2 SUMMARY The factory shares a six storied rented building with N.A. Garments, Sidko Apparels and Delta Ties Ltd. The factory shares ground floor, 3rd floor and roof. All the factories are owned and managed by the same group and share some facilities. The building was constructed in 1984 for industrial purpose and the production started in The Factory was surveyed for electrical safety by Woosun Energy and Construction Co., Ltd. (WEC). The purpose of the survey was to identify significant electrical safety issues and to provide recommendations for remediation based on applicable standards specified by the Accord. The scope of this initial electrical safety inspection was limited to the review and identification of major electrical safety issues. The inspection did not include identification of minor deficiencies, which will be further addressed as part of follow-up inspections. Table below summarizes the major electrical safety issues identified during the inspection. Recommendations have been provided to address each issue. An implementation schedule shall be developed by the factory to remediate each of the findings. The specific timing of improvements, including any requested extensions due to design / installation constraints, shall be submitted to the Accord for approval. Page 2 of 7

3 FINDINGS AND RECOMMENDATIONS E- 1 Cables terminating at panel are not firmly fixed. Cable terminating at the panel must be firmly fixed with glands and at gland plates, to reduce stress at the termination point. Cables, entering to the panel from the shaft E- 2 Panel base plates removed to allow cable entry. Panel base plates must be installed, at all time, and cable(s) entering panel must be firmly fixed with cable gland. Cables entering in the panel from the bottom E- 3 Wires terminating inside panel are not securely fastened. Wire terminating to devices inside panel must be connected firmly and wires approaching devices must be securely fastened to avoid unintentional contact with live parts. 4 E- 4 Barrier/separators between different phases are not installed. Install separators between different phases of MCCB. Standard separators provided by the MCCB manufacturer must be used. MCCBs are installed in the panel Within 1 Month E- 5 Multiple cables terminating to MCCB in panel. Multiple cable connecting at a MCCB terminal must be disconnected. Existing multiple circuits may be distributed through bus bars. Cables connected with the MCCB inside the panel E- 6 Smaller size cables connected to higher rating control devices. MCCBs controlling circuits connected through smaller size cables must be checked and coordinated as per the connected load. MCCB and re-wireable fuse are installed inside the panel Page 4 of 7

5 E- 7 Three phase MCCB connected to control two phases or less. Check and redesign the requirements to control the circuits. If three phase control is not required, the control devices suitable for the purpose may be selected (replaced). MCCBs are installed inside the distribution board Within 1 Month E- 8 Cables looped at the MCCB to extend supply to other SDBs. Extending main supply to SDBs from the MCCB must be avoided. The main panel may be redesigned with busbar systems to distribute branches to other sub-panels. Cables are terminated in the MCCBs inside the panel E- 9 Panel not provided with earth distribution bar (strip). 6 E- 10 GENERATOR ROOM Cables terminating to generator output terminal box are laid on floor. Cable terminating at Generator output terminal box must be supported on riser and protected. Existing cables laid on floor may be installed in cable trench or on trays. Diesel generators are placed at ground floor of the building E- 11 CABLE & CABLE SUPPORTS Excessive lint deposit in cable duct. Clean the ducts and cover tightly with non-combustible materials. Cables passing through the duct in the production floor Within 1 Month E- 12 CABLE & CABLE SUPPORTS Wiring ducts made from wooded planks and boards are used for supporting wiring. Existing wooden ducts supporting wiring may be replaced with non-combustible ducts, with ample strength and rigidity, supported at regular intervals.
