HS&S Alert No: 103 Date: 9 March 2017 RECENT CABIN FIRES WALL MOUNTED DOWN FLOW HEATER UNITS This is an update to Alert No 83, following a further cabin fire involving a wall mounted downflow heater unit. Prior to the recent A-Plant acquisition (2016) of our cabin fleet, GT Plant had changed the specification of the heaters to be fitted in all drying rooms and welfare units because of a number of similar incidents. However, it has been identified that some of the cabins currently being delivered to our projects by A-Plant still have the down- flow heater units fitted. Incident A wall mounted down-flow heater (Hyco-DF20) located in a drying room caught fire. This is believed to have been caused due to an overheating/faulty fan motor. The generated heat was enough to ignite the outer plastic casing. The appliance was fitted with a thermostatic safety cut-off device, however, this failed to isolate the electrical supply and the unit caught fire as a result. The fire caused significant damage to the appliance, as well as smoke/heat damage to the wall and ceiling of the cabin. The manufacturer, HYCO, stated that this particular model is best suited for occasional use in a domestic style setting i.e. bathrooms and shower rooms. In accordance with GallifordTry specification A-Plant drying rooms and toilets should be fitted with either Oil filled radiators or "Hyco Maestro MAC3X curtain heaters (shown below). Oil Filled Hyco Maestro MAC3X Curtain Heater Outcome All site teams are required to undertake an immediate inspection of the current heaters fitted within such areas. If the domestic down-flow type heaters are currently in use the site team must call A-Plant on 01332226220 to report the issue and request a replacement unit/s. A-Plant are aware of this issue and should organise replacement units in a timely manner. Until such a time as any down-flow heaters are changed to the correct specification, remember that down-flow heaters are NOT designed to be left on 24/7 or when site is unattended. Until the replacement units are fitted, you should appoint a member of the site team to be responsible for checking regularly and ensuring such heaters are not left on overnight. All heaters should be checked as part of the site weekly inspection routine; any wall heater that is found to show signs of damage or evidence of overheating should be turned off and isolated at the distribution board and reported to A-Plant immediately. The instruction manual for the MAC3X is appended to this alert and you should ensure that you familiarise yourself with the contents and practical safety measures. Maestro 3kW Air Curtain MAC3X 1. INTRODUCTION Thank you for purchasing a Hyco Maestro air curtain heater. With its three heat settings, including fan only, the Maestro air curtain provides heating for all conditions throughout the year. 2

2. IMPORTANT SAFETY POINTS THE APPLIANCE MUST ONLY BE CONNECTED TO A SINGLE PHASE SUPPLY. THE PRODUCT SHOULD ONLY BE CONNECTED TO A SUPPLY THAT MEETS THE SPECIFICATIONS DETAILED ON THE RATING LABEL. THE APPLIANCE IS NOT INTENDED FOR USE BY PERSONS (INCLUDING CHILDREN) WITH REDUCED PHYSICAL, SENSORY OR MENTAL CAPABILITIES, OR LACK OF EXPERIENCE AND KNOWLEDGE, UNLESS THEY HAVE BEEN GIVEN SUPERVISION OR INSTRUCTION CONCERNING THE SAFE USE OF THE APPLIANCE BY A PERSON RESPONSIBLE FOR THEIR SAFETY. DO NOT USE THE HEATER IF THE POWER CORD SHOWS ANY SIGN OF DAMAGE. CHILDREN SHOULD BE SUPERVISED TO ENSURE THEY DO NOT PLAY WITH THE APPLIANCE. NOT SUITABLE FOR OUTDOOR USE. WARNING: IN ORDER TO AVOID OVERHEATING, DO NOT COVER THE HEATER. THE SURFACE OF THIS APPLIANCE CAN BECOME HOT; NEVER LEAVE VERY YOUNG CHILDREN OR VERY VULNERABLE PEOPLE UNSUPERVISED WITH THE HEATER. CHILDREN AND VULNERABLE PEOPLE SHALL NOT BE ALLOWED TO CLEAN OR PERFORM MAINTENANCE OF THE HEATER UNSUPERVISED. 3

3. INSTALLATION SELECT A SUITABLE SURFACE TO MOUNT THE HEATER ON. THE HEATER MUST NOT BE LOCATED IMMEDIATELY BELOW A SOCKET. THE HEATER MUST BE MOUNTED AT LEAST 1 METRE AWAY FROM ANY COMBUSTIBLE MATERIALS SUCH AS FURNITURE, CURTAINS, BEDDING ETC... WARNING: THIS HEATER IS NOT EQUIPPED WITH A DEVICE TO CONTROL THE ROOM TEMPERATURE. DO NOT USE THE HEATER IN SMALL ROOMS WHEN THEY ARE OCCUPIED BY PERSONS NOT CAPABLE OF LEAVING THE ROOM ON THEIR OWN, UNLESS CONSTANT SUPERVISION IS PROVIDED. 11 Remove the four screws and lift away the air inlet cover as shown below. 4

22 Loosen the bolts retaining the bracket inside the heater and remove the bracket from the heater completely. ENSURE THERE ARE NO HIDDEN CABLES OR PIPES BEFORE COMMENCEMENT OF DRILLING. 33 Choose the final mounting position of the heater. Consider the orientation required for the heater to ensure it will adhere to all previous warnings. CEILING WALL WALL WALL 5

44 Ensuring a horizontal level, offer up the bracket and mark the mounting holes onto the surface. Drill the marked holes and insert the wall plugs before screwing the bracket firmly to the surface (ensure the bracket is the correct way up as per the diagram). 55 Offer up the heater to the bracket and locate the mounting bolts into the bracket lugs. Tighten the lugs enough to aid the orientation of the heater before tightening them firmly to prevent any future movement. Finally replace the air inlet cover on the front of the heater. 6

4. ELECTRICAL CONNECTION ELECTRICAL INSTALLATION MUST BE CARRIED OUT BY A QUALIFIED ELECTRICIAN IN ACCORDANCE WITH THE LATEST EDITION OF THE IEE WIRING REGULATIONS. IF THE HEATER IS TO BE CONNECTED TO THE FIXED WIRING OF THE PROPERTY THEN INSTALLATION MUST INCLUDE A DOUBLE POLE ISOLATING SWITCH WITH A BREAK CONTACT DISTANCE OF AT LEAST 3MM. Electrical connections should be made as follows: Green/Yellow earth wire to the terminal marked E or Brown/Red live wire to the terminal marked L Blue/Black neutral wire to the terminal marked N 7

6. CLEANING & MAINTENANCE BEFORE CLEANING AND MAINTENANCE ALWAYS ENSURE THE UNIT IS DISCONNECTED FROM THE POWER SUPPLY AND COOLED COMPLETELY. Use a vacuum cleaner to clear the inlet and outlet vents of any dust or debris. To clean the surface simply wipe with a clean damp cloth. Do not use any harsh or abrasive cleaning chemicals. 7. THERMAL PROTECTION The heater is fitted with two levels of overheat protection. An auto resetting thermal cut-out will disable the heating element in the first instance of an over-heat, this will automatically reset upon the heater cooling to a safe temperature. The second level of safety against over heat is a thermal fuse which will not reset and cannot be replaced. For technical faults contact Hyco on 01924 225200. 9

9. GUARANTEE AND SERVICE POLICY This product is guaranteed against faulty materials and manufacture for a period of one year from the date of purchase. Hyco will in its sole discretion replace, repair or refund any faulty unit. Defects arising from incorrect installation are excluded. Consequential costs such as labour charges or damage to fittings and surroundings are expressly excluded. 11

INFORMATION FOR CORRECT DISPOSAL OF THE PRODUCT IN ACCORDANCE WITH THE EUROPEAN DIRECTIVE 2002/96/E. At the end of its working life this equipment must not be disposed of as household waste. It must be taken to a local authority waste collection centre or to a dealer providing this service. Disposing of electrical and electronic equipment separately enables its components to be recovered and recycled to obtain significant savings in energy and resources. In order to underline the duty to dispose of this equipment separately, the product is marked with a crossed out dustbin.
