Stuart, Va. 24171 Emergency Preparedness Plan Severe Weather/Earthquake/Building Damage/Fire/Missing Person Policy: Per page 22 of the PCS licensure manual: 530 part A: All services will develop a written emergency preparedness and response plan. All staff should be aware of the plan and the procedures used in the event of specific emergencies such as severe weather, water damage, disasters, loss of utilities, and emergency evacuations. Each program/facility will develop and maintain an emergency supply inventory list, check off list, procedures for disasters/emergency events, training, and practice drill schedule. Each program/facility s emergency supply inventory will include at minimum: Operable flashlights or battery lanterns A battery charged Radio Supplemental charged batteries Stacked First Aid Kit as designated by Health and Safety Management Policy Supplemental First Aid Supplies per program or license guidelines and indicated needs. Tape for windows Fire extinguishers Blankets Charged Cell Phones Emergency health information on each of the program participants Portable containers for priority health or medical supplies if needed Disaster and other Emergency Procedures Disaster and other emergency procedures are to be utilized as follows: Severe Weather In the event of severe weather the staff member(s) on duty will remain on duty until program participants are evacuated to their homes or a designated safe location. Drill Procedures for Severe Weather and Earthquake: If there is enough advanced notice all program participants will be brought to their homes or other designated safe location. If not, the following steps will be taken: 1. Practice having all people get in protected area doorway, under secured furniture or going in the basement. 2. Practice placing outdoor materials not anchored (chairs, etc.) into the garage.

3. Practice protection of windows by simulating the taping of windows to reduce broken glass flying. 4. After shock, simulate turning off water and gas at the main valves and appliances. 5. Practice checking for broken water pipes, leaking fuel. 6. Practice alerting authorities. If emergency situation call 911 and as situation allows PCS maintenance staff, supervisor and coordinator and follow through with critical incident report procedures. 7. Practice safe guarding records, data files, and client health supply needs (medications etc.) 8. Practice evacuation of building procedures 9. Ensure complete evacuation of building take a head count to make sure all people are safe and accounted for. 10. Simulate assisting, assessing those trapped, injured, and those with special needs. 11. Simulate determining need and location of emergency transportation etc. Procedure after the drill: 1. Return to building after given permission by appropriate personnel. 2. After event, practice simulating procedures to restore services: public authorities, maintenance staff, contracted assistance. 3. Complete documentation on drill. Loss of Utilities If utilities are lost, all participants are to take appropriate precautions to protect themselves. Staff should be notified immediately. Staff will notify the supervisor or person on call. The supervisor or person on call will take the appropriate action to ensure a safe environment or direct staff to take participants home. Missing Persons Program participants at Horizons require a level of supervision. The level is different for each individual. Participants will be given the level of supervision determined by their disability and medical need. In the event of a missing person the staff will notify the proper authorities and supervisor as soon as it is known that the consumer is missing. Severe Injury Training Protocol In the event of a severe injury, the staff person first noticing the injury shall do the following:

1. Secure the immediate area making it for both the victim and others who may attempt to assist. 2. Request assistance from other staff if present. 3. Notify the rescue squad by dialing 911 (if needed) 4. Employ CPR and First Aid as needed. 5. Execute Critical Incident Procedures. Emergency Evacuations Follow the same procedures as evacuation due to fire. Fire Drill Procedures Procedures Prior to Drill: Schedule monthly drills alternating time of drill between 8am and 4pm. Meet at the designated assembly point outside the building Tree in the front of the yard. Establish secondary means of exit (depending on probable and alternate locations of fire) Assign responsibilities for : i. Person(s) to evacuate consumers with special needs ii. Person(s) to close doors, windows, and other fire barriers iii. Person(s) to ensure complete evacuation of building Procedures During Drill: In the event of a fire or other emergency, the person first discovering the fire shall alert others in the immediate area by a loud clear voice. If the fire can be safely extinguished, the person shall take appropriate action with the fire extinguisher and other means immediately available to him or her. shall: If the fire cannot be immediately extinguished, staff and residents Notify the building occupants of the fire s presence by shouting and activating the smoke alarms. Assure that the Fire Department has been notified immediately by dialing 911. Practice evacuation of the building procedures Staff shall make certain that people in the rest rooms, offices, basement, etc. are aware of the emergency and exiting the building. Evacuate program participants with special needs.

Close doors and windows and other fire barriers. Exit the building, and take a head count. Inform program participants about appropriate use of fire fighting equipment. Procedures After the Drill Training in Fire Procedures Return to the building Complete documentation regarding the drill. Each new staff member will be trained in emergency preparedness plans during the first day of employment and be given a copy of the emergency preparedness plan. The Manager shall be responsible for the training and orientation of each employee at PC Horizons. In addition, all staff shall participate in periodic emergency preparedness training (minimum annually), drill and reporting procedures, training and practices. The program manager will supervise frequency and content of on-going retraining, drill completion, and maintain documentation consistent with PCS policy and procedures, and licensing guidelines. Fire drills and Safety Inspections of Equipment and the Building PC Horizons: Monthly inspection of each fire extinguisher to assure that each unit is in readily and available and in operational condition. This should be completed at the time of each fire drill. Also each unit must be professionally maintained at least annually (with maintenance records kept and available). Monthly testing of emergency lighting (if available) and smoke detectors, as well as documentation of testing and subsequent findings. These findings shall be documented and shall include the date and initials of the person conducting the inspection. This should also be done at the time of the monthly fire drill. Monthly fire drills completed and record of the drill outcomes documented. Records shall be maintained for two (2) years subsequent to the drill. This record shall include: o Date and time of drill o Amount of time taken to evacuate the building o Specific problems encountered o Specific tasks completed Doors and windows closed Practice notifying fire officials Head count

o Summary of drill o Signature of the drill responsible staff person Poison Control The telephone number of the regional poison control center shall remain posted on or next to every telephone in the program. Use of Vehicles Any transportation provided by the program, directly or through contract, for program participants shall be in compliance with all PCS policy and procedures as well as state and federal laws relating to vehicle safety and maintenance, licensure of vehicles, and licensure of drivers. Control of Deviant or Criminal Behavior PC Horizons staff shall take all reasonable precautions to assure that no program participant is exposed to or instigates such behavior that might be physically or emotionally injurious to himself or another person. Critical Incident All incidents deemed critical, or any incident that could become critical involving a program participant or staff person shall be reported using the guidelines set by PCS policy and procedures. A copy of these procedures may be found in the personnel manual as well as the PSR policy and procedures manual. Revised January 2009
