A.1 Approval Sheet 1. This Quality Assurance plan shall be utilized by radon measurement technicians employed by Clarity Home Inspections (Clarity) for the conduct of radon measurements. The following Certified Radon Measurement Technicians are currently employed by Clarity: a. John Mattox 2. This quality assurance plan shall be reviewed and reissued annually. Submitted by John Mattox (Quality Assurance Officer) on November 17, 2017. page 3 of 18

A.2 Distribution List 1. This plan shall be distributed to all certified radon measurement technicians via email. The current version of this procedure on Google Drive in the Operations/Radon folder. 2. This plan shall be reissued after approval of any revision. Certified Radon Technicians listed in this document shall submit recommended changes to the Quality Assurance Officer. Copies of this document shall be distributed to all parties identified above upon completion of such changes. A.3 Work to be Accomplished 1. Clarity offers radon measurement services both with and independent of a home inspection. A.4 Quality Objectives 1. The quality objective for radon measurements by the Kansas Radon Program is to produce radon measurements which are accurate, consistent and reproducible. A.5 Personnel 1. John Mattox is the designated Quality Assurance Officer. 2. The Quality Assurance Officer will be responsible for: a. Drafting and Updating this QAP on an annual basis. b. Assuring compliance with this procedure. 3. At the time of the publication of this procedure John Mattox is the only personnel involved in radon measurement. John Mattox will also be responsible for: a. Performing all residential measurement activities. b. Performing all commercial measurement activities. A.6 Documents and Records 1. Client measurement reports will be kept in digital format in Google Drive in the Client Files/[Last Name].[First Name] folder. 2. Quality Assurance and Control records and logs will be kept in digital format in Google Drive in the Operations/Radon/QA Docs folder. 3. Records above will be maintained for a minimum of three years. B.1 Measurement Generation 1. Radon will be measured using continuous radon monitors, and passive activated charcoal devices. a. Continuous radon monitors will be operated according to the applicable manufacturer s operating manual. Continuous radon monitors employed by Clarity are as follows: i. Airthings Corentium Pro serial number 2700005713. b. Activated Charcoal devices manufactured by AirChek, inc. will be utilized according the instructions enclosed with each test kit. Appendix E contains Standard Operating Procedures for each device. page 4 of 18

B.2 Sampling Methods 1. Radon measurement/sampling will be performed in accordance with EPA Publications "Indoor Radon and Radon Decay Product Measurement Device Protocols" and Protocols for Radon and Radon Decay Product Measurement in Homes. 2. The radon measurement technician shall advise the client of the necessity for closed house conditions. A sign or label stating the need for maintenance of closed house conditions and advising against tampering with the measurement device shall be placed at the measurement site. B.3 Analytical Methods 1. Activated Charcoal devices will be analyzed by AirChek, Inc., which is certified as a Radon Laboratory by the State of Kansas. B.4 - Instrument Calibration 1. Instruments requiring calibration shall be returned to the manufacturer or a facility approved by the manufacturer for calibration. 2. Calibrations will be performed on an annual basis. 3. Any instrument whose calibration date is greater than one year will be removed from service until calibration is performed. B.5 Quality Control 1. Quality control measurements will be taken in accordance with EPA National Radon Proficiency Program Guidance on Quality Assurance, EPA 402 R 95 012, Oct. 1997. a. The following warning and control limits shall be utilized: i. For spiked samples, warning limit relative percent error of 20%, control limit relative percent error of 30%. 1. Perform the measurement of the spiked sample (this is the measured value). 2. Obtain the expected result provided by the agency which supplied the spiked sample (this is the reference value). 3. Relative percent error is calculated by dividing the measured value by the reference value and multiplying by 100. ii. For duplicate samples, warning limit of range ratio of 2.2, control limit of range ratio of 3.3. 1. Range ratios are based initially on 14 percent relative percent difference. a. The smaller result of two measurements is subtracted from the larger result. (This is the observed relative percent difference). b. The expected relative percent difference is determined by multiplying the average of the two results by 14 percent (0.14) (This is the expected relative percent difference). c. The range ratio is determined by dividing the observed relative percent difference by the expected relative percent difference. page 5 of 18

iii. For activated charcoal device blank measurements, result is expected to be less than the lower limit of detection of 0.3 pci/l. 1. Control charts will be maintained with warning limit of 0.6 and control limit of 0.9 pci/l. 2. Background readings above the control limit will be investigated by the QA officer. The investigation should include attempting to determine sources of radon ingress during transport and storage. 2. The following device specific QC measurements will be performed: a. Continuous Radon Monitors: i. Each continuous radon monitor will be calibrated annually. ii. Each continuous radon monitor in service shall have a label affixed which lists the calibration facility, the date of calibration and the date the next calibration is due. iii. Current calibration certificates will be maintained on hand in the business address for each instrument in use. iv. Background measurements shall be performed for each continuous radon monitor during calibration. v. All CRM measurements will be consecutively numbers in the following syntax, CRM_[YEAR]_[Measurement Number]. For example, the fifth measurement of 2018 will be CRM_2018_005. vi. Crosscheck measurements (side by side radon comparison measurement using two different types of measurement devices) shall be performed at the rate of ten percent of measurements. Crosscheck measurements will be conducted with an activated charcoal device. A log for documenting crosscheck measurements for continuous radon monitors is illustrated in Appendix E. vii. Routine instrument checks are performed by the instrument software upon startup. viii. Radon measurements shall be reported using the report generated by the continuous monitor which corrects the readings for the measured instrument background. b. Activated Charcoal devices: i. Charcoal devices will be stored in a low radon environment when not in use. ii. Appendix A will be used for device placement. iii. Duplicate (side by side) measurements shall be made in at least ten percent of measurements, with a maximum of 50 per month. A control chart for documenting activated charcoal duplicates and tracking precision is illustrated in Appendix B. iv. Field Blanks shall be employed equal to at least 5 percent of measurements, with a maximum of 25 per month. A control chart for documenting activated charcoal blank measurements is illustrated in Appendix C. v. Three spiked samples shall be performed for every 100 measurements, with a minimum of three per year and a maximum of six per month. Devices shall be sent to Bowser Morner for spiking per the device manufacturer (Air Chek Inc.) page 6 of 18

recommendation. A control chart for documenting spiked samples and calculating the mean and standard deviation is illustrated in Attachment D. 3. Chain of custody will be maintained as follows: a. Continuous radon monitor printouts shall be initialed by the radon measurement technician at the time/date for placement and retrieval as applicable. These printouts shall be maintained with the client record. b. Activated charcoal device custody shall be monitored using Appendix G, which shall be maintained with the client record. Activated charcoal devices shall be stored in the Clarity office, which is a low radon environment. 4. A background radiation measurement shall be accomplished during continuous radon monitor calibration. Background for activated charcoal devices shall be determined using field blanks. C.1 Assessment and Oversight 1. Measurements performed by Clarity Radon Measurement Technicians shall be submitted to the Quality Assurance Officer for review. 2. Complaints and suggestions regarding programmatic issues shall be reviewed as directed by the Quality Assurance Officer. Upon resolution of issues, programmatic changes required will be documented and this Quality Assurance plan shall be amended as necessary. D.1 Data Validation and Usability 1. Continuous radon data shall be reported using the standard reporting provided by the Corentium Pro, which is corrected for instrument background. 2. The radon measurement technician performing the measurement shall adhere to all applicable protocols and standard operating procedures to ensure that measurements are properly conducted. The Quality Assurance Officer or designee shall routinely review measurement results prior to them being reported to the client. page 7 of 18

Appendices Appendix A - Activated Charcoal Device Placement Checklist (An electronic version of this checklist may be implemented) Placement Obtain activated charcoal device(s) for site to be tested. Record test kit number(s),. Open the packet being careful not to tear the brown paper inside. Insert the foam block into the opening per the kit instructions. Record start time and date on packet and here - Date Time Initials. Hang the packet(s) at normal breathing level. Attach information sign (example is illustrated on the following page) where clearly visible. Place on lowest occupied level of the home Do not place in drafts from heating or air conditioning vents Do not place in closets, kitchen, bathroom, laundry room or other closed or high humidity areas (bedrooms are good spot) Place at least 20 inches above the floor Place at least 10 inches from other objects Place at least 36 inches from windows and 12 inches from exterior wall Retrieval Retrieve the device, remove foam block and seal the packet. Record stop time and date on packet and below. Initial below to verify the time the test kit was sealed - Date Time Initials. Place the devices in mail as soon as practical. page 8 of 18

Appendix F - Standard Operating Procedures for Radon Measurement Devices SOP1: Procedure for Airthings Corentium Pro Placement Mount CRM on Provided Tripod Place on lowest occupied level of the home Do not place in drafts from heating or air conditioning vents Do not place in closets, kitchen, bathroom, laundry room or other closed or high humidity areas (bedrooms are good spot) Place at least 20 inches above the floor Place at least 10 inches from other objects Place at least 36 inches from windows and 12 inches from exterior wall Initiate a new test by pressing and holding button A from the picture below until the device beeps audibly. The device completes a 45 second self-test. A new test is initiated. Testing Duration If closed house conditions are known to be have been maintained for the 12 hours leading up to the test, leave the test running for a minimum of 48 hours. If closed house conditions are unknown or known to not have been maintained the 12 hours leading up to the test, leave the test running for a minimum of 60 hours. Retrieval End the current test by pressing and holding button A from the picture below until the device beeps audibly. Connect the ios of Android smartphone to the device via Bluetooth and retrieve the results via the Airthings Pro application. page 14 of 18

SOP2: Procedure for Air Chek Activated Charcoal Devices Placement Using a waterproof pen (no pencils please), please print clearly your name and complete address, then circle the starting hour and day of the month, and the location of the test in the building. Insert the included foam filter into the "squared" opening of the packet. Push the foam in ONLY enough to fill the opening. Match up the top of the foam with the edge of the packet opening. Make sure the foam fills the opening and is holding the packet open as shown here. Now hang the packet, from the hook provided, at your normal breathing level. Testing If closed house conditions are known to be have been maintained for the 12 hours leading up to the test, leave the test running for a minimum of 48 hours. If closed house conditions are unknown or known to not have been maintained the 12 hours leading up to the test, leave the test running for a minimum of 60 hours. Retrieval Remove and discard the foam-filter-plug. Fold back the envelope flap. Remove the adhesive's protective cover and press flaps together to seal. DO NOT REMOVE from the test area until the packet is sealed. Make certain the packet is sealed air-tight. Repair any tears with a small piece of plastic tape. Leaks will invalidate the test. Mail the device to the lab within 24 hours via either USPS priority mail, or UPS/FedEx 2-day mail. page 15 of 18
