement homes to use in obtaining three requests for quotation ( RFQ ) from licensed fire sprinkler installers ( bidders ). It enables consistent information that facilitates transparency, clarity and appropriate detail for decision making. Operators may modify the template or request their own counsel as required, at their own cost. 4 RFQ Overview The Ministry of Seniors Affairs (MSA) is providing grant funding to small or rural retirement homes that are not otherwise able to afford the cost of installing automatic fire sprinklers in their homes. Installation is required by January 1, 2019, pursuant to the Ontario Fire Code. This RFQ is for the installation of automatic fire sprinklers in retirement homes. Given the technical nature of this work, this RFQ template will facilitate consistency in bids to help guide a well informed decision by the retirement home. Retirement home operators seeking a grant can use this RFQ template or a comparable document and obtain at least three RFQs from vendors. All three RFQs must be provided to MSA as part of the retirement home s grant application. The retirement home operator will recommend the bidder, subject to MSA s review and approval. Upon MSA review and approval, the successful bidder is approved. Guidance for Retirement Homes to help complete this RFQ template is set out in Appendix A. It is strongly suggested that you read appendix A as you complete this RFQ Template. The retirement home operator and successful bidder should complete a Canadian Construction Committee Documents (CCDC) template agreement number 2, to govern the terms of agreed upon installation work. The successful bidder should provide the CCDC 2 agreement to the retirement home for its review and consideration. The RFQ package should include the retirement home s site drawings, floor plans and asbestos report (If any) as an appendix prior to forwarding to bidders. These are typically part of your fire safety plan. If an innovative approach is proposed, bidders should attach a schedule to outline any alternative solution proposed that would vary from the Ontario Fire Marshal (OFM) mandated steps. This allows for innovative ideas on behalf of the bidders. The retirement home will recommend the successful bid to MSA. Upon approval, the retirement home will receive grant funding from MSA to provide a portion of the cost for the retrofit and will enter into an agreement for the work to be performed by the bidder. The successful bidder should provide executed copies of a CCDC 2 agreement and a CCDC 11 or comparable documents, providing references of similar work. 4

5 Successful Bidder s Obligations The successful bidder is expected to do the following: 1. Complete a CCDC 11 (Contractor s Qualification Statement) and attach it with this RFQ, 2. Provide a CCDC 2 (Stipulated Price Contract) template for the retirement home s review and consideration as the contract for the performance of the installation work, 3. Maintain a high level of tenant and staff safety and security throughout their time on the property, 4. Complete the work in a timely manner, 5. Provide the retirement home operator with a material test certificate, 6. Ensure that the work concludes with building permit closure, and 7. Submit two copies of all the required documents, listed above, to the retirement home for its records and submission to the MSA. The successful bidder s work will be inspected by various provincial regulators for completion, accuracy and compliance, and may be inspected by MSA. Requirements of Bidder And Quotation Information The successful bidder must employ installers that are a member in good standing of the Ontario College of Trades (OCOT). In addition, the successful bidder must ensure: 1. That the installer is a journeyman or apprentice member in good standing of OCOT. The material test certificate must indicate the installer s OCOT license number, and their Certificate of Qualification number. 2. If any apprentices work on the project, they must also provide their appropriate OCOT and apprenticeship registration information. 3. Any relevant fire alarm work must be completed by registered Canadian Fire Alarm Association (CFAA) fire alarm technicians and /or OCOT licensed electricians. In addition to traditional fire sprinkler installation, bidders will be required to provide a price which includes all of the following elements: 1. Engineering, 2. Water supply improvements necessary to meet the fire sprinkler demand, 3. Costs related to applying for a building permit, and 4. Costs related to final inspections and commissioning. 5

6 Included Work in This Project Each bid must consider the following elements in its proposed work: 1. Fire sprinkler design and hydraulic calculations, drawings must be stamped and submitted to the local authority having jurisdiction and the retirement home operator for permit approval, 2. Any necessary improvements to the water service to meet the demands of the fire sprinkler system, 3. The installation of the fire sprinkler system, 4. The necessary remediation of the property in order meet all applicable listings, standards and codes, 5. Completion of an Installer s material test certificate, for the retirement home to submit to MSA, 6. Completion of the building permit closeout with the local authority having jurisdiction, for the retirement home to submit with all related documents, to MSA. Excluded Work Cosmetic remediation is not a component of fire safety and shall not be considered. The quoted prices the bidder lists on the attached document will represent the entire cost to perform the services outlined in this RFQ; this includes all fees, permits, taxes, and any other costs, including fuel surcharges associated with performing the services in accordance with this specification. Scope of Work The required work is the installation of a sprinkler system in a care occupancy as set forth in Ontario Regulation 150/13, under Fire Protection and Prevention Act, 1997, S.O. 1997, c. 4. Sprinkler systems (1) An automatic sprinkler system shall be installed in each building in accordance with NFPA 13, Standard for the Installation of Sprinkler Systems. (2) Despite Sentence (1), in a building not greater than 3 storeys in building height that provides sleeping accommodation for not more than 10 persons, a sprinkler system may be installed in accordance with NFPA 13D, Standard for the Installation of Sprinkler Systems in One- and Two- Family Dwellings and Manufactured Homes, except that the minimum quantity of available water shall be adequate for a demand duration of at least 20 minutes if stored water is used as the sole water supply source. (3) Despite Sentence (1), in a building not greater than 6 storeys in building height, a sprinkler 6

7 system may be installed in accordance with NFPA 13R, Standard for the Installation of Sprinkler Systems in Residential Occupancies up to and Including Four Stories in Height. (4) An existing sprinkler system that provides an average sprinkler discharge density over a design area that is equal to the minimum density corresponding to the area for light hazard classification as defined in NFPA 13, Standard for the Installation of Sprinkler Systems, is deemed to be in compliance with Sentence (1). (5) Sprinkler systems shall be equipped with local electric waterflow alarms. (6) If a fire pump is required by NFPA 13 or NFPA 13R, it shall be installed in accordance with NFPA 20, Installation of Stationary Pumps for Fire Protection, if it has a rated net head pressure greater than 280 kpa. This work must be competitively bid by the retirement home in order to meet program requirements. 8 Pricing The following table details the pricing for delivery of the services outlined in this proposal. 9 Appendix A: Guidance for Licenced Retirement Homes This RFQ is a template for your procurement of three quotations for the completion of the installation of fire sprinklers in the home. MSA requires you to obtain three quotations, and submit them with your application for grant funding, along with the notation RECOMMENDED on the quote you wish to proceed with. 1) Ensure that you complete all of the boxes on page 1 (cover). You can add the site visit dates before you submit this to bidders, or you can write Upon Request, if you prefer potential bidders to contact you and set up a time to visit your site. 2) The phone number you provide should be a number that bidders can reach you easily during regular business hours. 3) If you have site drawings showing the layout of the building, attach these to this RFQ as an appendix to help the bidders. These site drawings should be in your fire safety plan. 4) If you are aware of asbestos in the premises, please provide a report with the RFQ as an appendix. 5) Mark the bid you recommend with RECOMMENDED. 6) Ensure that all bids are forwarded to MSA for review. 7) If you choose to proceed with the work of the successful bidder, you must enter into an agreement with the bidder. The bidder should provide for your review the CCDC2 and CCDC 11. 8) Copies of the executed CCDC 2 and CCDC 11 or comparable documents, engineered stamped design drawing and hydraulic calculation shall be forwarded to the ministry for review. 9
