Your first kit is free, the second is available at a $5.00 co-pay. Seniors may receive 2 kits free, with any additional kits available for a $5.00 co-pay. Limit one at a time. Fill up your kit at your own convenience. When it is full, simply mail it back using the pre-paid postage label that is included in the kit.
