The policy changes relate to the addition of bench and ledger right fees and respective policies, an Outside Dealer Monument Policy, an Outside Dealer Bronze Memorial Policy and a required Unveiling Policy for Customers of Outside Dealers. Attached please find a 2014 cover sheet with the augmented policies and the 2014 Outside Dealer Foundation & Monument Setting Permit Application for your reference. Please note that burial location requests must be made by fax (21.5) The following sections of page 1 of the application must be completed in entirety for your request: Outside contractor, the type of memorial you would like to install, the decedent's full name, the registered lot owner, the outside contractor signature, the signature and address of the owner of the interment rights. Your request will be processed within three business days of our receiving the faxed, request. These changes will expedite the processing of your requests and give clarity to the proper location for the memorial installation. Effective January 1, 2013, the permit price is $ Please submit payment payable to Forest Hills Cemetery/Shalom Memorial Park. 2. Installations of foundations, memorials and inscription work are limited to Monday, Wednesday and Friday between the hours 8:00 a.m. and 3:00 p.m. only. Weekend work is not permitted in the cemetery. 3. It is necessary for you to come into our office before you proceed to the gravesite to do any work. Please present the outside dealer's name, the deceased's name and grave location, the size of the memorial & base, and payment, if necessary, to the representative in the office, 4. To ensure proper grave layout identification, the cemetery requires a 5-business day advance notice of your arrival at the cemetery for placement of a foundation, monument, bronze or inscription. Please submit by fax the appropriate new foundation, memorial or inscription layout request form located on the last pages of this packet. If you have any questions, please contact us at Forest Hills Cemetery (215) or Shalom Memorial Park (215) The setting location (lot number only) must be cast on the lower left corner of bronze memorials and engraved on the top, left, front corner of a monument's base. 6. Please note that the cemetery does not veil outside dealer memorials or lay greens on the gravcsites for an unveiling. The family must notify the dealer for unveiling set up requests Please note the following 2014 changes for both cemeteries: As of January 1, 2014, the Ledger Right and Bench Right fees of $2, respectively, payable to the cemetery by means of a contract agreement between die purchaser and cemetery, must be paid in full before application can be submitted for the installation of a ledger or bench. The contract agreement number must be submitted with all ledger and bench permit applications. Furthermore, the corresponding policy page from this packet must be signed by all parties and submitted with the permit application. In addition, a bronze memorial policy and a monument policy have been added to the packet. Lastly, a 2014 Unveiling Policy for Customers of Outside Dealers is now a requirement, for application. The Cemetery, therefore, reserves the right, without notice, to make exceptionx, suspensions, or modifications in any of the,rules and Regulations when, in its judgment, the same appear advisable. Any such temporary exceptions, suspension or modifications shall in no way be construed an affecting the general application of these policies. Complete sketched or typewritten inscriptions must be given to the cemetery and the document must include the dimensions of all letters. Be careful that names are spelled correctly and dates are verified. The cemetery assumes no liability for errors once an inscription is approved by the family, A drawing of the memorial must be made on page 4. The exact dimensions of the base of the memorial must be given on this form. In addition, you may submit blueprints with a facsimile (or typewritten copy) of the complete inscription to be placed on the memorial, including names, dates, etc. Please complete the memorial type at the top of page 4, enter the drawing on the page and write "See Attached" in the open space provided, The memorials will not be accepted and may not be placed unless it is in accordance with the design, carving, and lettering shown on this application for permit. The application consisting of pagos 1, 2, 4, 5, 6 and any additional signed policies relevant to this application must be accompanied bxacheck payable to the cemetery in the amount of the layout and inspection fee before a permit will belssued."please refer to the cover sheet of this application for the current fee amount. Installation work may not proceed until the permit is issued. Forest Hills Cemetery or Shalom Memorial Park may refuse to permit the erection of any monument, marker, stone, mausoleum, underground vault or exedra (and in case the same has been erected, to remove same without notice, and at the cost and expense of the lot owner) which is not in strict conformity with the drawings, blue prints, sketch, design, specifications, and kind of stone of any such monument, marker, headstone, mausoleum, underground vault or exedra, and the inscription thereon as appears in this form. All installations are subject to the Rules and Regulations of the Cemetery in addition to the provisions of this application. Dealer may not enter the cemetery before applying in the office for a permit. It is the responsibility of the memorial contractor to come into the cemetery office before going to the gravesite to insure that his/her work is inspected upon completion by the Superintendent or their designee. MEMORIAL DEALER WILL BE HELD RESPONSIBLE FOR ALL DAMAGES TO PLANTINGS AND THE PROPER SETTING OF THE MEMORIAL AND THE REMOVAL OF ALL DEBRIS. All damages must be reported to the office immediately. Contractor must carry at all times the proper insurance coverage evidenced by the Certificate of Insurance on file in the cemetery office. As you are purchasing your memorial from an outside dealer not; associated with Shalom Memorial Park or Forest Hills Cemetery, please.schedule your unveiling with the dealer. 2. If you would like the gravcsite cleaned of debris for your unveiling, please contact a cemetery representative by calling the. appropriate telephone number listed below or visit our office during regular business hours and submit, a Cemetery Service Request (work order) two (2) weeks before the scheduled unveiling date. Please inform the contacted representative that you have scheduled an unveiling with an outside dealer and give the vmveilinr date to ensure the completion of the work, order within the two week lime frame, 3. In so much as you are not purchasing your memorial from Forest Hills Cemetery or Shalom Memorial Park, the cemetery does not veil the memorial. However, Allowing too many ledgers in a particular garden would severely hamper die cemetery's ability to open graves in a. timely manner. Accordingly, the cemetery may have, to prohibit the placement of ledgers on certain graves. The decision l.o allow or deny the placement ol a ledger on a particular gravesite will be made at the sole discretion of the cemetery considering all laclors that il deems appropriate. Only single ledgers, no more than 2' wide and 6' long with rock pitrh aides will be allowed. As of.january I, 2014., the ledger Right fee of.f^.ooo.oo, payable to die cemetery by means of a contract. agreement between the purchaser and cemetery, must be paid in full, before application can be submitted for the installation of a ledger. On occasion, din: u.> the design or size of a ledger, it. may deem necessary to contact the outside denier for ihc removal, re-positioning1 and re-inslajlrncnl of their product for a nearby burial. In this case, the monument dealer will be notified as soon as the cemetery is aware of the need. The cemetery staff will contact die outside dealer via telephone and a written request will be given to die denier in such a way that is mutually satisfactory io both the cemetery and denier. Howe.vc.i-, allowing too many benches in a particular garden would severely hamper the cemetery's ability lo open grave* in a timely manner.
