Purpose / Background The MIT building inspection process is conducted yearly by the Cambridge Building Inspector and MIT personnel to ensure all laws and related City Ordinances which pertain to the Massachusetts State Building Code are implemented. The inspection process fulfills the city requirements to receive the Certificates of Inspection for Assembly Areas, liquor licenses, and to ensure the health, safety and welfare of the MIT community. 2. Scope This procedure applies to the Department of Facilities, the Department of Student Life, the Environmental Health and Safety Office, and DLC-EHS Coordinators (when applicable or requested) working in conjunction with City of Cambridge Building Inspector s office. 3. Prerequisites Not applicable. 4. Procedures Frequency Building inspections occur annually, usually starting in August or September and continuing until they are completed. Generally, the start of these inspections is dependent upon the schedule of the city inspector. The building inspector will look at all areas of assembly, places with 49+ people, (such as dorms, CAC buildings, athletic buildings, class rooms, conference rooms, and areas requiring a liquor license), and the corresponding egress routes out of the building. MIT is not required to inspect buildings that do not have assembly areas, with the exception of dorms. Areas of non-compliance with the Massachusetts State Building Code and the International Building Code will be identified, documented, and corrected prior to the next year s inspections, unless otherwise noted, and approved by the city. Inspection Team The composition of the inspection team should consist of the City Building Inspector, the Zone or House manager, and a representative from the EHS Safety Program staff. In large DLC s such as DoF and DSL, the EHS SP staff member can be substituted with the DLC-EHS Coordinator. Process The following is a recommendation for the building inspection process, and is susceptible to change. An intuitive first step is to look at the available evacuation map that is generally posted near the main elevator. This will indicate the evacuation routes so the inspection team can evaluate if the exit signs are adequate. When / if the inspector would like to see the fire alarm panel, they are generally located at the building entrance. After evaluating the assembly area, inspect the corridor in either direction and the stairway. Then inspect the other assembly areas in the building systematically on the way down. If fire doors are found propped open, remove and discard the door chocks. If the inspector wants to enter an alarmed stairway or other area, then call before and after entry. Documenting and Recording Findings The Zone Manager or House Manager, along with the EHS SP representative are responsible for maintaining their own record of the findings. The findings recorded by the EHS office will be submitted to building repairs in SAP (the entire building will be entered into one Building Repair Form ). The Zone Manager / Housing Manager will use the list they created to immediately start correcting the findings, and to help eliminate confusion when the findings come out in SAP. The building inspector will indicate during the inspection if he would like to come back and re-inspect at a later time, if not he trusts that MIT will resolve the issue before the next year s inspection. Closing SAP Findings When the building findings are submitted to SAP, it is the responsibility of the zone manager to complete, or delegate the items to be corrected for completion. If there are items on the list that are outside the responsibilities of facilities, than they are to indicate this to the EHS contact or the DLC-EHS contact for that building. When the list has been completed, the SAP work order can be closed out. Hanging Permits After the Certificates of Inspection are paid for by the EHS Office, MIT will then receive the certificates. The EHS Office will sort the certificates by building and then by zone, to be distributed to the zone managers / housing managers. The zone / housing managers will then have the permits hung in there respected locations, according to the Hanging Building Certificates instructions located in Appendix B. Any permits that are not hung according to the instruction provided will be recorded and put into SAP to be corrected. 5. Roles & Responsibilities Department of Facilities - Notify the EHS office of the addition or removal of assembly areas, through renovations, new construction or demolition. - Zone Managers shall be available to participate with the inspections of their buildings / zones. - Give proper funding to fix non-compliance issues found with the building inspection process. - Identify and provide a list or map indicating the areas and buildings that each zone is derived of (Appendix D ZONE MAP). - Hang permits according to instructions provided in Appendix B. EHS Office / Representative - Update list yearly to include occupancy areas that have been created or removed. - Act as the liaison between the Cambridge Building Inspectors Office and MIT. - Participate in all building inspections, unless relieved by the DoF or DSL DLC-EHS Coordinator, for there respected zones. - Document and record findings separate from the recorded zone / housing manager s findings. These findings shall be used to create building repair invoices on SAP. - Maintain a current list of all assembly areas on campus (Appendix A). - Pay for assembly area permits. Ensure we are not paying for duplicates, deleted, or child day care certificates. - Receive and sort certificates of inspection by building and zone, to distribute to facilities zone managers and DSL managers. - Obtain liquor license for alcohol serving establishments. - Make copies of original permits and file in the EHS Office. Zone Managers - Document and record findings separate from the recorded EHS findings. These findings shall be used to immediately start fixing identified non-compliance issues in the manager s zone. - Delegate findings in SAP to the proper individuals to accomplish completion. - Ensure employees hang permits according to instructions provided in Appendix B. DSL Managers - Document and record findings separate from the recorded EHS findings. These findings shall be used to immediately start fixing identified non-compliance issues in the managers building / areas. - Delegate findings in SAP to the proper individuals to accomplish completion. - Ensure employees hang permits according to instructions provided in Appendix B. 6. Training Training for building inspections are not extensive, and can be provided through on the job training. 7. All stairwells and corridors are maintained free of all storage. - A minimum 3 foot clearance is maintained in front of all electrical equipment and emergency equipment including fire alarm pull stations, alarm panels and fire extinguishers. - All areas are clean and orderly and in a sanitary condition to reduce the risk of fire and to prevent slips, trips and falls. - Extension cords are not used in place of permanent wiring. Only approved surge protection strips are used when permitted. - Gas cylinders are properly secured. - Fire and smoke doors are not propped open. They may remain open or held open with magnetic hold back devices only. - Emergency numbers and evacuation procedures are posted in conspicuous locations. - Fire extinguishers are placed, inspected and maintained according to OSHA and NFPA requirements. - Emergency equipment such as fire alarms, sprinkler systems and fire pumps are inspected and maintained in accordance with NFPA requirements. - When provided, automatic door closures operate properly on all fire and smoke doors. - All fire and smoke doors have appropriate fire rating and are properly labeled. - Handrails are provided when required and are maintained in accordance with OSHA requirements. - The appropriate number of exits are provided from each building and they are located in accordance with NFPA, OSHA and building code requirements. - Draperies, curtains, and other similar loosely hanging furnishings and decorations shall be flame resistant as demonstrated by complying with NFPA 701, Standard Methods of Fire Tests for Flame-Resistant Textiles and Films. - Combustible decorative materials such as, but not limited to, cotton batting, paper, foam plastics, cloth, vegetation, moss, straw, hay, vines, split bamboo, leaves and similar material shall not be used unless the materials are flame resistant or treated with flame retardant material. - Bulletin boards are prohibited in enclosed stairwells. Definitions 10.1 Assembly Area A building or facility or portion thereof, used as a gathering place (such as an auditorium, lecture hall or gymnasium) that is occupied by large numbers of persons during major periods of occupancy Egress Routes A continuous and unobstructed path of vertical and horizontal travel from any occupied portion of a building or structure to a public way Fire Door A door with a fire-resistance rating as part of a passive fire protection system to reduce the spread of fire or smoke between compartments and to enable safe egress from a building or structure. 10 Appendix B: Hanging Occupancy Permits Thank you for your assistance with hanging the Assembly Area Permits. Hanging these permits is required by the City of Cambridge Inspectional Service Department. Written on the permit, is the location where each permit needs to be posted. Some post-its notes are on the permits in an effort to reduce confusion, remove the post-its before hanging the permit. Please put the permit into the frame, and re-hang the frame (Employee s should use the black frames that are at some locations on campus). Please do not cut the certificate of occupancy s in order to get them into the frame, as we are required to hang the originals. Additionally, permits that are not located in the rooms will be included into the Building Inspector s findings, and will have to be hung again before the permit is renewed. Room Permits These permits are for specific rooms on campus. Please hang these permits near the main entrance of the room. If there are two entrances that are used equally, you can choose between the two. The majority of permits have already been hung, and the same location should be used. Please be cognizant of the room s aesthetics. Building Permits These permits are for entire buildings on the MIT campus. Please hang these permits near the main entrance of the building on the first floor (preferably in the lobby). The majority of permits have already been hung, and the same location should be used. Please be cognizant of the building s aesthetics. If you have permits that are not in the appropriate zone, please return them to your zone supervisor, so it can be returned to EHS. Also, the EHS Office has a limited amount of plastic replacement frames if needed. The Following are examples of what not to do: Do not hang permits over permits. All permits should be easily seen. Do not hang plastic frame over black framed occupancy holder. Use the black frame to display the permit when available.
