Observed front area refrigerator unit not holding 41 degrees F as required. Unit temperature holding between degrees F. Unit needs to be repaired at this time. Observed floor drains, walls, PVC piping and the entire flooring throughout the entire rear area of the food facility is extremely dirty with trash, dirt, soil, static dust, old food debris/splash and grease residue and in need of cleaning at this time. Observed flooring of walk-in cooler/freezer of the food facility is extremely dirty with trash and old food debris and in need of cleaning. The person in charge does not have adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Food employee observed in cooking/prep area wearing bracelet on arm at the time of inspection. Food employees observed in cooking/prep area not wearing proper hair restraint such as nets or hat. Observed inside of ice machine reflector is heavily soiled with a black bacteria/moldlike substance and in need of immediate cleaning. Observed inside of juice nozzle located in front area of store, a food contact surface, to have old caked-on food residue and was not clean to sight and touch. Observed light shield in walk-in cooler is cracked/chipped and in need of repair or replacement. Observed a large number of fly (insect) activity all around the entire store area at the time of inspection. Facility advised to seek or start their own pest control program.
3 Observed bottom shelving of refrigerator, non-food contact surfaces, not cleaned at a frequency to preclude large accumulation of dirt and soil. Manager's comments: All issues have been addressed. June 21, 2018 Echo Frezco, Shrewsbury Township Thermometers for ensuring proper temperatures of food are not available or readily accessible inside refrigerator units. Water, as observed at three-compartment sink, is not producing enough water pressure to fill sinks. Observed hose from utility sink be used to fill three-compartment sink and other miscellaneous usages. Unused equipment (oven) stored in kitchen area and should be removed from food facility. Unit needs to be cleaned and storing dished and miscellaneous items inside of unit. Observed oven, in kitchen area, with a large accumulation of dirt and old caked-on food residue on non-food contact surfaces and in need of cleaning. The person in charge does not have adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Observed can of whipped cream being stored inside of ice bin used for customer water/refreshment drinks at the time of inspection. Observed several food ingredient storage containers in the kitchen area is not labeled with the common name of the food. Observed boxes and wrapped food stored directly on the floor in walk-in freezer area, rather than 6 inches off the floor as required. Can opener, a food contact surface, was observed to have old food residue and was not clean to sight and touch. Dirty dishes (glass bowls, cast iron dishes and chip bowls), food contact surfaces, were observed to have old food residue and were not clean to sight and touch. Observed working containers of sanitizer located in wait staff area, used for storing cleaner taken from bulk supplies, were not marked with the common name of the chemical. Observed a case of soda being stored on the floor in a box that has been allowed to be wet by leaking reach-in cooler water in the back room. Observed ¼ bag of cold milk in cold holding unit near coffee maker being held at 49 degrees F rather than 45 degrees F or below as required. Disposed of milk valued at $3.50. Observed an accumulation of black and white mold-like residue and dirt on the floor under both reach-in coolers in back room. Observed an accumulation of trash on the floor in the walk-in cooler. Observed an accumulation of trash and dirt on the floor behind the ice machine in the back room. Lights are not shielded or shatter proof over food in the R and K display case. Lights are not shielded or shatter proof over food on the sales floor. Lights are not shielded or shatter proof over food in the back room. Observed two half-gallon containers of Tru Moo 1% Chocolate Milk with an expiration date of June 19 in retail case. Disposed of milk valued at $7. Observed two one-gallon containers of Swiss 1% Lowfat Milk with an expiration date of June 18 in retail case. Disposed of milk valued at $7.25. Observed an accumulation of black mold-like residue on the fan guards in the walk-in cooler. Observed an accumulation of old food debris on top of and inside the oven on the make line. Observed an accumulation of old food debris on the door of the reach-in freezer on the make line. Observed an accumulation of black mold-like residue on the gaskets of the walk-in cooler door. Plumbing system not maintained in good repair. Observed water leaking from the nozzle of the mop sink in the back room. Plumbing system not maintained in good repair. Observed water leaking from the nozzle of the two-bay sink on the make line. Observed the floor tiling in several areas of the back room and sales floor to be broken or missing, and is not a smooth or easily cleanable surface. Observed accumulation of insect webbing on the shelving behind the soda syrup boxes in the back room. Observed a container of floor cleaner being stored on the drying rack side of the threebay sink.
5 Observed several containers of soap being stored on a shelf over the three-bay sink in the back room. Observed several containers of spray paint, a poisonous or toxic material other than those required for the operation and maintenance of a food facility, being stored on a shelf over the three-bay sink in the back room. Food employees observed on make line not wearing proper hair restraints, such as nets, hats or beard covers. Two-compartment sink being used for a food facility manually cleaning multi use equipment and utensils in a continual flow. Food facility does not have available chlorine sanitizer test strips or test kit to determine appropriate sanitizer concentration. Observed several cases of single-service, single-use articles stored directly on the sales floor and not 6 inches above the floor as required. Food facility does not provide lockers or storage for food employee personal clothing and possessions. Observed a purple binder being stored on top of a box of soda syrup in the back room. The person in charge does not have adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Sponges observed in the two-bay sink on the make line are being used to clean food contact surfaces. Observed an accumulation of water on the floor under and behind the reach-in cooler in the back room. Observed a torn rubber door gasket on the walk-in cooling unit. Observed an accumulation of old food debris in the soda nozzles on the customer selfserve soda machine. Observed an accumulation of old food debris on the inside of the microwave near the coffee maker. The food facility does not have the original certificate for the certified food employee posted in public view. Paper towel dispenser empty at the hand wash sink near the coffee maker. 6 Observed cardboard cases of canned consumables displayed directly on the floor in register area, rather than 6 inches off of the floor as required. The person in charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Rear product storage area of the facility is extremely dirty, dusty with various products directly on the floor. This is a repeat violation also found during inspection on June 13. Observed shampoos, laundry detergent, insect repellent, alcohol wipes, fabric softener and pesticides stored above and in direct contact with foods such as baby food, Pop- Tarts, bottles of water/teas, etc. on the clearance items retail shelving. This is a repeat violation also found during inspections on June 2 and 13. Manager's comments: A manager was not available. June 27, 2018 Broadway Mini Mart, Hanover Person in charge does not have adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Observed boxes/containers of cream and miscellaneous food containers being stored inside the cabinet underneath PVC piping of hand wash sink. Prepackaged food (sandwiches) is not labeled with the name of product, ingredient statement, net weight, distributed by statement and/or nutritional facts. Prepackaged sandwiches (egg/chicken/hoagies) are not labeled to clearly indicate any Big 8 allergen ingredients and/or the allergen warning statement. Ceiling tile in retail area is extremely soiled and needs to be replaced. Food facility inspection indicates evidence of insect (ants) activity on top of food counters and cabinet areas, but facility does their own pest control at this time. Time in lieu of temperature not being used as a control for potentially hazardous foods (sandwiches) at the time of inspection. Thermometers for ensuring proper temperatures of food are not available or readily accessible in deli sandwich cold hold unit. Food facility hot water heater is not producing enough hot water to supply hand sink, bathroom sink and three-compartment sinks at the time of this inspection. Observed commercially processed refrigerated, ready to eat (sandwiches), time/temperature control for safety food, held more than 24 hours, is not being marked with the date it was opened.
7 Observed no hot water being produced through numerous sinks within food facility due to hot water heater issues. Facility is manually turning off hot water due to leak. Observed floor drain located in walk-in cooler area of the food facility is extremely dirty, dusty and in need of cleaning. Manager's comments: Waiting for reinspection. Speedway No , Fairview Township Person in charge does not have adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Observed several containers of eggs being stored in direct contact with several containers of butter on a shelf in the walk-in coolers. Observed an accumulation of old food debris and spillage on the floor in the walk-in cooler. Observed an accumulation of old food debris in all the hard to reach areas of the food facility. Food facility does not provide lockers or storage for food employees personal clothing and possessions. Observed a dirty apron and coat being hung on the leg of one of food storage rack in the doughnut prep area, and two purses being stored on a prep table in the doughnut prep area. Observed an accumulation of ice on the ceiling and floor in the reach-in freezer in the walk-in cooler, which is in need of repair. Observed an accumulation of insect webbing on the floor and shelving in the soda area of the retail sales floor. Observed a case of Penzoil oil being stored on top of a case of Gehli s Cheese and Chili Sauce on the retail sales floor. Observed an accumulation of old food debris around and on all liquid food dispensing nozzles. Observed an accumulation of red mold-like residue on both deflector plate ice shoots on the customer self-serve soda machine. Observed an accumulation of old food debris on the bottom shelving of all reach-in coolers and freezers. Observed an accumulation of old food debris on all moveable carts. Observed an accumulation of static dust on top of the pizza warmer on the retail sales floor and in the hood system over single-use items in the doughnut prep area.
8 Observed several clean utensils being stored in old food debris in two pull containers in the doughnut prep area. Mops are not being hung to air dry. Manager's comments: A manager was not available. June 29, 2018 Clarion Hotel, Fairview Township Person in charge does not have adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. The person in charge did not demonstrate adequate knowledge of the Pennsylvania Food Code as evidenced by incorrect responses to food safety questions. Food employee observed in ware washing area wearing a watch on left arm. Food employee observed sanitizing washed equipment and utensils without a separate water rinse between. A food employee certification is displayed, but the individual is no longer employed by the retail facility. Food facility is not removing dead or trapped pests from control devices frequently, as evidenced by observing 10 dead or old roach-like insects in pest control device located on a shelf under the coffee makers in the back-prep area. Food employee is not using available sanitizer test strips or test kit to determine sanitizer concentration. Food employee observed donning single use gloves without a prior hand wash. The quarternary ammonia concentration in the sanitizing solution of the three-bay ware wash sink was 0 parts per million (ppm) rather than ppm as stated on the manufacturers use directions. Observed one drain stopper not available to hold water for manual ware washing. Plumbing system not maintained in good repair. Observed at time of inspection cold water faucet on three-bay sink not operating properly. Food facility does not provide lockers or storage for food employees personal clothing and possessions. Observed a purse, coat and dirty aprons hanging on the legs of one shelf and on another shelf all next to or over food in the prep area of the kitchen. Manager's comments: Person who answered the phone said the restaurant has been closed.
