4. KITCHEN Do you prepare meals in the kitchen? If so, do you have any difficulties with preparing meals? Safety concerns? 6. LAUNDRY Do you do your own laundry? Any difficulties? Build a counter for sorting and folding clothes Adjust clothesline to convenient height Relocate laundry appliances Clean dryer vent or replace with metallic hose 7. BASEMENT Do you use the basement? If so Identify and eliminate sources of water in basement (usually gutters our plumbing) Add additional lighting as needed Remove combustible materials and hazardous waste Clear pathway to utilities

8. TELEPHONE AND DOOR Any difficulty using your phone or identifying visitors? Do you have a personal emergency response system? Lifeline? Answering machine? Install wireless phone system near bed, sofa, and chair Carry phone in fanny back, on walker, etc. Install peephole at convenient height Install flashing light or sound amplifier to indicate ringing doorbell for those with visual or hearing problems Install mailbox at accessible height 9. STORAGE SPACE Any difficulty reaching clothes in your closets or other closets you use regularly? Install adjustable closet rods and shelves to appropriate height Install bi-fold or pocket doors

10. WINDOWS Any difficulty opening and closing windows you use regularly? Install lights inside closet Install handles and locks that are easy to grip, placed at appropriate heights Replace windows or storms that are not functional 11. ELECTRICAL OUTLETS AND CONTROLS Do you have enough outlets for your needs? Install light fixtures or outlet for lamps Install switches at top and bottom of stairs Install Ground Fault outlets in kitchen and bathroom Install wireless light switches where needed

12. HEAT, AIR, LIGHT, SMOKE, CARBON MONOXIDE, WATER TEMP CONTROLS Do you have adequate light, heat, water temperature control? Smoke and carbon monoxide detector?
