Fire rated doors has been installed but few problems has been observed such as door coordinators are not properly working. Also few non listed door has been found in the N-W corner exit stair. Dining Building: Exit stair is not enclosed by fire rated construction. Stair shall be enclosed with a minimum 1-hr fire-rated construction. Seal all unprotected openings and provide minimum 1- hr fire rated doors to separate the exit stairs from work areas and other building spaces on all floor levels. Ensure that the fire doors are self-closing, positive latching and that they are provided with fire exit (panic) hardware where serving production floors. If fire doors are required to be held open for functional reasons, provide automatic-closing devices tied to the fire alarm system. Note: Doors assemblies shall conform proper fire rating as mentioned in Accord standard (Section 4.5.4, Part-4). 2 Unsealed penetrations and openings were located in the fire rated floor and ceiling assemblies. Location: All floors (N-E & N-W Corner) Provide a minimum 2-hr fire rated shaft to separate the utility risers and elevator from each floor level. or Seal all penetrations and openings in floor and ceiling assemblies to maintain the fire separation.

3 Areas used for in-process storage of combustible materials are open to the surrounding occupancy. Location: 1st, 2nd & 6th Floor Provide dedicated storage rooms separated by minimum 1-hr firerated construction. Or Provide defined storage areas and limit the storage arrangement as follows: - Maximum height of 2.45m (8ft.) and maximum area of 23m2 (250 ft2) - If sprinkler protected: maximum height of 3.66m (12ft.) and maximum area of 93m2 (305ft2) Separate areas of unenclosed combustible storage by a minimum clear distance of 3m (10ft.)

6 Loading/Unloading (in side the building) area is not separated by the fire rated construction from North part of the building. Un protected openings has been observed. Separate the interior loading/unloading area by minimum 1-hr fire rated construction. Seal all penetrations and openings in floor and ceiling assemblies to maintain the fire separation. Within 03 Months 7 Egress Egress doors with locking features are provided at main exits (ground level) of the building. Remove locking features from all egress doors and gates. If locks are required for security reasons, utilize special door locking features complying with NFPA 101 and ACCORD standard 6.8.2. Immediately

8 Egress Sliding gates are provided at main exit (ground level) of the building. Replace all gates along the means of egress with side-hinged, swinging egress doors. If locks are required for security reasons, utilize special door locking features complying with NFPA 101. Within 1 month 9 Egress Doors of idle machine room installed such a way which is making an obstruction to reach exit stair. Location: N-E corner (Main building) Relocate the position of door of idle machine room. Or Modify the swing direction of the door of idle machine room. 10 Fire Protection A listed fire alarm was under process of installation. The factory has been submitted the design of fire alarm system to Accord for review. Acceptance test and commissioning shall be done as per NFPA 72. Within 03 Month 11 Fire Protection Inspection, testing, and maintenance for the fire alarm system was not in accordance with NFPA 72. Inspect, test and maintain the fire alarm system, and keep written records on-site, in accordance with NFPA 72. Within 3 months

12 Fire Protection Class -III standpipe is required for the building. However Class-II standpipe system has been installed. Install 2.5" landing valve in each stair enclosure to upgrade the system into Class-III. Within 03 months 13 Fire Protection Inspection, testing and maintenance of portable fire extinguishers is not in accordance with NFPA 10. Inspect, test and maintain the portable fire extinguishers and keep written records onsite, in accordance with NFPA 10. 14 Fire Protection 15 Fire Protection 16 Egress Lighting Egress Lighting 17 Inspection, testing and maintenance of the standpipe system is not in accordance with NFPA 14. Inspection, testing and maintenance of the fire pump system is not in accordance with NFPA 20. Inspection, testing and maintenance for the emergency lighting system was not in accordance with The ACCORD standard. Based on the number and location of emergency lights observed, adequate illumination levels are not anticipated along egress routes. Location: Main & Dining Building Inspect, test and maintain the standpipe system and keep written records on-site, in accordance with NFPA 14. Inspect, test and maintain the fire pump system and keep written records on-site, in accordance with NFPA 20. Inspect, test and maintain the emergency lighting system in the accordance with The ACCORD standard. Keep written records onsite. Test the emergency lighting system on each floor and provide additional emergency fixtures to provide adequate illumination along the means of egress. Provide a minimum illumination of 10 lux at the floor level within exit stairs and exit discharge paths and minimum 2.5 lux along exit access aisles.
