Task For construction products according to EN 13986 woodbased panels for use in construction characteristics, evaluation of conformity and marking a system of assessment and verification of constancy of performance according to Regulation 305/2011/EU of the European Parliament and of the Council of 9 March 2011 (the Construction Products Regulation or CPR) shall be applied. The Regulation 305/2011/EU differentiates several systems of assessment and verification of constancy of performance. According to system 1 a notified body shall carry out an initial type testing, an initial inspection of the manufacturing plant and of factory production control as well as a continuous surveillance, assessment and evaluation of factory production control. According to system 2+ a notified body shall carry out an initial inspection of the manufacturing plant and of factory production control as well as a continuous surveillance, assessment and evaluation of factory production control. According to system 4 there are no tasks for the notified body. In individual cases a continuous surveillance, assessment and evaluation of factory production control was ordered by manufacturers on voluntary basis. The continuous surveillance of the factory production control shall cover all the performance characteristics relevant to the intended application (see EN 13986, Tables 1 to 7).

Execution The inspection of the factory and of the factory production control took place on 16 February 2016 on the manufacturer's production site: LV1016 Riga As representative of the manufacturer was present: Ms. Shanna Sirokvasha Laboratory Manager The inspection was carried out by the WKI employee Dipl.Ing. (FH) Oliver Meistring. The manufacturer made available the latest minutes of the factory production control. 3. Results The manufacturer runs a quality management system. The manufacturer has introduced relevant quality control systems according to technical specifications. The manufacturer possesses all relevant standards, process descriptions and working instructions which are accessible for the personnel. The manufacturer has specified the raw materials and checks their ability for the production. The manufacturer has adequate production facilities to produce the supervised products. The manufacturer records relevant production parameters, which allow tracing of production errors. Samples are taken according to statistical aspects and in required quantity. Tests are performed within the specified period of time after production. The manufacturer proves that tests become carried out separately for each product type and production line. Test results of the factory production control can be evaluated separately for each product type and production line. The manufacturer calculates the average value and 5% respectively 95% quantile values of the last 30 results of the factory production control tests. The testing equipment is complete and regularly maintained and calibrated. Reagents and additives are complete, checked for their durability and stored safely. The laboratory personnel are trained and have relevant experience for performing quality control tests. A procedure for handling nonconform products is implemented and followed by responsible personnel. The manufacturer marks the products correctly and keeps necessary product documentation. The manufacturer has issued a declaration of performance for each CE marked product.
Page 5 to 5 Report No. QA20160935 Deviation: The fire reaction class for 8 mm OSB shall be corrected in the DoP. Furthermore the minimum density requirements shall be respected in the product specification. Additional information: A training plan for new personnel shall be issued. In present case V. Galaks joined a new postition (OSB supervisor) without having a training plan shall be issued. 4. Assessment The factory production control for the products mentioned in section 1 fulfils the requirements of EN 326 2:2000 "woodbased panels sampling, cutting and inspection: quality control in the factory". According to the indented use of the products the performance characteristics specified in EN 13986 "woodbased panels for use in construction characteristics, evaluation of conformity and marking" are under control.
