ii Performance Test Instructions This performance test provides detailed performance checklist items for candidate testing. Performance tests should not be conducted until the candidate has successfully completed the academic part of the CDC. However, it is strongly encouraged that this supplement and the checklist it contains be used during the normal course of study. Candidates may practice the performance test at anytime during study and up until performance testing is conducted. Practice is highly encouraged. This particular course uses four workstations. Within each workstation there are several tasks and objectives (NFPA line items). A Performance Summary Sheet precedes each workstation or group of evaluated tasks. This sheet lists the NFPA line items evaluated and the specific tasks to be done. Each performance test lists the setting and tools/equipment needed for the listed tasks. It is important also to understand the grading process used during the evaluation. Additional information on the grading is on the next page of this supplement.

iii Grading Schedule The following criteria will be used to evaluate and determine the pass/fail status of a candidate. Each item in the Performance Test Checklist is given a rating. Critical (C) This rating has been assigned to items, which, if omitted or performed incorrectly, would result in severe injury to, or death of, an individual. Should a fire fighter fail to perform any one item rated as Critical (C), the fire fighter would be unsuccessful in demonstrating the required proficiency level for that standard. Major (M) This rating refers to any item that is very important to the general safety of personnel and the successful completion of the evolution. Should a fire fighter fail to perform any three items rated as Major (M), the fire fighter would be unsuccessful in demonstrating the required proficiency level for that standard. General This rating although there is not symbol, has been given to all remaining items that in combination are relevant to the successful completion of the evolution. Should a fire fighter fail to perform any four items rated as General, the fire fighter would be unsuccessful in demonstrating the required proficiency level for that standard. Should a fire fighter fail to perform any combination of Major or General rated items resulting in a sum total of four, the fire fighter would be unsuccessful in demonstrating the required proficiency level for that standard.

SKILLS TEST # 1. Fire Department Communications Performance Test Summary Sheet Objectives: NFPA Standard 1001, Chapter 6, Paragraphs 6.2.1, 6.2.2. Tasks: 1. Demonstrate the ability to complete a basic incident report. 2. Demonstrate the ability to operate communication equipment through notification of a need for team assistance (skill is completed in Skills Test #3). Candidate must complete skills 1 and 2.

Stations 2 Personnel Classification: Fire Fighter II Objective: NFPA Standard 1001, Chapter 6, Paragraph 6.2.1. Task: Demonstrate the ability to complete a basic incident report so information is accurate. Performance Conditions Setting: Fire Alarm Communications Center. Tools/Equipment: Computer, incident report procedural guide, pen/pencil, paper, applicable incident reporting tool, and applicable codes. Attainment Standard: Completion of all elements/steps within 20 minutes. 1. Stations 4 Personnel Classification: Fire Fighter II Objective: NFPA Standard 1001, Chapter 6, Paragraph 6.4.1. Task: Wearing full protective equipment (without SCBA), and as a member of a team, demonstrate the use of cribbing and shoring, hydraulic tools/equipment, pneumatic tools and hand tools, while removing or displacing a vehicle roof, windshield, windows, steering wheel, steering column and/or dashboard. Disentanglement of an entrapped victim is overall objective. Performance Conditions Setting: Training Ground Tools/Equipment: Full protective equipment, cribbing/shoring material, hydraulic tools/equipment, pneumatic tools, hand tools, adult-rescue manikin, and wrenched vehicle. Attainment Standard: Successful completion of all element/steps within 30 minutes. 1. As a team, demonstrate the use of cribbing/shoring material, hydraulic tools/equipment, pneumatic tools, and hand tools while extricating a victim from the wreckage (The lead evaluator will construct a scenario that meets the need of this skills test. Keep in mind the outlined criteria that must be met found on the Skills Test #2 summary sheet 1. In accordance with Essentials of Fire Fighting a. (M) Wear full protective equipment including eye protection b. Stabilize the vehicle using cribbing/shoring material c. (M) Protect victim at all times d. Remove the windshield e. Remove the driver's side door using a hydraulic rescue tool f. Stations 5 Personnel Classification: Fire Fighter II Objective: NFPA Standard 1001, Chapter 6, Paragraph 6.4.2. Task: Wearing full protective equipment, as a team member, retrieve various tools and equipment in a timely manner ensuring completion of a specialized rescue. Performance Conditions Setting: Training Ground Tools/Equipment: Full protective equipment, self-contained breathing apparatus, available specialized rescue equipment (i.e. rope, tripods, cribbing/shoring, saws, harnesses, stokes litters, scuba gear, and so forth). Attainment Standard: Successful completion of all element/steps within 30 minutes. 1. Demonstrate the ability as a team member to retrieve various tools essential to the success of a specialized rescue operation The lead evaluator will construct 1 scenario, which will involve one of the specialized rescue operations below. a. structural collapses b. confined space rescues c. trench collapses d. water and ice emergencies e. cave/tunnel emergencies f. elevator emergencies g. escalator emergencies h. industrial accidents i. energized electrical line emergencies 1. Stations 6 SKILLS TEST # 3. Fire Ground Operations Performance Test Summary Sheet Objectives: NFPA Standard 1001, Chapter 6, Paragraph 6.1.1.2, 6.2.2, 6.3.1, 6.3.2, 6.3.3, 6.3.4. Tasks: 1. As a team member, extinguish an ignitable liquid fire utilizing the proper foam stream. 2. As a team member, coordinate an interior attack line for team's accomplishment of a structure fire assignment. a. Upper-level fire b. Below grade level c. Energized electrical 3. As a team member, control a flammable gas cylinder fire. 4. Demonstrate the ability, as a team member, to protect evidence of fire cause and origin. Stations 7 Personnel Classification: Fire Fighter II Objective: NFPA Standard 1001, Chapter 6, Paragraph 6.3.1. Task: Wearing full protective equipment including SCBA, as a team member, extinguish a ignitable liquid fire using an affective foam blanket, extinguishing the fire, preventing reignition, protecting the team with a foam stream, and approach and retreat facing the hazard. Performance Conditions Setting: Training Ground Tools/Equipment: Full protective equipment, self-contained breathing apparatus, foam concentrate, nozzles, fire hose, fully equipped pumper, foam eductor, combustible liquid, flammable gas cylinder mock-up, foam proportioning device, and water supply. Attainment Standard: Successful completion of all element/steps within 30 minutes. 1. As a member of a team, extinguish an ignitable liquid fire with a foam fire stream. Scenario: A team of four fire fighters is assigned to extinguish an ignitable liquid fire using a foam fire stream. The objective of the exercise is to extinguish the fire in a safe manner. Two fire fighters are assigned the foam attack line and two are assigned to a backup line with adjustable fog nozzles 1. In accordance with Essentials of Fire Fighting and local policies and procedures a. Prior to ignition demonstrate the ability to prepare foam concentrate supply for use (to add to a vehicle, drum to be used with an eductor, and so forth) per Standard Operating Procedures b. Assemble foam stream components in accordance with equipment availability c. Connect and charge hoselines d. (M) Place foam attack line in position upwind from fire. Check flow of line e. Performance Conditions Setting: Training Ground Tools/Equipment: Full protective equipment, self-contained breathing apparatus, nozzles, fire hose, fully equipped pumper, water supply, structural fire trainer, and standpipe pack. Attainment Standard: Successful completion of all element/steps within 30 minutes. 1. As a member of a team, extinguish a fire in an elevated location within a structure NOTE: Evaluator will conduct a safety brief as well as introduce scenario prior to exercise/evaluation 1. In accordance with Essentials of Fire Fighting and local policies and procedures a. Upon arrival, candidate conducts size up as well as establishes incident command (vocalizes this to alarm center) b. Task: Wearing full protective equipment including SCBA, as a team member, extinguish a fire involving energized electrical equipment. Performance Conditions Setting: Training Ground Tools/Equipment: Full protective equipment, self-contained breathing apparatus, nozzles, fire hose, fully equipped pumper, water supply, and structural fire trainer/transformer mock-up (or equivalent). Attainment Standard: Successful completion of all element/steps within 30 minutes. 1. As a member of a team, extinguish a fire involving energized electrical equipment. 1. In accordance with Essentials of Fire Fighting a. Candidate receives briefing from evaluator, which includes safety as well as scenario. b. Candidate conducts size up. Depending on the briefing, candidate addresses Incident Commander (lead evaluator) for additional assistance. (Only if dictated by evaluator's briefing) c. No indication if power is on or off d. Call for electrical utility personnel to respond to location e. Delay attack until arrival of utility personnel; simply protect exposures f. Utility personnel confirm that electricity is disconnected g. Attack fire with dry chemical extinguisher with full protective equipment and backup hoseline h. Stations 14 Personnel Classification: Fire Fighter II Objective: NFPA Standard 1001, Chapter 6, Paragraph 6.3.3. Task: Wearing full protective equipment including SCBA, as a team member, control or extinguish a fire involving a flammable gas cylinder. Performance Conditions Setting: Training Ground Tools/Equipment: Full protective equipment, self-contained breathing apparatus, nozzles, fire hose, fully equipped pumper, water supply, and LPG cylinder mock-up. Attainment Standard: Successful completion of all element/steps within 30 minutes. 1. AS a member of a team, control or extinguish a fire involving a flammable gas cylinder. Scenario: Fire team is assigned to control or extinguish a fire involving a LPG container. The vapor line from the container has broken and the vapors ignited causing flame to impinge on the upper portion of the tank 1. In accordance with Essentials of Fire Fighting a. Identify product by utilizing placards, bill of lading, manifest, labels, and so forth. b. Retreat safe havens are identified c. (C) Stay clear of tank ends d. (M) Lay and charge two attack lines e. Approach, facing the cylinder but staying a relatively good distance. f. Assure continuous supply of water g. Apply water to tank from a distance with straight or solid streams h. (M) Apply water at point of flame impingement. i. Apply water so that tank is covered with a thin film of water j. Do not extinguish flame on relief valve k. (M) Upon ensuring area is safe for entry, fire team enters using a fog pattern for protection l. Member of fire team closes/caps valve m. (M) Leak secured, fire extinguished and cylinder cooled. n. Task: Wearing full protective equipment including SCBA, protect evidence of fire cause and origin from disturbance. Performance Conditions Setting: Training Ground Tools/Equipment: Full protective equipment, self-contained breathing apparatus, flashlight, overhaul tools, rope, cordon tape, clipboard, paper, and pencil/pen. Attainment Standard: Successful completion of all element/steps within 15 minutes. 1. Demonstrate the ability to protect evidence of fire cause and origin from disturbance 1. Prepare a preincident survey in its entirety. 2. Maintain power plants, power tools, and lighting equipment. 3. Perform annual service test on fire hose. 4. Test the operability and flow from a hydrant.

Stations 17 Personnel Classification: Fire Fighter II Objective: NFPA Standard 1001, Chapter 6, Paragraph 6.5.1. Task: Prepare a preincident survey in its entirety. Performance Conditions Setting: Local facility Tools/Equipment: Facility, forms, pen/pencil, paper, clip board, facility folder, and prefire plan. Attainment Standard: Successful completion of all element/steps within 60 minutes. 1. Prepare a preincident survey on a local facility Note: Evaluator will choose a facility on his/her installation for the candidate to survey 1. In accordance with Essentials Of Fire Fighting and local policies and procedures a. Made an appointment with facility manager b. Candidate introduces himself/herself as well as crew c. Sketched facility and compared to existing external and internal maps d. Identified components of fire suppression and detection systems and their locations e. Stations 18 Personnel Classification: Fire Fighter II Objective: NFPA Standard 1001, Chapter 6, Paragraph 6.5.2. Task: Maintain power plants, lighting equipment, and power tools in a constant state of readiness. Performance Conditions Setting: Fire Department Stalls and/or Training Area Tools/Equipment: Power plants, power tools, lighting equipment, applicable checklists, hearing protection, eye protection, gloves, and helmet. Attainment Standard: Successful completion of all element/steps within 30 minutes. 1. Service and maintain portable power plants, lighting and power tools/equipment 1. In accordance with Essentials Of Fire Fighting, local policies/procedures, and manufacturer guidance a. Check fluid levels weekly or after each use b. Change bulbs, if necessary, in lighting equipment (remember to wear gloves when dealing with quartz bulbs) c. Inspect electrical cords, plugs, reels, and junction boxes at weekly intervals d. Start and run power plants once a week and after each use while testing electrical devices for operating status e. Start and run all equipment items once a week and after each use for operating status f. Inspect spark plugs, spark plug wires, and carburetors weekly. A new spark plug should be readily available g. Change extra gasoline approximately every three weeks to ensure freshness. (ensure have adequate quantity of special mixtures on hand) h. Inspect all hydraulic lines for operational condition (see manufacturer's guidance) i. Check all tools and equipment for corrosion

Stations 19 j. Ensure all cutting blades are in inspection order (see manufacturer's guidance) k. Ensure that hydraulic spreaders have had required non-destructive inspection (NDI) per manufacturer's guidance l. Ensure all chains are sharpened and lubricated adequately per manufacturer's guidance (chain saws) m. Clean as necessary with a water and mild soapy solution or other preferred cleaning solution (see manufacturer's guidance)

Stations 20 Personnel Classification: Fire Fighter II Objective: NFPA Standard 1001, Chapter 6, Paragraph 6.5.3. Task: As a member of a team perform annual service test for fire hose. Performance Conditions Setting: Training Ground Tools/Equipment: Protective equipment, nozzles, hose, hose clamp, pencil, tags, hose test valve, hose testing machine or fire department pumper. Attainment Standard: Successful completion of all element/steps within 30 minutes. 1. As a member of a team perform an annual service test for fire hose 1. In accordance with Essentials Of Fire Fighting, local policies/procedures, and manufacturer guidance a. Examine hose for jacket defects, coupling damage, and gaskets prior to testing (if damaged do not test) b. Test site preparation 1. Room to lay out hose 2 Smooth surface 3 Isolated from traffic 4. Stations 21 2. Attach hose opposite pump panel on pump 3. Test length not to exceed 300 feet 4. Stand away from the discharge valve connection when charging hose e. Test procedures 1. Connect hose sections to be tested 300 feet maximum, tighten connections with spanner wrenches 2. Connect an open test gate valve to each discharge valve, tighten with spanner wrench 3. Connect test length to test gate valve, tighten with spanner wrench 4. Tie rope hose tool or hose strap to each test length of hose 10 to 15 inches from the test gate valve connections. Secure other end to the discharge pipe or other nearby anchor 5. Attach shut off nozzle to open end of each test length 6. Fill hoseline with water at a pressure of 50 psi. Open nozzle discharge away from test area discharge all air 7. Close nozzle. Make chalk or pencil mark on jacket against each coupling. Check for kinks and leaks 8. Remove all hose that is leaking behind a coupling 9. Tighten couplings and replace gaskets as required 10. Close each hose test gate valve 11. Increase pump pressure to required test pressure as specified in NFPA 1962. 12. Maintain test pressure for 5 minutes. Inspect all couplings for leaks (weeping at the point of attachment) 13. After 5 minutes, reduce pump pressure. Close each discharge valve and disengage pump _

Stations 22 14. Open nozzle to bleed off pressure. Break all connections and drain 15. Look at all marks placed on hose couplings NOTE: If coupling mark moved during the test tag hose for recoupling. Expect 1/16-1/8 inch uniform movement of the coupling on newly coupled hose. Normal slippage for initial testing but should not happen on subsequent testing 16. Record all test results per standards, local policies and procedures

Stations 23 Personnel Classification: Fire Fighter II Objective: NFPA Standard 1001, Chapter 6, Paragraph 6.5.4. Task: Test the operability of and flow from a fire hydrant. Performance Conditions Setting: Training Ground Tools/Equipment: Pitot tube, pen and paper, hydrant, and nozzles. Attainment Standard: Successful completion of all element/steps within 10 minutes. 1. Read and record flow pressures with a pitot tube from three orifices while a hydrant if flowing water. 1. In accordance with Essentials Of Fire Fighting, local policies/procedures, and manufacturer guidance a. Grasp pitot tube with first two fingers of left hand. Hold air chamber with right hand rest little finger of left hand on outlet or nozzle b. Open petcock on pitot tube to drain air chamber. Close petcock c. Edge blade into stream from orifice. Hold away from orifice a distance that equals 1/2 the diameter of the orifice d. Hold pitot tube parallel to outlet opening, air chamber above the horizontal plane. Tube opening should be in the cent of the stream. e. Read and record pressure f. Repeat a-e for orifice #2 g. Repeat a-e for orifice #3
