The Extended Service Agreement (ESA) provides PARTS and LABOR coverage for: Residential Equipment o New equipment coverage up to 0 years after installation. o Equipment is considered new up to year after installation and is eligible for Extended Warranty Coverage during that time. Dealer shall submit order with End User information (name, address, and equipment information) to Trinity for processing. 8. There is no overtime or holiday rate. If there is a discrepancy, the Dealer is to notify Trinity immediately. Failure to notify Trinity may negate coverage in the future. Dealer agrees to provide a copy of the ESA to the end-user. 0. Any use of the Trinity Warranty name in any sales or marketing program (including but not limited to direct mailing, brochures, or advertisements) must first have written approval from Trinity. 4. 5. 6. Any ESA sold by the Dealer to an End User remains your customer to service as long as the Dealer remains in business or in the event that Trinity discovers fraud or misrepresentations on the part of the Dealer. 2. In the event that Trinity discovers fraud or misrepresentation on the part of the Dealer, Trinity shall promptly notify the Dealer of its evidence and findings. Upon notice, Trinity may take such actions as reasonable and necessary including, but not limited to, requiring the dealer to immediately terminate offering the Trinity Warranty, conduct an accounting review of the Dealer s records, or terminate this Dealer Agreement. Claims submitted to Trinity by the Dealer shall represent services actually performed by the Dealer on the equipment listed on the Trinity ESA. 7. Dealer shall have the right to refuse any service calls. Any written modification to the information listed above shall not be effective or accepted without prior written consent of Trinity Warranty. Claim Procedure You have 60 days from the date of failure to submit your claim. To file a claim, please fill out the enclosed Claim Form and submit it to Trinity Warranty via mail, email or fax for reimbursement. Residential equipment is defined as single family dwellings / condominiums / apartments.. Repairs Include... Mechanical failures of covered equipment during normal business operations. You must guarantee labor for ninety (90) days on all repairs performed within the terms of the ESA. Parts are inclusive of the OEM Warranty, parts process allowance is $35.00 per part replaced for residential. With companion repairs, the parts process allowance can be claimed on the primary part only. The refrigerant reimbursement is at dealer cost and is up to $8.00 per pound for R40A. Refrigerant is only reimbursed for leak repairs or burn-outs. Proof of costs is required for refrigerant and for any parts over $00. hour reclaim/recovery when opening system. 2. Exclusions Repairs as a result of installation error. Repairs resulting in a lack of proper maintenance. Acts of God (Earthquake, flood, lighting, hurricane, etc), war and terrorism.

Multiple and Companion Repairs Multiple Repairs Multiple repair codes allow for the highest cost repair to be applied first at 00%, then the second repair is at 75%, and any additional repairs on the same ticket are 50% of the applicable repair code. Companion Repairs Companion repairs are claims that are submitted as multiple repairs when they should be considered one () repair. We refer to these as companion repairs, as they are in fact part of the same failure. The examples below are considered for claim purposes to be one () repair.
