EFI was hired to determine the cause and origin of fire. Mr. Shaver worked closely with the insurance company Special Investigation Unit (SIU) through site investigation and interviews with witnesses to determine the cause of the fire. It was determined that while cleaning the vehicle, the propane stove burner was left on and unattended, igniting cleaning rags in close proximity. The leasee discovered the fire and attempted to utilize another towel to extinguish the fire. Industrial wood chipper fire Fire origin and Cause Investigation EFI represented the insurance carrier who insured the business owner. EFI was hired to determine the cause and origin of fire. Mr. Shaver worked closely with the insurance company through site investigation and interviews with witnesses to determine the cause of the fire. It was determined that employees were conducting hot work on the machinery and discarded used welding rods into the wood chips under the machine, which ultimately ignited the wood chips.
