INTRODUCTION Indications for Use The PlatinumGMT IPL System Cloud device (including the hand unit used to deliver pulsedlight energy) is indicated for use in surgical, aesthetic and cosmetic applications in permanent hair reduction, moderate inflammatory acne vulgaris, benign cutaneous pigmented and vascular lesions. Permanent hair reduction is defined as the longterm, stable reduction in the number of hairs regrowing when measured at 6, 9, and 12 months after the completion of a treatment regimen. PlatinumGMT IPL System Cloud features all of the technological advances needed to perform safe, efficient, fast and very comfortable treatments. The machine is designed for the progressive reduction of unwanted body hair, treating moderate inflammatory acne vulgaris, and treating pigmented and vascular lesions. PlatinumGMT IPL System Cloud works based on the selective photothermolysis principle, in which pulsed light is adjusted to selectively treat specific conditions without affecting the adjacent tissue. PlatinumGMT IPL System Cloud consists of three main parts: the central unit, the control cover with touch screen, and the hand unit, which contains the luminance unit (Xenon lamp and sapphire lens). By using the touch screen, the operator of the PlatinumGMT IPL System Cloud can adjust the power level parameters to suit each client s specific hair and skin characteristics. The energy level parameter defines how much energy is delivered to the site to be treated. 2. PRECAUTIONS Users should carefully read this Manual, its precautions and protocols before working with the PlatinumGMT IPL System Cloud. PlatinumGMT IPL System Cloud is an electrooptical device designed to offer safe and reliable treatments. Users of this equipment must strictly observe the security precautions given in this section. PlatinumGMT IPL System Cloud is not designed to be used by people (including children) with reduced physical, sensory or mental abilities, or who lack experience or knowledge, unless they are supervised or have received training on its proper use. Children should be supervised to ensure that they do not play with the machine. PlatinumGMT IPL System Cloud is designed to maximize the safety of those receiving (clients or patients) and applying (operators) the treatment. Any area of the body can be treated, subject to the following precautions: Hair Removal (HR) 1. Use only on areas that DO NOT exhibit the following characteristics: Wounds, injuries, redness, edemas, erythemas, crusts, burns, irritation, dermatitis or any change in the normal skin. Natural or artificial tanning agents of any kind. Hair removal using waxing methods. Tattoos, moles, dark spots or any hyperpigmented skin. Dermal Lesions 1. Pigmented lesions, including mainly solar lentigines, melasmas and chloasmas. Any kind of lesion exhibiting the following characteristics: Does not project above the surface of the skin. Is not painful. 5

6 Brown color (spots like coffee with milk) Exhibits different degrees of coloration. Is not perfectly symmetrical. 2. Vascular lesions: Includes telangiectasias, redness associated with rosacea or flushing, broken capillaries, vascular spiders. b. Telangiectasias The chromophore is the hemoglobin that is in the capillaries. This absorbs the heat. It does not have a coagulant effect. Only small, superficial telangiectasias can be treated. IMPORTANT Before starting any treatment, carefully read the list of contraindications and the exclusion criteria. 3. WORKING PRINCIPLE Principle of Selective Photothermolysis When applied to the skin, the PlatinumGMT IPL System Cloud intense pulsed light (IPL) is absorbed by the chromophores in the treated area. The light energy absorbed by the chromophores is converted into heat energy, which acts to coagulate cells. 1. Hair Removal a. Working Mechanism. IPL works by using light to produce an increase in the hair s temperature. The light, which is absorbed by the hair s melanin and not by the skin (to be precise, to a lesser extent by the skin, which is lighter than the hair), heats the hair to 70 / 80 C, carbonizing and coagulating its root. The thermal relaxation time allows the skin to constantly reduce any excess temperature, thus avoiding damage. 2. Dermal Lesions a. Radiation spots The high-energy light is absorbed by the melanin chromophores. This makes the temperature of these cells rise, altering the melanin, which is eliminated by macrophages and the area repopulated with new cells. 4. SYSTEM SAFETY Though the PlatinumGMT IPL System Cloud is a safe mechanism for the eyes, clients must wear protective glasses during facial treatments to shield them against any eye damage. Only personnel authorized by Global Medical Technology should perform any work inside the PlatinumGMT IPL System Cloud protective covers. Inside, the system has dangerous electric voltages and currents. Ensure the proper voltage is supplied to the PlatinumGMT IPL System Cloud (see the label at the base of the central unit). The operator s routine maintenance should be performed at least 10 minutes after the system is shut down and disconnected from the power main. Carrying out the maintenance procedures with the system plugged in or within 10 minutes of disconnecting it can be dangerous to the operator and can damage the system. Electrical and mechanical safety Do not open or remove any covers on the PlatinumGMT IPL System Cloud. Removing any covers poses a safety hazard. PlatinumGMT IPL System Cloud produces high voltages, which can be dangerous. Some components can hold a charge once the power is off. Only personnel authorized by Global Medical Technology should remove the covers. PlatinumGMT IPL System Cloud is connected to the ground through the connection in the power cord. This connection is essential for safe operation. Be sure the electric output at your site is properly connected to the ground. 6

7 Fire danger The photothermic energy absorption increases the temperature of the absorbent material. You should take preventive measures to reduce the risk of flammable materials in the place of treatment. Do not use flammable substances, like alcohol or acetone, when preparing the skin for treatment. Do not use flammable substances to clean or disinfect any part of the PlatinumGMT IPL System Cloud, mainly the spot (glass on the hand unit). Labels The following labels are attached to the rear of the PlatinumGMT IPL System Cloud. The owner of the equipment should be familiar with the meaning and location of these labels and check them regularly. Dangerous voltage inside. IMPORTANT Never turn on the machine without connecting the hand unit first. Never use the machine with a low water level. Warning that only authorized personnel should work inside the PlatinumGMT IPL System Cloud unit, which contains dangerous electrical voltages and currents. WARNING > Sending excessive energy to the treatment site can cause thermal damage to the skin, crusts, redness of the skin, hypo or hyperpigmentation. > Never look directly into the main lens, as this can injure the eye. > Never turn the machine on without first placing the hand unit. > Never aim the hand unit to emit a pulse of light into space. Be sure to aim it only at the treatment site. > All light-emitting device pulses can cause injuries if not used correctly. > There are high voltages inside the PlatinumGMT IPL System Cloud. > Personnel who work with pulsed light sources should always be aware of the possible danger and take the corresponding precautions, as described in this User Manual. > Warning- With an IPL device, opaque eye-shield protection should be worn during treatment. > Warning- Children must not be treated with this device, even with adult supervision. This machine complies with European standards for marketing. Information on the manufacturer, trademark, and applicable standards and directives. Rated voltage of the unit. 7

8 Characteristics of the treatment room The room or cabin in which the treatments will take place with the PlatinumGMT IPL System Cloud must comply with the following requirements: a) The cabin will be clearly identified and have a WARNING sign at the main entrance, like this: Room with INTENSE PULSED LIGHT EQUIPMENT Safety glasses required b) There should be 60 centimeters between the machine and the closest wall. The dimensions of the machine are: Wide: 41 cm Long: 48 cm Height: 103cm (150 cm with the hand unit support) c) The machine should be far away from any heat sources. d) Electrical requirements: V 50/60Hz 12A e) The presence of corrosive materials in the work area can damage the electrical system of the machine and its optical components. f) The presence of a high dust concentration and suspended particles in the work area can damage the machine by getting inside the covers. It can also absorb a portion of light emitted by the system. WARNING Room with INTENSE PULSED LIGHT EQUIPMENT Safety glasses required g) The presence of metallic dust in the environment can damage the machine. The PlatinumGMT IPL System Cloud should be used in a non-corrosive atmosphere. between 20 and 25º C and a relative humidity under 80%. The work area should be air conditioned. Optical Safety Requirements The following recommendations should be followed meticulously to avoid harmful effects to the eyesight of both operators and patients. Be careful with accidental exposure to the light emitted by the machine. Everyone present in the treatment area should use special protective glasses, provided by Global Medical Technology. Clearly identify the work room so as to avoid having anyone enter unprotected. Only those whose presence is required should be in the treatment room. Anyone involved in providing treatment must be properly trained on and informed about the use of the equipment. Never look directly at the light emitted by the machine. Never look directly at the lens through which the light is emitted when the machine is on. Never look at the reflective surfaces during a treatment. Never aim the hand unit at any surface or object that is not specified in this user manual. Not work with the machine close to any flammable or explosive substances. Never allow the presence of reflecting surfaces, like mirrors, glasses, watches, jewelry and surgical instruments, in the treatment area. Power key The power key on the machine connects and disconnects the power to the system. The machine works only with the power key supplied by Global Medical Technology. IMPORTANT Never leave the key in the machine unsupervised. h) Temperature of work area: The work room should be kept at a constant temperature 8

9 Ensure the system is not on when you remove the power key. Safety systems on the machine Emergency circuit breaker (Stop) One automatic breaker (Red Stop Button) located at the front part of the machine controls the electricity consumption of the main unit, automatically interrupting the power in case of an accident, overcurrent or any irregularities in the electrical supply. Moving the machine To move the machine from one place to another, observe the following requirements: 1) Unplug the power cord from the outlet. 2) Disconnect the hand unit. 3) Lift the brake wheels. 4) Push the machine slowly on its wheels. Auto Test When the machine is turned on, a self-test checks the electrical system and the network connection. This process is carried out every time the hand unit is changed, to avoid overload or the existence of remaining amounts of energy that may alter the amount of energy delivered with every pulse. Machine classification Electric shock protection: Class I, Type BF. Anti-liquid protection: Ordinary machines. Work mode: Continuous. System Installation Follow these steps to install the PlatinumGMT IPL System Cloud: Unpack the machine and place it in an appropriate place. Check the machine s integrity and its components Fill the tank with distilled water. Connect one hand unit. Plug the machine into an electrical outlet. Switch on the machine by turning the power key. Ensure that the control screen turns on correctly. IMPORTANT Never turn on the machine without connecting the hand unit first. Never use the machine with a low water level. 5) The machine should not be put on its side or in any position that is not vertical. 6) If it must be moved horizontally, empty the water tank and put a stopper in the fill valve. 5. The machine consists of a console with an internal computer (which ensures the monitoring of the working parameters), a water pump, a control cover and two hand units. 9

10 The control cover consists of a touchscreen LCD. The options can be selected by directly touching the screen with a finger or with a suitable pen. The hand unit is made up of a light source (Xenon lamp), a filter (sapphire lens) and a light guide. Light reflector Lamp Filter Light guide Hand unit The hand unit is the part of the machine that contains the light emission and filtering system. It is connected to the main unit by a power cable. The lens that shapes the filtered light protrudes from the surface of the head. The light is activated when the push button on the handle of the hand unit is pressed. The area covered by each shot is about 8 mm x 34 mm (272 mm²) or 450 mm2 (10 mm x 45 mm). The main lens should be clean at all times. Push button Hand unit Main lens Cooling System (inside) 10

11 CONTROL COVER WITH TOUCHSCREEN The touch screen allows for the simple and convenient operation of the machine. The graphical user interface is a program that uses images and graphic objects to represent the information and actions available. Their primary use is to provide a simple visual environment to communicate with the machine s operating system. The actions are performed through direct manipulation (touch screen), to facilitate user interaction with the machine. The program consists of a series of screens that, as a whole, provide the operator with tutorials or guides for each treatment. It also shows additional information in the form of help, if required, by pressing the appropriate button. 12 General Working Parameters 1) Simple pulse. Emits one pulse during the shot. A 9.9-ms duration is recommended. 2) Multipulse System. Emits a sequence of pulses during one shot. A maximum of 6 small pulses can be specified. 3) Pulse Duration. Emission time of the light ray. Generally the first pulse should be longer. 4) Interval between pulses. Time between each of the small pulses emitted in the multipulse system. Provides thermal relaxation, allowing the skin to cool between pulses. 5) Interval between shots. Can be programmed for a period between 1 and 3 seconds. This is only effective when the button on the hand unit is kept depressed. 6) Amount of energy emitted. The machine allows working with an energy flux range between 10 and 60 J/cm² for IPL. Help screens Pressing the symbol brings up information on the associated parameter. Thus, we have: In addition, the operator can adjust the sound and language parameters by pressing Settings. Once adjusted, press RETURN to save the changes and go to the main screen. 12

13 Modifying the treatment parameters The parameters can be adjusted during treatment. The energy output, pulse width, and number can be changed to adjust the treatment parameters and vary the effects of the therapy. Width of the first pulse: adjusts the width of the first pulse. Its range is between 1 and 9.9 ms. First delay: adjusts the delay between the first pulse and the following pulse, the range is between 1 95 ms. Shot power: adjusts the energy output through the lens. During the treatment, the energy window will display the current energy output value, in a range between 10 and 60 J/cm2. This can be adjusted directly during the treatment. To do so, press the upper or the lower triangle to increase or decrease the energy, respectively. Keeping either button pressed will quickly change the value of the output energy. Pulse number: selects the number of pulses, between 1 and 6. Discharge interval: adjusts the frequency of the shot repetition when the button on the hand unit is kept depressed. Its range is 1 to 4 seconds. Width of the second pulse: adjusts the width of the second pulse. Not shown when working in simple mode. The range is ms. Delay between the second and the third pulse: adjusts the delay between the width of the second and the third pulse. The range is 1 95 ms. Width of the repeated pulse: adjusts the time between the third and future pulses. The range is 1 to 9.9 ms. Repeated delay: adjusts the delay of the repeated pulse. The range is from 1 to 95 ms. 14 6. TREATMENTS The instructions below must be followed when using PlatinumGMT IPL System Cloud to safely remove hair. This information is based on the observations of clinicians experienced in the use of this technology. This document is intended as a guide. It is not a substitute for professional training, certification, and supervised practice. For correct use, follow the instructions of this user manual. The use of a large amount of energy or of repeated or excessive pulses can cause overtreatment, leading to undesirable results, like crusts, blisters and temporary changes in pigmentation. Careful monitoring and a conservative treatment will avoid these problems. Therapeutic indications The PlatinumGMT IPL System Cloud can be used for treating different parts of the body in patients with skin type I, II, III or IV on the Fitzpatrick skin chart. Exclusion Criteria for hair removal The following must be not treated: Persons under the age of 18. Pregnant and lactating women. Recently waxed areas. History of keloid formations or other skin diseases. Open wounds. Areas exposed to the UV-radiaton (sun, tanning bed) in the last 30 days. Clients with hirsutism or hypertrichosis. Clients who have taken isotretinoin (Accutane) in the last 6 months. Clients who have taken photosensitive medications in the last 4 weeks. Clients with epilepsy. Clients who want to eliminate hair from tattooed areas. Eyebrow hair. Clients who are allergic to sunlight, such as those suffering from porphyria, lupus, solar urticaria, etc. Pre-treatment considerations During the first visit, the areas to be treated must be evaluated. The following steps are also required: 1. Medical Record Fill out the client card, including previous treatment methods. 2. Determine if the client is suited to treatment with PlatinumGMT IPL System Cloud. 3. Explain to the client what treatment with PlatinumGMT IPL System Cloud entails. 4. Explain to the client: The hair growth cycle for different parts of the body. This will help clients understand the necessary treatment program. That the effective elimination of hair can require several sessions. That they may experience discomfort during the treatment. That erythema, edema or temporary blistering is possible immediately after the treatment. That there is a risk of side effects, like hypo or hyperpigmentation of the skin. These are generally temporary. 5. ANESTHESIA The use of conductive gel or cold gel strips is recommended. Some people and some areas are more sensitive to treatment with PlatinumGMT IPL System Cloud. That is why the use of a local anesthetic cream (EMLA, Elamax, Topicaína) is allowed to relieve any discomfort. If a topical anesthetic is used, follow these steps: Before the procedure, spread the ointment directly on the skin. The ointment s effect can last for up to three hours. Do not rub the cream on the skin. Cover with an occlusive dressing. I mmediately before the procedure (not less than 60 minutes after the ointment 14

15 is applied), remove the dressing and the cream. Carefully clean the entire area to remove any residual ointment and begin the procedure. 4. Wait about 10 or 15 minutes to see the clinical response to the treatment. Select an energy level at which changes to the skin are visible with no side effects. 6. Before the treatment, the client should sign the informed consent form (ICF). 7. EVALUATION - Evaluate the client skin type and decide if it is sensitive to the sun. Does the skin in injured areas heal in a darker color? This can reveal potential pigmentation changes. 5. THERAPEUTIC OBJECTIVES - Some visible effects immediately after the treatment, or within 10 to 15 minutes, are: Whitening of the hair. Darkening/burning/thinning hair. Perifollicular erythema. Redness of the skin around the hair. 8. DETERMINE THE SENSITIVITY OF THE SKIN Is the patient s skin easily irritated while shaving? Did he/she have a reaction to wax hair removal? Is the skin sensitive to creams or lotions? 9. Do a test by applying 2-3 shots, as shown below. Pre-treatment preparation 2. Cut before the treatment, the hair must be cut to a length of about 1 mm, or the area shaved 4 to 6 days before the treatment until it grows to the desired length of 1 mm. 6. ADJUSTMENT FOR THICK AND DENSE HAIR - reduce the energy for the initial treatment until hair grows finer and thinner. Increase the energy for the subsequent treatment if necessary. 7. ADJUSTMENT FOR FINE HAIR - If the energy level used for most of the hair works, but not in areas with fine hair, increase the energy level to reach the clinical point. 8. TREATMENT OF FACIAL AREAS 3. Clean thoroughly clean the treated areas with soap and water. DO NOT USE flammable materials like alcohol / acetone for cleaning. 4. Dry dry the skin to be treated. Do not leave anything wet close to the treatment area. 5. Moles Moles should be covered with a white adhesive. Do not use flammable materials. Test procedure 1. Evaluate the client s skin type as per the aforementioned Fitzpatrick chart. 2. Select the test location. 3. Once the skin is prepared, apply two or three pulses to the test area at two or three different energy levels, taking into account the client s skin type, as shown in the variation table of the test spot. 9. The PlatinumGMT IPL System Cloud must not be used to treat areas around the eyes. Areas near the eyes must be treated with extreme care, and only those areas: From the eyebrows up. Leaving two centimeters from the corners of the eye. From the cheekbone down. 10. CLEANING AND MAINTENANCE OF THE HAND UNIT - After each treatment with PlatinumGMT IPL System Cloud, the hand unit must be cleaned and dried. Keep moisture from entering the lamp cover. Check the integrity of the lamp and the protective screen before each treatment with PlatinumGMT IPL System Cloud. 11. INTERVAL - The interval between the treatment sections with PlatinumGMT IPL System Cloud depends upon the area. Different parts of the 15

16 body can have different growth cycles. As a general guideline, treatments should be 4 to 6 weeks apart. Every individual has a different amount of hair with different needs. Cleaning the hand unit The hand unit should always be cleaned after use. For reasons of hygiene, never use on a patient without clearing the main lens. Some clients might want to be treated again earlier, but the PlatinumGMT IPL System Cloud is most effective when the hair is at least 1 mm long. After some treatments there should be a greater interval between hair growth cycles, depending of the amount of hair and on the client s particular growth cycle. Post-treatment precautions Avoid exposure to the UV-radiation (sun, tanning bed), and use a sunscreen for a month after the treatment. If edema appears, cool the area for 15 minutes until the burning sensation disappears. Typically, a minor edema lasting one day or a minor erythema lasting 1 to 3 days should be monitored. If the side effects last longer than this, the client should see a doctor. The night after the treatment clients must avoid hot water, clean the skin gently with warm water, and keep it hydrated. Makeup can be immediately applied. 7. MACHINE MAINTENANCE The safe and effective use of the PlatinumGMT IPL System Cloud requires proper care in order to keep the machine in good working condition. Cleaning the main unit The machine must be clean at all times. Before cleaning the main unit or the hand unit, unplug the machine from the outlet. Do not use flammable and abrasive substances. Use a soft cloth and specific cleaning solutions for metals. The machine must be cleaned at least once a week, primarily the grooved parts where the air that cools the water circulates. The following approved cleaning procedure should be followed: Soak a gauze pad in a disinfecting solution (we recommend CIDEX OPA solution) and clean the surfaces (concentrating on the lens). Wipe the surfaces dry with 2 gauze pads. Inspect the surfaces. Repeat if necessary Switch knob Light guide crystal Figure 3.2 head diagram Never submerge the hand unit in liquids. If liquid contacts the unit s internal components, unplug from the power source and immediately call the Global Medical Technology technical service on (+34) Changing the distilled water The machine may overheat if the water level is low. Check the water level daily. Every 3 months of continuous operation or each pulses, the water must be replaced. Use distilled water only. Any other kind of water will result in the formation of calcareous plates inside the system, causing it to break. To change the water: a) Insert the short hose in the drain valve (Drain) until the tank is completely empty. Handle TE cooler (inside) 16

17 b) Insert the short hose in the overflow valve to let the air out while the tank is filled with water. c) Connect the fill hose between the bottle and the water inlet valve. d) Fill with water until the transparent indicator exceeds the minimum level indicated, or better yet, until water issues from the overflow valve. e) Remove the hoses from the valves. Fill water Duration and replacement of the hand unit Each lamp has an indeterminate lifetime that depends on the power used during treatments. The PlatinumGMT IPL System Cloud machine is rated to emit 150,000 pulses, guaranteed by a certificate for each hand unit. After the 150,000 pulses, there is usually a small loss of power in the lamp, which is why it is necessary to gradually increase the power of the machine to obtain similar results, until this process stops working, meaning that despite increasing the power on the machine, the hand unit does not produce an effective treatment, at which time it will be necessary replace it. 19 9. APPENDIX On the use of the hand units and lenses of the PlatinumGMT IPL System Cloud: 1) Replace the lens when the machine is off. 2) Store the lens in a safe place. Warning! The components can be damaged if struck, if there is an abrupt environmental change, or if the hand unit is removed. If you see anything abnormal, please stop the machine and do not use the hand unit. 3) After using the lens, allow it to return to its normal temperature to minimize wear and tear of the components. 4) Clean the outside of the light guide with a soft cloth. Warning! Clean the glass on the light guide after using it. 5) Be sure to use the correct gel to obtain a safe and efficient treatment. Warning! Improper use of the hand unit or using an inappropriate gel may damage the light guide on the lens. It could also cause bad treatment results, such as redness and slight damage to the skin. 6) Read this user manual carefully before using the machine. 19
