WELCOME TO TEMPORARY HOUSING MISSION STATEMENT The professional employees of NewYork-Presbyterian Hospital's Real Estate Department are dedicated to providing the Hospital's staff, visitors, and guests the highest quality of customer service in its housing, parking and Guest Facility accommodations. Under the umbrella of Royal Charter Properties the staff strives to enhance the value of the Real Estate Assets and investments that contribute to the hospital's fiscal well being. "WE ASPIRE TO INSPIRE" On behalf of the NewYork-Presbyterian Hospital's Real Estate Department we would like to welcome you to Temporary Housing. Please read the enclosed information provided in this package as it should answer many questions you may have. We want you to optimize your living experience while residing in the Annex Residence therefore, your cooperation in observing these rules will ensure that everyone receives the maximum benefit from the facility. GENERAL MOVE IN PROTOCOLS AT ALL BUILDINGS All moves must be scheduled prior to move in with the Leasing Office. Move in hours need to be scheduled between 9AM TO 4PM through the leasing office. Monday thru Friday ONLY, No Exceptions. Move ins on Holidays are not permitted. Once move-in is scheduled you must arrive on time, we have the right to refuse your move if you arrive late and all associated costs will be your responsibility. Contact (212) 746-9096 to schedule your move in. On the day of your move, pick up your keys at the Leasing Office before 4:00pm. All must provide valid identification (i.e. Hospital, College or State ID) prior to release of keys. Belongings cannot be delivered prior to your lease start date. BUILDING ACCESS You are able to gain access to the Annex Building with your employee ID. If you have not received one before your Move-In date, you are able to use your Assignment Sheet until you have received your Employee ID. A signed copy of your Assignment Sheet has been given to you in your Welcome Folder. Those Residents working in the West Campus or without an Employee ID must ring the Security Access Bell located outside Annex Building for building access. 2 P a g e J u l y 2 0 1 4

KEYS On the date of Move-in, you will be given One (1) room key. If you lose your room key, report it immediately to the Leasing Office at (212) 746-9096. There is a fee of $25.00 for the loss and replacement of room keys. Duplicating keys is strictly prohibited. FURNISHINGS Most rooms are furnished with the following: a bed, mattress, dresser, desk and chair. Also, included is a medicine cabinet, sink with vanity, closet and air-conditioner with remoter control. Rooms are supplied with one time linen (Sheets & pillowcases). Rooms measure an approximate 8 x 11. Removal of furnishings without permission of the Real Estate Department is unacceptable. Missing furnishings or damages to furniture, linen, lighting, or anything else in your room that is not your property, as well as to items in the common areas will be charged against your security deposit. MAILBOXES Mailbox keys are obtained from the mailroom, located in the sub-basement of the main Hospital. Upon presentation of a housing assignment, a mailbox key will be issued and must be returned at the end of the resident s occupancy. ROOM/COMMON AREA ISSUES Contact the Leasing Office at (212) 746-9096 or email zeynep.sanli@cis.cushwake.com to inform us of any issues you are having within your room. It is your responsibility to report any issues immediately. We will work with staff to resolve the issue as quickly as possible. For other issues, contact the following departments: Electrical, Plumbing, Heating or Air-conditioning contact Engineering at (212) 746-1920 Kitchen, Garbage, Bathrooms or Shower room, and public areas contact Environmental Services at (212) 746-0630 EXTERMINATION Residents are responsible to report to the management office immediately any type of pest activity or infestation in your room by contacting the Leasing Office (212) 746-9096. This includes such things as bedbugs, roaches, and/or mice, etc. These issues can affect not only your room but can spread to adjacent room if not addressed in a timely manner. We have a very proactive pest control program. However, if you do not do your part by reporting a problem or even a suspected problem, we cannot do our part to address the issue. This will lead to unnecessary inconveniences and expenses. VACATING We require 15-days notice of terminating your Occupancy Agreement. All vacate dates must coincide with your Occupancy Agreement Expiration date. Failure to return your keys by your expiration date will result in a charge for the entire month s rent. ( For example, your Occupancy Agreement ends on 3 P a g e J u l y 2 0 1 4

September 30 th, and you return your keys to management on October 5 th, you will then be responsible for the entire month of October s rent. Pre-Vacate Inspection. The Housing Supervisor and/or Leasing Personnel will examine the room for damages while insuring all your questions are answered. Upon vacating the room, immediately drop off your room keys and air conditioner remote control to the building concierge at Payson House Doorman/Concierge (435 East 70 th Street). Keys and remote control should be returned in an envelope with your name and room #) There is a fee of $50.00 for the loss of the air conditioner remote and an additional $25.00 for the loss and replacement of room keys. This fee will be taken out of your security deposit. STATEMENTS Rent bills are distributed on the 1 st of each month and place on your door with a self addressed envelope. Remittance is required by the 5 th of the month. UTILITIES Residents are NOT required to contact the utility company; utilities are included in your rental charge. MEDIA A public television set is located in the Lounge Area. Rooms are not cabled for television. Personal television sets are not recommended, as reception is poor. No Wifi connection available. COIN-LESS LAUNDRY FACILITIES Laundry rooms are located in room 907; Machines are operated by the use of a laundry card, which can be purchased at the card dispensing machine. Instructions on proper use of the card system are displayed on each machine. If you have any questions on how to operate the equipment or are experiencing difficulty with the machines, please contact HERCULES at (800) 526-5760 (website: www.hercnet.com) Note: For fire safety, unplug all appliances (irons, and /or steamers) when you are done with them. NO SMOKING Local Law prohibits smoking in all public areas of the building including the lounge, kitchen, bathrooms, laundry rooms, corridors, stairways and elevators. We very much appreciate your cooperation. KITCHEN APPLIANCES Kitchen appliances are prohibited in your dorm room, (i.e., Hot Plates, Toaster Ovens, Microwave Ovens, Mini Fridges, Coffee Makers) NEVER leave food unattended. DO NOT leave items of plastic on a hot stove, it will melt. 4 P a g e J u l y 2 0 1 4

SAFETY Always lock your dorm room door as you leave, even if you will only be out of your room for a few minutes or will remain close by. Royal Charter Properties, Housekeeping staff, nor NewYork Presbyterian Hospital wil be held responsible for items lost or stolen from your room. HALLWAYS/PUBLIC AREAS NYC Fire Department Regulations stipulate that hallways must be kept free of clutter. In accordance with this regulation, personal belongings including, but not limited to door mats, umbrellas, shoes, or hygienic products left in the hallway will be removed without notification. Areas shall not be obstructed or used for any other purpose than for egress and egress to and from the respective rooms. If personal belongings are left in the hallways/public areas, they will be discarded at the Resident s expense. FIRE EXTINGUISHERS Fire extinguishers are located on the floor (see floor plan for location) and smoke alarms are in each room for your protection. Become familiar with the location of the Fire Emergency Exits, Fire Alarm Pull Stations and Fire Extinguishers on the 9 th floor. Any tampering with this equipment will result in disciplinary action and the cost of replacement equipment will be billed to the resident. If you suspect a problem, notify the Real Estate Department. ENERGY CONSERVATION Help save on unnecessary utility costs. Except for the refrigerator which must be left on, please turn off electrical items such as room lights, air conditioners, fans, when you leave your room. CLEANLINESS Remember to be respectful, kind and considerate of other Residents in the Annex Residence. You are responsible for the cleanliness of the room you occupy. Food should be cleaned off and wiped down from tables and floors after every use. Do not place unclean utensils, plates, cups in the sink. Do not leave any items soaking in water. All items must be cleaned immediately. Microwave, the interior/exterior of the microwave must be cleaned immediately after each use! Be conscious of foods that have lingering smells (especially fish). Be courteous. Don't fill the refrigerator, freezer and pantry with so much food that no one else has space. Do Not take food or drinks that do not belong to you, Seal and label all food stored in the refrigerator and pantry closet, including condiments. Do Not leave opened tin cans in the refrigerator. Be responsible. If you know you are going away, do something with your food, whether it's taking it home or throwing it out. 5 P a g e J u l y 2 0 1 4

All garbage must be placed in the trash can. Food and garbage should not be left in any of the common areas. Do Not leave items on top of the refrigerators, counters and/or window sill. REFUSE ROOM Residents are responsible for removing your own trash and placing it in the garbage receptacles in the Refuse Room. Place all recyclable items in the receptacles provided in the refuse room. Recycle items include newspapers, magazines, catalogs, telephone books, corrugated cardboard, metal cans, aluminum foil wrap as well as glass bottles and jars. Use receptacles for its intended purpose. Do-Not throw any non-recyclable trash in the recycling receptacles. Garbage bags are stored in the refuse room for your usage. VISITORS The Security Control Room located in the first floor of the Annex Building, requires residents to accompany visitors to their rooms between 8:00pm and 6:00am. At other times, visitors must report to the security control room and state the name of the resident they are visiting. Security will verify the visit with the resident before allowing entrance to the building. No over-night guests are permitted. NOISE Residents must be considerate of neighbors located adjacent to their rooms. Music (amplified or via musical instruments) can be disturbing to the rooms located around you. We urge you to be particularly sensitive about volume/noise. PROHIBITED Any activity that jeopardized the working relations between employees of NewYork-Presbyterian Hospital s Real Estate Department, suppliers or customers is prohibited including: Fighting, horseplay, boisterous conduct, and verbally or physically threatening of intimidating other residents and/or visitors Theft, destruction or damaging of Real Estate or Resident property Deliberate misconduct such as defacing or damaging client or tenant property Gambling, selling goods or solicitation of any kind Possession of alcoholic beverages or illegal substances Possession of weapons, explosives or firearms Harassment (Sexual, Physical and/or Verbal) Furniture may not be removed from the room or public areas Do Not attempt repairs or adjustments on any appliances that does not belong to you Do Not apply adhesive-back materials, wallpapers, or decals to the walls, since removal can cause damage 6 P a g e J u l y 2 0 1 4

Storage of all other flammables is prohibited; including but not limited to gasoline, kerosene, incense, or anything that produces an open flame OWNER ROOM ACCESS During reasonable hours and with reasonable notice, Owner may enter the room for the following reasons: To make any repairs or changes Owner decides are necessary. If at any time you are not personally present to permit Owner or Owner s representative to enter the room and entry is necessary or allowed by law or under your Occupancy Agreement, Owner or Owner representative may enter the room. However, if you do not do your part by reporting a problem or even a suspected problem, we cannot do our part to address the issue. This will lead to unnecessary inconveniences and expenses.
