Affirmative responses to all five questions indicate the need for a home. If there are negative responses, but clinical suspicion of mold exposure remains, complete Part. If there are negative responses, and no clinical suspicion remains, do not recommend a home Part :. A majority of RED responses indicates the need for a home. If a majority of responses are not RED, but clinical suspicion remains, the health care provider may still recommend a home.
