1 SITUATION 1.1 Introduction The Department of Biomedical Engineering was established in 2002. The Department academic staff has varied background in engineering, life sciences and medicine and many of whom has joint appointments with the School of Medicine, Faculty of Science, Faculty of Engineering, Mechanobiology Institute and A*STAR Research Institutes. This signifies the multidisciplinary and integrative approach in bioengineering research and education. The core research expertise available in the department includes Biomaterials, Biomechanics, Bionanotechnoloy, Biosignal Processing, Biosensors, Biomicrofluidics, Computational Methods with application in areas of Tissue Engineering, Therapeutic Delivery Systems, Biomedical Imaging and Instruments. The Department is under the jurisdiction of: Department Head: Prof James Goh Cho Hong Department of Biomedical Engineering National University of Singapore Tel: (65)-65165259 Fax: (65)-68723069 Email: biegohj@nus.edu.sg 1.2 Area of Operations 1.2.1 Location of Installation and its main access road Department of Biomedical Engineering Office is located at E4-04-08, 4 Engineering Drive 3, and Singapore (117583). The main access road is Kent Ridge Crescent followed by Engineering Drive 3. (See Annex for Location Plan) The laboratories are mainly located at E3 level 5 and E3A level 7. (Please see annex A) 1.2.2 General Description of Site The E3 building is adjacent to T-labs, which consists mainly of research labs, while E3A building is situated next to the EA building, which consists of a mix of research laboratories and administrative offices. There are no hostels near these two buildings 1.2.3 Layout of Installation Please refer to annex C1 and C2 for the internal building layout plan.
1.2.4 Location, storage and quantities of hazardous products, chemicals and gases Please see annex B for Inventory list of Hazardous Substances. Please see annex C1 and C2 for location of the Hazardous Substances. 2 AIM The aim of this emergency response plan is to detail the various operational actions that need to be undertaken to respond in case of emergency situations such fire, chemical leaks/spillages, biological emergencies etc within the installation so to prevent and reduce injury to personnel and minimize property damage and loss. 3 EMERGENCY RESPONSE 3.1 FIRE Raise the alarm by activating the nearest fire alarm break glass call point. Attempt to extinguish any incipient fire without taking personal risk (refer to Appendix A) provided subject has been trained in the proper use of a fire extinguisher and is confident in his/her ability to cope with the hazards of fighting a fire. There are several different fire extinguishers available in every area of a building on campus. Occupants are expected to know the location of the fire equipment by referring to the evacuation floor plan found on every floor. The acronym PASS can be used to operate a fire extinguisher as follows: P A S S Pull the pin to allow user to squeeze the lever in order to discharge the extinguisher Aim the hose at the base of the fire. Stand a safe distance away from the fire. Squeeze the handle fully to discharge the fire extinguisher agent Sweep the hose from side to side Notify Campus Security (Tel No: 6874 1616) and Singapore Civil Defence Force (Tel No: 995) of the activation of fire alarm and state the following: i) Location of the fire ii) Nature of fire, if known iii) Injury to personnel, if known iv) Informant s particulars and contact number The caller shall not replace the telephone set until the address has been repeated by the operator at the SCDF Control Room.
Stop all work, shut down electrical equipment / heating appliances/ machinery Alert people that there is a fire in the building Rescue any person in immediate danger to safety as long as it is safe to do so If clothing of person is on fire, adopt the stop, drop & roll method or drench with water using the water shower if available Close all doors to rooms, escape staircase and corridors to contain the smoke and/or fire. Proceed to nearest exit and evacuate safely Do not use life in case of fire Proceed to the designated Assembly Point Do not re-enter building until it is safe to do so See annex for list of fire wardens 3.2 CHEMICAL SPILL Minor spills (not for concentrated acids and >500ml spill) in the labs: Notify lab supervisor and report the location of the spill and type of chemical involved Follow lab procedures for eyewash rinse or shower Evacuate persons in the immediate area if necessary Clean the spill ONLY by those with suitable training and equipment (SDS sheets and spill kits are available in each lab, use these documents for information and correct procedure) Wear personal protective equipment (goggles, gloves, respirators, shoe covers) When toxic chemicals come into contact with skin, remove any clothing and immediately flush the affected area with clear water for at least 15 minutes in the nearest Emergency Eyewash station location. (see Appendix C for location of Emergency Eyewash station) If a chemical splashes in the eye, immediately flush it with clear water for 15 minutes in the nearest eye wash fountain Use the appropriate kit to neutralize and absorb chemical spill Collect waste & seal in proper container and label it clearly Major spills in the labs: Notify Campus Security (Tel No: 6874 1616), Singapore Civil Defence Force (Tel No: 995) & lab supervisor of the chemical spill and state the following: i) Location of the incident ii) Nature of material spilled, if known
iii) Injury to personnel, if known iv) Informant s particulars and contact number Assist any injured persons. Isolate contaminated persons Avoid contamination or chemical exposure Close doors or control access to spill site Communicate critical spill information to first responders Follow evacuation instructions External hazardous material release (Toxic Cloud): Listen for instructions on public address system if available Ensure windows and doors remain closed Inform occupants to stay indoors until hazard is declared over Ensure maintenance of the safety and comfort of occupants Prior to responding to any chemical spills, lab users should be familiar with the hazards involved. DO NOT attempt to handle a spill without understanding the risk involved. If lab users are unfamiliar with handling chemical spillage: Leave the area and DO NOT attempt to clean up the spillage Warn others of the spill Notify lab supervisor & Campus Security Wait for further assistance. Decontamination 3.3 BIOHAZARD and Clean-up MATERIAL procedures SPILL Notify lab supervisor and report the location of the spill and material involved For spill outside biosafety cabinet, evacuate area and allow potential aerosols to settle for at least 10 minutes. Post signage indicating spill cleanup in progress, if necessary. For spill inside Biosafety cabinet, leave the cabinet TURNED ON Put on the proper Personal Protective Equipment (such as long-sleeved gowns, safety glasses or goggles, rubber gloves, and respirator, as necessary) Use the appropriate decontamination solution to treat the spill area. Pour it gently, directing its flow into the spill area, so that the area of spill in not increased. Cover the area with absorbent and allow sufficient contact time to absorb the spill (see decontamination solution recommendations for contact time)
Transfer all materials from the spill area into an autoclavable pan and autoclave all the clean-up materials. Alternatively, place in an appropriate biohazard waste container for collection by a contract waste collector. Wash and mop the spill area with decontamination solution for a second time Remove gown, rubber gloves, safety glasses or goggles, and respirator (if necessary) and be sure to use appropriate decontamination solution prior to autoclaving for disposal or before disposal in the biohazard waste container Wash hands thoroughly after the clean-up procedures Prior to responding to any biohazard spillage, lab users must thoroughly understand the hazards involved. DO NOT handle any spillage without understanding the risk involved, without the appropriate personal protective equipment, or if you simply do not feel comfortable doing so. If lab users are unfamiliar with handling biohazard spillage: Leave the area and DO NOT attempt to clean up the spillage Warn others of the spill Notify lab supervisor & Campus Security Wait for further assistance. Inert or non-hazardous gas leakage 3.4 GAS LEAKAGE Open all windows and doors to ventilate the area Use a gas leak detector or soapy water or other suitable solution to detect location of leakage Secure leakage by tightening, reconditioning or replacing the valve, regulators or tubing connections to equipment If leak persists, move cylinder to a ventilated area, report to lab supervisor and consult supplier for more instructions Cryogenic gas leak Open all windows and doors to ventilate the area Shut off source of leakage if it s safe to do so If leak persists, evacuate everyone out of the laboratory to avoid asphyxiation Notify Campus Security (Tel: 68741616) & lab supervisor and report the location of the leakage and type of gas involved. Consult supplier for instructions Toxic Chemical / Flammable gas leak
Open all windows and doors to ventilate area Shut off source of leakage if it s safe to do so If leak persists, evacuate everyone out of the laboratory and raise the building fire alarm by activating the nearest fire alarm break glass call point. Notify Campus Security (Tel: 68741616), Singapore Civil Defence Force (Tel No: 995) & lab supervisor and report the location of the leakage and type of gas involved. Do not light any heat or electrical sources If the leak resulted to a fire, please follow the fire emergency response in section 3.1. 3.5 RADIATION LEAK Immediately notify lab supervisor & Campus Security (Tel: 68741616) of the incident and report the location of the incident. Warn others within the vicinity of the incident. Regulate entry to the scene of the incident If evacuation is necessary, switch off apparatus producing ionizing radiation Protective apron should be donned when switching off apparatus Safety officers to identify and immediately isolate all persons who might have received high exposures from the radiation All necessary medical surveillance & personnel monitoring should be provided. Exposures of persons involved should be recorded All working areas exposed to the radiation should be thoroughly monitored before work is allowed to recommence. All affected equipment should be checked and cleared safe to be used Contact OSHE at 65161084 to coordinate waste pick-up 4 COORDINATING INSTRUCTIONS Key Personnel Emergency Contact Numbers The key personnel to be notified must be contactable during office hour and after office hour (Contact No. must include either home, mobile contact number) (See Annex D for Key Personnel Emergency Contact Numbers) 5 SAFETY & EMERGENCY RESPONSE EQUIPMENT All lab users should know the procedures for responding to emergencies in their respective work area. The emergency response plan should be periodically reviewed and always be familiar of the locations of the safety & emergency equipment in the labs such as: First aid kits
