Booz Allen Hamilton, Washington, DC July 2000 April 2011, Lead Associate Federal Aviation Administration (FAA) Mr. Davis was the System Safety Engineering lead for the FAA s SE 2020 Nextgen Facilities Comparative Safety Analysis (CSA) task. For this project, Mr. Davis assembled the scope of services and schedule to successfully implement the project under short time constraints. He led a team that prepared a draft Preliminary Hazard List (PHL) to use later in conducting the CSA of Nextgen ATC facilities. Washington Metropolitan Area Transit Authority (WMATA) Mr. Davis was the Project Manager for the WMATA Technical Assistance task to address Federal Transit Administration (FTA) and Tri-state Oversight Committee (TOC) safety recommendations. This effort was targeted at helping WMATA re-build a new system safety program that improves safety across the organization while working within the constructs of conflicting departmental goals, operational requirements, culture, performance, and public perception. Specific activities included: Assessment of department resources and expertise necessary to implement a successful safety program Review and update safety related procedures and the System Safety Program Plan Develop a system-wide hazard management system and required resource plan for implementation Develop an internal process to improve communication of safety-related information across departments Develop a process to assess the effectiveness of rules or operational changes, with respect to safety, prior to system-wide implementation Determine how hazard analysis regarding procedural changes to workers on the right-ofway can be incorporated into the rules revision process Develop an effective and sustainable internal safety audit program Page 2 of 6

3 Support implementation of changes required for an improved safety culture program. Mr. Davis also helped manage and implement safety certification activities for three WMATA vehicle programs. These programs included the 5000 Series New Vehicle Procurement, 2000/3000 Series Vehicle Overhaul, and 6000 Series New Vehicle programs. The activities Mr. Davis assisted with included conducting Preliminary Hazard Analysis (PHA), developing System Safety Program Plans (SSPP), serving on and managing the System Safety Working Groups, reviewing Failure Modes, Effect, and Criticality Analyses (FMEA), and assessing the flammability, smoke emission, and toxicity properties of materials selected for the vehicles. Additionally, Mr. Davis led three investigations for catastrophic bus fires that have occurred in WMATA owned transit buses. Mr. Davis performed a detailed root-cause analysis to determine the events that led to the origin and spread of the fires. In addition, Mr. Davis provided recommendations for corrective actions to prevent future fires from occurring. U.S. Department of Defense (DoD) - Mr. Davis assisted with the leadership effort to manage the System Safety programs for the U.S. Navy Mobile User Objective System (MUOS), the U.S. Air Force Space and Missile Systems Center (SMC) Transformational Satellite Communication System (TSAT), and the Defense Information Systems Agency (DISA), Teleport Program (TPO). Additionally, Mr. Davis led the System Safety and Operational Safety, Suitability, and Effectiveness (OSS&E) program for the Global Broadcast Service (GBS) Joint Program Office (JPO). These acquisition programs consisted of modernizing the Military Satellite Communications (MILSATCOM) network and involve a space segment including geosynchronous satellites and associated ground control equipment and facilities. Mr. Davis aided DoD in meeting system safety requirements and objectives by: Leading or participating in System Safety Working Groups Assisting with development and management of all required programmatic safety related documents including the System Safety Program Plan (SSPP), System Hazard Analysis, and Programmatic Environmental, Safety and Health Evaluation (PESHE). Providing direct safety support during all phases of the acquisition program, including system design, test, installation and integration Conducting facility Environment, Safety and Occupational Health (ESOH) and Fire/Life Safety Inspections Developing and managing the Hazard Tracking System Provide safety support to Test and Evaluation activities and procedures. U.S. Federal Aviation Administration (FAA) Mr. Davis was assigned as the lead system safety engineer to deliver a Safety Management System (SMS) of the FAA Telecommunications Infrastructure Project (FTI). This included conducting an integrated system hazard analysis. The FTI system hazard analysis focused on the identification and resolution of safety critical hazards associated with the revamp and replacement of the National Airspace System telecommunications equipment. Mr. Davis was instrumental in designing and implementing a system safety methodology and hazard documentation process that is unique to the needs of the FAA and telecommunications environment. This safety methodology utilized a Fault Tree Analysis (FTA) of the National Airspace System (NAS) air/ground and ground/ground communications system to identify system hazards, causes, and overall system risks. Mr. Davis used the results of the FTA to identify a mitigation program that successfully reduced hazard risks to acceptable levels while maintaining program budget, schedule and performance requirements. 4 National Aeronautics and Space Administration (NASA) Mr. Davis assisted NASA with their Constellation Program (CxP) safety program. CxP is NASA s future manned space program scheduled to replace NASA s current Space Transportation System (STS) commonly referred to as the Space Shuttle program. Specifically, Mr. Davis assisted with: Conducting the Functional Hazard Analysis (FHA); Completing the program level Integrated Hazard Analysis; Preparing and presenting hazard reports (HRs) based on FHA and IHA results. Mr. Davis also supported the NASA Johnson Space Center (JSC) Safety and Mission Assurance (S&MA) Directorate with developing and institutionalizing a strategic plan to assure the safe, successful completion of the Agency s mission and goals (to include completing the International Space Station (ISS), retiring the Space Shuttle by 2010 while developing a new replacement spacecraft, and returning humans to the moon in preparation for a mission to the planet Mars). As part of a foundation step, Mr. Davis led the effort to develop a best practices diagnostic tool, based on a multi-industry scan of safety programs, used to assess the S&MA directorates safety performance. Specific roles included: Supported the identification of safety improvement efforts by developing and implementing a tailored S&MA Best Practice Review process; Conducted S&MA stakeholder analysis through document reviews and stakeholder interviews; Identified key S&MA Directorate challenges; and, Developed guidance on meeting challenges through identified industry best practices and provided focus area for updating fiveyear strategic plan. Federal Emergency Management Agency (FEMA) Mr. Davis led the effort to conduct a hazard analysis related to impacts of Space Weather events (i.e. solar flares, geomagnetic storms, plasma bursts, etc.), to safety critical systems (i.e. space programs, satellite communications, utility infrastructure (i.e. power, communications, gas, etc.), transportation, healthcare, and emergency response). This analysis effort resulted in a report and presentation that identified high risk hazards and provided an implementation plan for mitigation that was highly praised by FEMA. This report is currently being circulated throughout FEMA and its stakeholders to educate decision makers on space weather hazards and take necessary preventative actions to minimize impacts from future predicted solar storms. Maryland Transit Administration (MTA) Mr. Davis supported the safety programs for the Metro, Light Rail, Bus, Paratransit, and MARC vehicles. This includes preparing system safety program plans, hazard analysis, safety data management systems, and preparing standard and emergency operating procedures. Mr. Davis assisted with development of the emergency preparedness plan for the Baltimore Central Light Rail Line documenting the management structure, roles and responsibilities, planning and preparedness, recovery and reporting requirements of MTA personnel in the event of an emergency or disaster involving the light rail line. Mr. Davis was assigned as the fire protection engineer for the MARC Commuter Train Service and developed the Fire Hazard Analysis (FHA). This analysis documents fire hazards and assesses the severity and probability of each hazard associated with MARC I, II, III and locomotive vehicles, as well as all operating environments, including tunnels and overpasses, as required by 49 CFR Part 238. The FHA methodology, developed by Mr. Davis for MARC, is currently used as an industry benchmark. U.S. Federal Transit Administration (FTA), Washington State Ferries Security Technical Assistance Project Mr. Davis assisted the Washington State Ferries System in meeting federally mandated system security requirements. Tasks included developing Maritime Transportation Security Act compliance documents, plans and procedures; find common Page 4 of 6

5 vulnerabilities and identify workable solutions; assess and improve access control methods for restricted areas; support in developing training for facility and vessel employees; develop a comprehensive System Security Program Plan; update Emergency Management Program Plan; design programs to enhance coordination between Ferry Staff, Police, U.S. Coast Guard, and local Fire Departments in response to an emergency or terrorist act; and conduct benchmarking with other ferry operators. Seattle Monorail Project (SMP) Mr. Davis was assigned as the lead fire protection engineer to conduct a Fire Safety Analysis (FHA). This analysis documents fire hazards and assesses the risks associated with the Red and Blue Train Monorail vehicles. In conducting the fire safety analysis, Mr. Davis leads several supporting tasks including conducting a fault tree analysis, emergency evacuation assessment and flammability, smoke emission and toxicity assessment of materials used in the railcars. U.S. Department of Energy, Yucca Mountain Site Characterization Project Mr. Davis supported the development of a comprehensive fire hazard analysis of the Cross Drift and Main Drift tunnels of the Exploratory Studies facility. Specifically, Mr. Davis performed facility and equipment inspections, conducted personnel interviews, performed detailed compartment fire modeling of the tunnels, performed egress analyses, and provided recommendations for the improvement of fire safety in the tunnels. Additionally, the information obtained from this analysis was used to aid in determining the appropriate design efforts required to construct emergency refuge stations and egress system for personnel in the event of a fire emergency. Hernandez Engineering, Greenbelt, MD March 1997 July 2000, Fire Protection and System Safety Engineer National Aeronautics and Space Administration (NASA) Mr. Davis supported payload and facility system safety and fire protection projects for the NASA Goddard Space Flight Center (GSFC). Mr. Davis supported the Safety and Mission Assurance Directorate, Code 302, with leading the safety programs for several Expendable Launch Vehicle (ELV) programs. Mr. Davis coordinated all system safety activities between the satellite developers (including the Johns Hopkins University Applied Physics Laboratory (APL), the Southwest Research Institute (SwRI), and the University of California, Berkeley, Space Sciences Laboratory), the launch vehicle providers (including Boeing, Orbital Sciences Corporation, and Lockheed Martin) and the launch range (Cape Canaveral Air Force Statoin and Vandenberg Air Force Base). Tasks involved identifying and assessing hazards associated with various complex subsystems throughout all phases of system development. The types of subsystems analyzed included mechanical, electrical, thermal, cryogenic, propulsion, structural, communication, and software. Mr. Davis also assembled safety data packages that summarize hazard analysis results and provide recommendations for hazard-risk elimination or control. Mr. Davis conducted spacecraft orbital debris assessments that analyzed and calculated the probability of debris impact on orbit and spacecraft reentry survivability. Mr. Davis also helped develop spacecraft processing and launch-site procedures used during transportation and operations at processing facilities and launch sites. Page 5 of 6

6 Mr. Davis performed an extensive formal investigation of a catastrophic fire that occurred in a mission-critical manufacturing facility at the Goddard Space Flight Center. Mr. Davis was assigned as the primary fire protection engineer on the mishap investigation board and performed a detailed root-cause analysis to determine the events that led to the origin and spread of the fire. In addition, Mr. Davis provided recommendations for corrective actions to prevent future fires from occurring. Mr. Davis performed extensive building safety surveys. His tasks required identifying and correcting inadequacies in facility safety, fire protection, design, and space utilization in accordance with all applicable National Fire Protection Association (NFPA), Occupational Safety and Health (OSHA), NASA, and Building Codes.
