Cleaning Contract Maintenance Specifications General Information The Janitorial Service (hereinafter called Contractor) hereby agrees to perform the following outlined tasks and Maintenance Specifications. The Contractor agrees that the Maintenance Specifications can be tried and changed if it is not what Highline Electric Association (hereafter called HEA) needs to maintain its facility to HEA s expectations. I. ALL OFFICES, HALLWAYS, CUBICLES, RECEPTION AREA AND DRIVE-UP A. Daily Duties Empty all trash containers. Wash the containers as needed to maintain them in a clean condition. Vacuum all open carpeted areas. Clean all counter tops (Reception area). Dust all furniture, telephones, cabinets, picture frames, ledges, window sill, and partitions on a rotating basis, so everything will be done in a weeks time. Main areas will be done daily. Dust mop all resilient floors as necessary. Wet mop all resilient floors as necessary. Arrange all furniture in a neat and orderly fashion. Spot clean carpeted areas (five to ten minute task). Spot clean all door glass. Clean entry areas. Sanitize drinking fountains. B. Periodic Duties Clean all inside glass partitions as needed. Spot clean doors, door frames, wall and switch plates as needed. Clean all door glass. Dust baseboards and vacuum edges of carpet. Dust light fixtures and clean out bugs as needed.
II. BOARDROOM Empty all trash containers. Check daily but clean as necessary. III. RESTROOMS (All four) Empty all trash containers. Empty and replace tampon bags. Check daily and refill all dispensers, as needed (Toilet paper, hand towels, soap, etc.). Clean and polish all mirrors and bright work. Clean and sanitize all sinks. Clean and sanitize all toilets and urinals. Sweep floors and wet mop with disinfectant. Spot clean partitions, walls and frames. B. Periodic Duties Machine scrub floors as needed. IV. STORAGE ROOM, COMPUTER ROOM, JANITORS ROOM AND VAULT Empty all trash containers. Dust mop tile floor as needed. Wet mop tile floor as needed. B. Periodic Duties Vault cleaning will be upon request and arrangements made for door to be unlocked. Scrub and wax tile floors as needed. V. LINEMAN ROOM, BREAK ROOM, SMOKING ROOM, HALLWAY AND LINE SUPERINTENDENT OFFICE Empty all trash containers. Wash the containers as needed to maintain them in a clean condition. Dust mop all resilient floors. Wet mop all resilient floors as necessary.

Dust mop all resilient floors. Wet mop all resilient floors as necessary. Dust all furniture, computers, telephones, cabinets, picture frames on a rotating basis, so everything will be done in a weeks time. Main areas will be done daily. Arrange all furniture in a neat and orderly fashion. Empty and clean all ashtrays. B. Periodic Duties: Spot clean doors, door frames, wall and switch plates as needed. Clean all door glass. Dust baseboards. Scrub and wax tile floors as needed. VII. LOCKER ROOM A. Daily: Sweep cement floor. B. Periodic Duties: Twice a year wash lockers. VIII. WINDOWS A. Periodic Duties: Contractor will wash all interior and exterior windows twice a year.

PROPOSED SUPPLY LIST AND DESCRIPTION Listed below are products currently furnished by present contractor. Whatever products you use and furnish, Material Safety Data Sheets (MSDS) must be furnished also. Inspection: Regular walk throughs can be made by the contractor (Janitorial Service) and building supervisor (HEA). Supervision: Supervision of all employees will be provided by contractor. Termination: This agreement may be terminated by either party with a thirty (30) day written notice. Terms: The monthly custodial charge includes all costs for cleaning supplies, supervision, wages, payroll and appropriate taxes and insurance s required to fulfill the specifications as outlined in this proposal. Cleaning Contract General Information: The cleanable areas of all Offices, Cubicles, Reception, Drive-up, Restrooms, Janitor Room, Board Room, Storage, Computer Room, Vault, Break Room, Lineman Room, Meter Lab, Smoking area and Locker Room. As per Cleaning Contract General Information and Maintenance Specifications. Prescribed Hours for Cleaning Service: Routine cleaning will be done after business hours five times per week, or as scheduled. Supplies: All cleaning supplies and equipment will be provided by contractor unless decided upon before contract is signed. Paper products and room deodorant not included. Holidays: Observed holidays will be New Year s Day, Memorial Day, Fourth of July, Labor Day, Thanksgiving Day, and Christmas Day. Contract Billing: Monthly fees will be due the tenth of the month for all services performed in the previous month. A statement will be sent. Special Charges: Special Charges will be billed separately from the contract. Special cleaning or maintenance of areas not specified will be charged at a negotiated rate for all work performed. Insurance: Contractor must have a minimum of $500,000 General Liability, Bonding Insurance, and Workman s Compensation Insurance. All contractor employees must be covered by these policies. Current Certificates of insurance coverage must be provided to HEA at all times (when bid is awarded and every insurance renewal thereafter).
