Slight change in trend. Appears to be slowing but more readings required to confirm. Tuesday, January 30, 2007 Movement caused by blasting very similar to movement caused by excavation. Need to see the velocity level off or drop before mining the area. Able to start mining once the movement begins to slow. Note: this is only an example and is not an estimation of time to mining. Prism 175 accelerates to failure. Prism 175 now What s Changed and how we move forward. FACTS Before First Failure (up to 18 th December 2005) Higher than normal prism movements. Unsure of outcome, large amount of stored energy. Understood instability mechanism After First Failure (up to 23 rd March 2006) isk of sudden collapse highly unlikely without external influence (rain, blasting). Stored energy reduced. Failure mechanism proven. After Second Failure esult of 190mm in 48hrs of rain destabilising failed material. Stored energy further reduced. Material at much lower angle of repose. APPOACH Before First Failure Take no chances. estrict access. Intensified monitoring. After First Failure Mine to monitoring. No incidents. Managed with prism monitoring system. After Second Failure Mine as a large muck pile (broken dirt). emove material that is not supporting rill. Manage removal of final material with the aid of adar. 7. system training 8

Feedback and involvement of mining crews was important. Developing an understanding and trust through providing accurate timely information Large scale wall movement was seen in the NW corner of the pit. Mining successfully completed to final design, flexibility and monitoring 10
