SUMMARY The Foptex (Pvt.) Ltd. is established in a 8 storied building, but factory uses only up to the third floor on rent. The construction of factory building was started in 1997 in phase manner and completed in 2005. The production was started in 1999. The building was approved for industrial purpose. There were 247 workers present in the factory during the inspection. The Factory was surveyed for electrical safety by Woosun Energy and Construction Co., Ltd. (WEC). The purpose of the survey was to identify significant electrical safety issues and to provide recommendations for remediation based on applicable standards specified by the Accord. The scope of this initial electrical safety inspection was limited to the review and identification of major electrical safety issues. The inspection did not include identification of minor deficiencies, which will be further addressed as part of follow-up inspections. Table below summarizes the major electrical safety issues identified during the inspection. Recommendations have been provided to address each issue. An implementation schedule shall be developed by the factory to remediate each of the findings. The specific timing of improvements, including any requested extensions due to design / installation constraints, shall be submitted to the Accord for approval. Foptex (Pvt.) Ltd. Page 2 of 13

FINDINGS AND RECOMMENDATION: Finding No.: E-1 Category: SERVICE LINE Service line passing through walls without protections. All cables passing through permanent wall must be protected in steel pipes and remaining holes around the pipe must be sealed. Remediation timeframe: 3 Months Main service line cables entering through walls of factory building kept without protections. Foptex (Pvt.) Ltd. Page 3 o f 13

Finding No.: E-2 Category: GENERATOR ROOM Inadequate working space around the generators Expand or relocate the existing generator room to provide safe working space as per BNBC table 8.2.9 or keep sufficient space (1 meter preferably) around the generator foe ease of maintenance. Remediation timeframe: 6 Months Diesel generator installed in small room with LT switch gear panels and other materials. Finding No.: E-3 Category: GENERATOR ROOM Combustible materials inside Generator room. Remove excess reserved fuel and other combustible items stored in the generator room. Finding No.: E-4 Category: GENERATOR ROOM Erath conductors are improperly jointed to extend earth connection. Provide earth bus bar for making any earth junction point. Ensure the bus bar is made of copper and cables being used for earth connection are 35 sqmm conductors green. Generator body and frame earth loop. Finding No.: E-5 Category: GENERATOR ROOM Oil spilled in generator room floor. Generator room floor must be kept free from oil spillage to avoid fire hazard. Establish a routine cleaning program to keep the generator room neat and clean. Remediation timeframe: 1 Month Lint, dust & other waste materials around the generator. Foptex (Pvt.) Ltd. Finding No.: E-6 Inadequate Illumination inside Generator room Install additional lights inside generator room and ensure minimum 150 lux illumination around the room for performing easy and smooth maintenance and inspection. Remediation timeframe: 1 Month No sufficient lighting LT switch gear and generator room. Finding No.: E-7 Combustible materials and inflammable items inside electrical room. Clear electrical room(s) from any item not necessary for regular operation and Inflammable items must be kept away from electrical facilities. Finding No.: E-8 Inadequate working space around panels. Every item of installation should be arranged so as to facilitate its operation, inspection, maintenance & access. Access of the DB must be kept obstacle free. Some of the panels may be relocated to provide adequate and safe working space. Provide at least 1 meter clearance in front the panels for ease of its operation and maintenance. Remediation time frame: 6 Months Control and distribution panels installed very closed to each other and no sufficient space for panel doors to open. Finding No.: E-9 Excessive lint deposit in Control Panel. Disconnect the panel from power source and clean the interior of the panel. Seal all unused opening of the panel using steel top/base- plate and proper sized cable gland. Establish a routine cleaning program to keep all the panels free from dust. Remediation time frame: 3 Months Messy cabling and wirings inside main distribution panel. Foptex (Pvt.) Ltd. Page 7 of 13

Finding No.: E-10 Panels not securely fixed to the foundation and no rubber mat is found in front of panels. Fix the panel base securely to the foundation with appropriate fastening devices. Provide cable support for incoming and outgoing cables. Provide the rubber mats in front of all the panels with proper size and grade. No rubber mat provided in front of any panels. Finding No.: E-11 Panel body and door not earthed. All electrical panels and doors must be connected with dedicated earth (frame). Earth contact established by installation of earth strip inside panel does not substitute the requirement. Both the panel and door left unearthed. Foptex (Pvt.) Ltd. Page 8 of 13

Finding No.: E-12 Switchgears and devices installed in locally fabricated distribution boards. Use standard and tested panel boards to protect devices inside and ensure safety to the operators. Locally made and repainted panel. Finding No.: E-13 Ammeters and lamp indicators not working. All the indicating meters and lamps must be in good condition to show the actual electrical parameters and healthiness of supply. Remediation time frame: 3 Months Defective ammeters and indicating lamps on LT panels. Finding No.: E-14 Category: CABLE & CABLE SUPPORT Protective devices are mounted on wooden plank. Protective devices should be encased in metal casing made of 20 SWG thickness metal sheets to reduce the risk of fire spreading due to short circuit. Remediation timeframe: 3 Months Cables laid on floor in electrical room without cable trenches/trays (haphazardly laid). Foptex (Pvt.) Ltd. Page 9 of 13

Finding No.: E-15 Category: CABLE & CABLE SUPPORT Cables terminating at panel not supported. Install the cable tray/ladder/ duct upto the cable entry of the panel in order to support the cables. Ensure the cables are tightly latched with the ladder and provide covers made of noncombustible material preferably metallic sheet to protect the cables insulation from any physical damage as well as prevent ingress of debris, dust and lint. Remediation time frame: 6 Months Cables distributed randomly from main distribution panel in LT room. Finding No.: E-16 Category: CABLE & CABLE SUPPORT Flexible PVC conduit wiring not supported. Use rigid PVC pipe for surface and exposed wiring through-out its length and supported properly (clamped with saddle, at regular interval of 600 mm).the conduit shall run vertically or horizontally, shall never at angle.. Flexible conduit must not be used for long point wiring (except for special wirings). Remediation timeframe: 3 Months PVC flexible conduits on walls kept unsupported and undressed. Foptex (Pvt.) Ltd. Finding No.: E-17 Category: CABLE & CABLE SUPPORT Wire/cable joints in wiring systems are not properly done and not protected adequately. Wire and cable joints in wiring system must be avoided. Unavoidable joints must be tightly connected and protected. Heat shrink tubes may be used for cables. Remediation Time frame: 1 Month Cable joints made by passing MCB on wooden board in production floor. Finding No.: E-18 Excessive wires crowding inside the panel. Assign an electrical engineer to determine the capacity of the installation and redesign the wirings of the panel. If the wirings and loads exceed the capacity of the panel, install additional panel. Establish a load management program for avoiding any installation exceeding its capacity in future. Install slotted wiring-duct inside the panel to arrange and latch the haphazard cables. Unused wires bundled and kept unsafe with plastics inside boiler control panel. Foptex (Pvt.) Ltd. Page 11 of 13

Finding No.: E-19 Category: WIRING Casing-capping wiring exposed partially in few sections of its length. Casing-capping wiring must protect wiring enclosed throughout length. Proper joints, bends and junctions must be used to negotiate corners. Remediation timeframe: 3 Months Casing-capping damaged and wires exposed at several places in production floors. Finding No.: E-20 Category: WIRING Wiring passing through walls are not protected and remaining gaps, not sealed Wirings passing through permanent walls must be protected with rigid conduits/pipes and remaining gaps must be sealed with fire rated materials. Remediation timeframe: 3 Months Wires passing through walls to other compartments are not protected. Finding No.: E-21 Category: WIRING Wooden switch boards used in the factory. Wooden boards must be replaced with switch boards fabricated from noncombustible materials. Remediation timeframe: 6 Months All SDBs in production floors are of wooden made. Foptex (Pvt.) Ltd. Finding No.: E-22 Category: EQUIPMENT AND MACHINES Compressor machine mounted on wheel. Compressor machine mounted on wheel must be anchored or the wheels must be locked to prevent from trolling. Remediation time frame: 3 Months Compressor machine just placed on floor on its wheel without anchoring. Finding No.: E-23 Category: BOILER AND COMPRESSOR Wiring and cables in boiler room not protected. All electrical installations, including wiring and cable works must be protected against heat from the boiler by supporting and separating at safe distance.
