Professional services and facilities The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. Basic professional services, overhead and basic use of facilities $2,350 Transfer of remains to funeral home $395 Additional mileage charge beyond 35 mile radius of chapel (per mile) $2.50 Professional embalming $550 Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. Memorial photo tribute includes the use of specialized DVD and large screen television equipment in chapel. The creation of the photo tribute by Jay Chapel staff and specialized software fee is $50.00. If a DVD is prepared by others, it will require review for compatibility with our system by the Jay Chapel Funeral Director. PERSONALIZED SERVICE PLANS PREMIER FUNERAL SERVICE PLAN Church or chapel service including evening service and graveside service $6,235 This service includes the services of the funeral director, assistants, and administrative staff for the necessary clerical work, arranging, coordinating and directing the services. Transportation of the deceased from the place of death to the mortuary (within 35 miles), professional embalming, dressing and casketing. Use of facility for viewing (one day), evening service, church or chapel service and graveside service. Automotive equipment includes the funeral coach, limousine for pallbearers, limousine for family and floral van. CLASSIC FUNERAL SERVICE PLAN $5,785 Church or chapel service and graveside service This service includes the services of the funeral director, assistants, and administrative staff for the necessary clerical work, arranging, coordinating and directing the services. Transportation of the deceased from the place of death to the mortuary (within 35 miles), professional embalming, dressing and casketing. Use of facility for viewing (one day), church or chapel service and graveside service. Automotive equipment includes the funeral coach, limousine for pallbearers, limousine for family and floral van. GRAVESIDE SERVICE PLAN $5,220 This service includes the services of the funeral director, assistants, and administrative staff for the necessary clerical work, arranging, coordinating and directing the services. Transportation of the deceased from the place of death to the mortuary (within 35 miles), professional embalming, dressing and casketing. Use of facility for viewing (one day) and graveside service. Automotive equipment includes the funeral coach and floral van. MEMORIAL SERVICE PLAN $4,195 This service includes the services of the funeral director, assistants, and administrative staff for the necessary clerical work, arranging, coordinating and directing the services. Transportation of the deceased from the place of death to the mortuary (within 35 miles), sanitary care of the remains and use of special refrigerated holding facility for two days. Use of facility for memorial service in chapel, or setup at the church or other location. Automotive equipment includes the service utility vehicle for transportation to the crematory in Fresno. The service plans listed above do not include merchandise selected such as a casket or alternative container, memorial supplies, urn or cash advances. 4
OTHER SERVICE Forwarding remains outside of United States $450 Due to coordinating and filing the required paperwork with embassies and consulates, there is an additional service charge for this type of arrangements. This charge does include our staff and vehicle. In some instances, there are extraordinary requirements where additional charges may be added. This charge does not include the consulate or other governmental agencies fees. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are fiberboard.
