3 Introduction In the event of an emergency or a threat thereof at Plaza Tower One, it is essential that staff, tenants and first responders work in concert to prevent or minimize personal injury and property damage. This handbook has been developed to assist the tenants to prepare and respond to a spectrum of emergency situations. These instructions require that each tenant orient its employees and develop implementing instructions as needed. EMERGENCY TELEPHONE NUMBERS For all emergencies, dial 911. After any call to 911, please call the Management Office (303) immediately to report the emergency. Other important phone numbers are listed below. 4 General Information Communications: In an emergency, instructions will be issued over the public address system. If available, phones, cell phones, fax and will be utilized for other than immediate emergencies. If all else fails, management personnel will notify each tenant in person. Tenant newsletters, s and orientations may also be used to update tenants on emergency planning as needed. In the case of an after hour emergency, management or security staff will call tenant contacts to advise them of the situation and whatever action might be necessary. Command and Control: Until first responders arrive, the management or security staff will direct and coordinate building staff and tenant actions in an emergency. Fire personnel normally assume command of an incident and direct activities as necessary. In some cases the incident commander could be from the police department. For incidents other than fire, employers, or the civil authorities can order evacuation. Property Management or fire personnel can only recommend evacuation. In case of employer directed evacuation, management office should be advised at (303) Basic Responsibilities: Tenants are expected to review and comply with the emergency procedures outlined herein as well as cooperate in rehearsals such as fire drills. Tenants are encouraged to report ANY suspicious activity or packages in and around the building, immediately to 911 and the management office at (303) Tenants are requested to report personnel terminations to management so that access to the building can be denied and management staff can be alerted. Tenants are reminded that the lease requires personal property insurance must be in effect to cover loss or damage to property located in their spaces. Each occupant in this building should know location of stairwells, building exits and fire alarm pull stations. 3

5 It is recommended that tenants consider storing duplicates of important documents/records off-site and/or electronically on an off-site server. After hours and during heightened levels of security tenants should not permit persons unknown to them to piggy-back on their access cards to enter the building or use the elevators. Coordinate with the Building Management Office the location of tenant assembly area outside building in event of evacuation. Emergency Supplies- It is recommended that each tenant maintain the following for use in an emergency: Basic first aid kit Flashlights(s) and spare batteries Portable radio and batteries Cell phone batteries, charged Company sign/placards to be used at assembly area upon evacuations Current roster of employees List of fire warden personnel List of mobility impaired personnel and assigned buddies Water Food Blankets Pillows 4

6 Medical Emergency In the event that an accident or illness of an employee or visitor takes place in your office area: Call 911 (be sure to request paramedics) Give dispatcher this information: Building name and location Plaza Tower One 6400 S. Fiddler s Green Circle Greenwood Village, CO Floor of building, tenant name, suite number, and exact location of emergency on floor i.e. southwest corner of the suite. Any details available of accident or illness. Do not hang up until the dispatcher hangs up. Do not move injured or ill person. Try to keep the victim comfortable and calm. If needed, AED devices are located in the east stairwell on floors 4, 11, 13, and 19, as well as in the Building Security Office on the 1 st floor, and the Building Management Office in suite 230. The AED Demonstration video for the Lifepak CR Plus can also be viewed at Call the Building Management Office at (303) to report the situation. Building Management will hold an elevator for the responding emergency crew. Inform the Building Management Office that you have already called 911. Have someone meet the emergency responders at the elevators on your floor to lead them to the victim. Paramedics will administer necessary medical assistance. 5

7 Severe Weather Emergencies Weather warnings are provided by the National Weather Service (NWS). Management will monitor the emergency weather radio for severe weather watches or warnings. A watch means that conditions are favorable for the severe weather to develop. A warning means that the severe weather has developed/ been sighted and is moving in our general direction. Tenants are encouraged to maintain a portable radio and batteries to monitor in the event of a watch or warning. 1. Earthquake: While unlikely in this part of the country an earthquake could hit with no warning. If the building begins to shake move away from the windows to the core of the building (lobby stairwell, restrooms, interior corridor). Sit down and cover your head. If caught in an exterior office get under a desk and as far away from windows as possible. When the tremors stop, report any damage to the Building Management Office. 2. Floods: Regardless if water accumulates from leaks or rain, it is unlikely that water will accumulate above the ground floor although if there are broken windows or broken water lines on upper floors, there could be water damage above the first floor and basement/ garage. Notify the Building Management Office at (303) of any water accumulation or damage. Building Management will take steps to minimize further damage, repair the breach and arrange for a recovery contractor to remove water and dry out the area. If there is flooding do not try to walk out of the building, but listen for instructions over the PA system. 3. Lightning: In any kind of lightening storm, you are safer inside the building then outside. Remain inside until the storm passes. 4. Snow: Heavy snowfalls can make driving difficult and dangerous. Employees should contact their company to determine if their office will open that day. If snow begins to fall during the workday, each tenant, based on the forecast will make a determination to close its office or not. If a decision is made not to open or to close early, please notify the Building Management Office. 6. Tornado: Some advance warning of a tornado is usually provided. However tempting, do not stand at the window to watch the funnel cloud. When you receive a TORNADO WARNING: Get away from the perimeter of the building and exterior glass. Close drapes, blinds, etc., if time permits. Leave your office if located on the building perimeter and close the door. 6

8 Go to the center of the building- the main corridor of your floor, the elevator lobby area, the stairwells, or the restrooms. Sit down in the corridor and protect yourself by putting your head as close to your lap as possible, or kneel protecting your head. Do not go to the first floor lobby, or outside the building. Keep your radio or TV tuned to a local station for information. Do not use your cellphone to get information or advice. This only ties up circuits. Updated information will be passed onto you via the building public address system as often as possible. If you are trapped in an outside office, seek protection under a desk. Keep calm. 7

9 Fire Emergency While fire is not an uncommon emergency, in modern office buildings like Plaza Tower One, it is a rare event. This building has been constructed to code with fire retardant materials. Heat and smoke sensors will detect fire or smoke and activate alarms for building occupants and the South Metro Fire Rescue. If the sensors or alarms fail, the building is equipped with manually activated pull stations on each floor. Also, on each floor, there are hand operated fire extinguishers as well as fire standpipes. The building is equipped with an emergency back-up generator which starts automatically upon loss of electrical service and will provide power for fire/life safety systems, emergency lighting, one elevator (on each the high rise and low rise elevator banks), fans and the fire pump. But the best defenses against a fire emergency are a fire prevention program and individual knowledge of what to do in a fire emergency. WHAT TO DO IF YOU DISCOVER A FIRE OR SMOKE IN THE BUILDING: A. RESCUE- Begin evacuation procedures. Go to the nearest fire alarm pull station and activate the alarm. This notifies the occupants so that evacuation can be started. B. CONFINE- Close the door to the room of origin, if possible. This will act to contain the fire and smoke to that one room for a period of time. C. ALERT- 1. Call the Fire Department at 911 (remember to dial any special numbers, such as 9, needed to get an outside line first) from a safe location and give them the following information: a. Nature of the emergency; fire, medical, etc. b. Your name. c. Complete street address (be prepared to spell the street name) d. Building name. e. Floor number and room number. f. Closest street intersection. 2. Call the Building Management office at (303) from a safe location. Give them the following information: a. Your name. b. Nature of the emergency; fire, medical, etc. c. Floor number d. Room or Suite number. e. Telephone number you are calling from. 8

10 D. FIGHT- If the fire is small and contained to one object (such as a trash can) locate the fire extinguisher appropriate for fighting that type of fire and use it. Never attempt to fight a fire unless you (1) know how to operate the fire extinguishing equipment, (2) have the appropriate type of extinguisher, (3) the fire is small, (4) you have access to an exit if you fail to put out the fire. Fire Alarm System: The following is a brief explanation of the fire alarm system for Plaza Tower One: A. The alarm system is activated by the: 1. Manual pull station 2. Smoke Detectors 3. Heat Detectors 4. Sprinkler activation B. When activated: 1. An audible signal will sound and strobes will flash. 2. Proceed to nearest stairwell to evacuate. If mobility impaired, proceed to the elevator lobby with your buddy, call elevator, and if it arrives, proceed down to evacuate. If the elevator does not come, call authorities to report your location. 3. Heating and air conditioning on the fire floor is automatically turned off. The floors above and below remain on to pressurize (smoke purge system). 4. Stairwell pressurization fans turn on automatically. 5. All electronic locks will automatically disengage. 6. Fire department will be automatically notified through a 24/7 fire alarm monitoring service. Fire Prevention & Tenant Awareness: The following rules/actions are considered the minimum to prevent fire from occurring at Plaza Tower One: There will be no smoking of any kind inside the building at any time. Outside the building, do not discard smoking material into the flower beds, planters or trash cans. Smoking is only allowed in the designated smoking area. No storage of any items in hallways or stairwells. Storage in suites must not be any higher than 18 from ceiling. 9

11 Insure all trash is properly disposed of, as this can be used as fuel by a fire. Do not obstruct sensors or sprinkler heads. Never leave cooking unattended, and turn off electrical appliances when not in use e.g. coffee pots, toaster ovens. Investigate electrical equipment that is not working properly or smells strange. Unusual odors from appliances or cords can be a first sign of a fire. Portable electronic or gas heaters are not permitted in the building. Due to the high amount of fires caused each year by poorly maintained and non-u.l. rated space heaters, these items are considered a lease violation. Live Christmas trees are not permitted in the building. Candles are not allowed in the building. Use only approved UAL rated circuit breaker power strips and the absolute minimum necessary. Extension cords are prohibited. There will be no tenant storage in telephone and electrical closest/rooms. Do not obstruct emergency exits in any way. Adhesives, cleaning fluids and any other flammable liquids must be stored in approved containers. Turn power off on office equipment at the end of the work day. Insure adequate ventilation for electrical office equipment e.g. copier. Stairwell doors on all floors and lobby doors on single tenant floors must be kept closed at all times. The stairwells are pressurized in the event of any fire alarm activation. If the door is propped open, the pressurization system becomes ineffective. Report any unsafe conditions observed to the Management Office including exit signs not functioning, doors that do not close or latch properly, or stairwell doors that do not open or close properly. 10

12 FIRE WARDEN DUTIES Each Tenant will assign Fire Wardens to assist First Responders. The number of Wardens and Assistants is based on the number of employees at your location. For Offices of less than 10 people one Fire Warden and One Alternate. For Offices of More than 10 people Two Fire Wardens and One Alternate. For Offices that occupy more than ½ of one floor Two Fire Wardens and Two Alternates. Buddies are for mobility impaired individuals including pregnant and temporarily impaired individuals. 2 Buddies per impaired individual. Duties for each of the above listed designations are as follows: Fire Warden: Must attend training sessions provided by Building Management. Become familiar with the building and Tenant suite and plan quickest evacuation route. Please reference page 15 for property site plan. Know the details of the emergency manual, educate all employees on the plan, and explain what is expected of them in an evacuation. Know the location of alarm systems and/or fire protection equipment in the building, on the floor, and in the Tenant suite. Relocate staff to a designated assembly area at least 300 ft. away from the building. Inspect the Tenant suite to ensure everyone has evacuated and all doors are closed. Also, inspect restrooms and other common areas. Notify First Responders of the location of any mobility impaired persons and/or any employees unaccounted for. Account for all employees who vacated the building to the designated assembly area and update them on the status of the emergency. Try to keep them from leaving the site. Maintain contact with Building Management to know when it is safe to re-enter the building. Exhibit leadership while providing for the safety of employees until the emergency has ended. Ensure restrooms are clear of all personnel during a fire or other emergency. In the event of an evacuation, an exit monitor opens the door to the stairs checking for smoke. Alternate Fire Warden: Must attend training sessions provided by Building Management. Assumes duties of Fire Warden in his/her absence. Assist Fire Warden with his/her duties listed above. 11

13 Buddies: Assign two buddies to each mobility impaired person. Mobile impairment can be either permanent or temporary: o permanent disability o temporary disability (e.g. pregnant, broken leg or foot) o mentally challenged o asthmatics o any person who cannot relocate safely without assistance Buddies should be selected based on their duties (which are primarily in the building), health and strength. Select several alternate buddies in the event a buddy is unavailable. In an emergency, the two buddies must relocate the impaired individual to safety. If possible please bring them to the nearest elevator lobby safe haven. If the elevator lobby is not available for any reason, proceed to the stairwell. If the individual is unable to negotiate the stairs, one buddy must remain with the individual while the other person notifies first responders of the impaired person s location. Never leave a mobility impaired person alone. Tenant Representative: Provide an up-to-date list of the mobility impaired personnel and their location in the building to Building Management. Maintain a current employee list. In an emergency this list can be used to verify all employees have exited the building and are accounted for. Maintain a list of emergency contacts with name, home phone number, cell phone number, and any alternate phone numbers if applicable. Notify Building Management of any changes in Fire Warden personnel, mobility impaired personnel, and emergency contacts. Ensure all new employees receive a copy of the Emergency Procedures Manual and are scheduled for training of occupant responsibilities. FIRE DRILLS: South Metro Fire Rescue recommends that at least one drill be conducted each year. The drill will be scheduled, but specific times may or may not be announced. Tenants and management will attempt to work out any conflicts in advance. The drill will be timed and evaluated. Whenever possible, drills will be conducted three floors at a time to minimize impact on each tenant. With full cooperation, drills should not require more than minutes. Fire Drills are mandatory and could help save your life in the event of an actual emergency. 12

14 ANNOUNCEMENTS: The following are sample announcements you may hear on the public address system: May we have your attention please? This is not a drill. An alarm has been activated on the (floor number) floor. The Fire Department has been dispatched. For your safety, elevators will not operate until the alarm is cleared. Evacuate down the stairwells upon reaching the ground floor and exit the building to your assembly areas. This is not a drill. Please stand by for further instructions. (Repeat Previous Announcement) May we have your attention please? This is only a drill. An alarm has been activated on the (floor number) floor. For your safety, elevators will not operate until the alarm is cleared. Evacuate down the stairwells and upon reaching the ground floor exit the building to your assembly areas. Please stand by for further instructions. This is only a drill. (Repeat Previous Announcement) May we have your attention please? The alarm you recently heard has been investigated by the Fire Department. All areas and systems have returned to normal. Thank you for your cooperation. You may resume normal work activities. (Repeat Previous Announcement) May we have your attention please? The National Weather Service has issued a Tornado Warning. This means that a tornado has been sighted. Please go immediately to the refuge area on your floor. The refuge area includes: the elevator lobby, public area corridor and restrooms on each floor. Do not go to the windows or the first floor lobby! Remain in your refuge area until further notice. (Repeat Previous Announcement) May we have your attention please? Greenwood Village Police have directed a full building evacuation while they investigate a (e.g. bomb threat). Please leave the building now and meet your fire warden in your designated assembly areas outside and away from the building. You may (or may not) use the elevators. This is not a drill. Response to Fire Alarm Building occupants must always leave any area affected by fire as well as any other locations where fire or smoke may cut off their path of egress. Should evacuation become necessary, use the enclosed stairwells. If mobility impaired, proceed to the elevator lobby with your buddy, call elevator, and if it arrives, proceed down to evacuate. If the elevator does not come, call authorities to report your location. Relocation will be downward and out of the building, or as directed by the Fire Department via the emergency PA system. In case of evacuation, occupants of endangered areas will be told to evacuate first. Upon request of the Fire 13

15 Department, occupants should proceed down the nearest stairwell to the ground level and exit the building, unless mobility impaired as previously mentioned. It is the responsibility of each employee to be familiar with the evacuation plan. Each employee should be familiar with the locations of the stairwells and the procedures for evacuation of any mobility impaired employees. If evacuation of the building becomes necessary due to a fire: Inform all persons in the area of emergency. Move mobility impaired personnel to the elevator lobby safe haven unless instructed otherwise. Close doors as you leave, do not lock. FEEL DOORS BEFORE OPENING THEM. DO NO OPEN DOORS IF THEY ARE HOT. Find another exit or notify 911 if you are unable to evacuate the floor. If smoke becomes heavy, crawl. Stay close to the floor, take short breathes and stay calm. If available, hold a damp cloth over nose and mouth. This will help filter and cool the air you are breathing. Follow the instructions from PA system and your Fire Wardens Use the entire hallway and stairwell for evacuation. Once you have left your area, DO NOT return for coats, purses, etc. Use enclosed stairwells for evacuation. If mobility impaired, proceed to elevator lobby with your buddy and call the elevator. If the elevator does not come, call authorities to report your location. Use hand rails in enclosed stairwells. Keep talking to a minimum, so that instructions may be heard. No smoking. In the stairwell, watch for Fire Department coming up the stairwell to handle the emergency. Be ready to merge with people evacuating from other floors. Listen for instructions over the building emergency PA system and follow them accordingly. 14

16 KEEP CALM. Do not run or create panic. In case of evacuation- DO NOT STOP IN THE STAIRWELL. CONTINUE ON TO THE GROUND FLOOR and exit the building unless instructed otherwise via the emergency PA system. Once outside, move away from the building. Companies need to establish a designated assembly area at least 300 feet from the building. Fire Wardens should account for all company personnel and report this to who is in charge. All employees need to know the importance of being accounted for as to not have emergency personnel endangered searching for them in the hazard area. Do not stand in the fire lane or near the fire hydrants or the Fire Department s connection to the sprinkler system. Do not attempt to remove your car. Do not return to the office until the ALL CLEAR announcement is given by the Fire Department or building authority. 15

18 Bomb Threat Basic Tenant Responsibilities Each tenant should brief their telephone receptionist(s) on bomb threat procedures. Each tenant should have a copy of the Bomb Threat Checklist near the receptionist s telephone (located at the end of this section, pages 19-20). In the event of a bomb threat each tenant will be contacted by the Management Office. Police or fire personnel will make the decision whether or not to evacuate. This decision will not be made by the Management Office. If necessary, police/fire departments will instruct occupants to evacuate over the emergency PA system. Bomb Threat Emergency Procedures Received Telephone threats: 1. When a bomb threat is received by telephone, immediately ask the caller the questions listed on the Bomb Threat Checklist (page 19-20). This information will be extremely helpful to the police. 2. Keep the person on line as long as possible. Have someone else notify police at the same time. Have a prearranged signal to alert manager or supervisory personnel to listen to or record the call if possible. Advise the caller, if you can, that detonation of the bomb may kill or injure innocent people. 3. Immediately call police/fire at 911. Tell them you have received a bomb threat. Give them your company name and suite number, building name, building address, floor and name of person receiving call. 4. Notify the Building management Office at (303) that you have received a bomb threat. 5. Fill out the remaining portion of the Bomb Threat Checklist. This information will be extremely helpful to the police. 6. Do not make statements to newspapers, radio or television news. Leave that to the police. 17

19 Receiving Written Threats: 1. Written threats are less frequent then telephone threats but must be considered just as carefully. 2. Avoid physical handling of the written threat. This evidence will be analyzed by the police department for fingerprints, postmarks, handwriting and typewriting. Bomb Searching Procedures 1. It will be the responsibility of each tenant to decide if their employees are to search their own work area, coat rooms, conference rooms, computer rooms, etc. Individuals have the right to refuse to search for bomb(s). It is important that all personnel are familiar with these procedures. 2. The Building Management staff will be responsible for searching the following areas: Public Restrooms Elevators Telephone equipment rooms Stairways Fire extinguisher cabinets Entrances to the building and lobbies Electrical Rooms Trash storage areas Outside of the building Planters and shrubbery 3. Commence search of all areas. If a suspected device is found, DO NOT TOUCH IT. Call 911 first, then contact Building Management (303) and clear the immediate area. Patrol the area, keeping people away, until a police officer arrives to check the device/package. 4. Police may respond with a bomb threat specialist. Report the results of your search to police. The Police Department will make determination to call in Fire Department and additional bomb squad personnel to further search the area. 5. If the police officer believes the suspected device to be a bomb, his authority immediately exceeds that of the tenant or landlord and his instructions are to be followed. 18

20 Evacuation 1. Evacuation of the building is a decision to be made by each tenant or the police or Fire Department officials only. Building Management will not make the decision to evacuate the building. 2. In the event the decision to evacuate is made, the fastest route to safety will be stairwells. 3. Emergency personnel will make an announcement at the end of the emergency situation. Do not return to work areas until an announcement is heard. 19

23 Power Outage Xcel Energy provides electric service for Plaza Tower One. This service can be interrupted in a geographic area or to just this building. The causes run the spectrum from a blown fuse, to a power station failure. In the event of a power failure, the emergency generator will automatically activate and provide emergency power to the fire/life safety systems, stairwell lights, passenger elevators, exit signs and minimal lighting in rest rooms, parking areas and tenant spaces. There will not be enough emergency power for HVAC systems, office equipment and most lighting. In the event of a power failure all elevators will return to the ground floor where the doors will open and allow any passengers to exit. One elevator on the low rise and one on the high rise will remain in service for emergency use. Property Management will determine the cause and duration of the outage and report to the tenants so they can make a determination to evacuate or not. Certain emergency situations may cause a power failure (e.g. fire or flood) in which case tenants will be instructed what to do over the public address system. Tenants should take the following actions: If no instructions are provided: Contact the Management Office at (303) and advise of the power outage. Open blinds to let in ambient light unless under a severe weather emergency, e.g. tornado. If instructed to evacuate, and it is not a fire emergency, lock all doors before leaving. Go to your assembly area or as instructed. Do not congregate in the lobby or on the street in front and back of the building. Do not panic, remain calm and follow instructions. 22

24 Elevator Malfunction Elevators are one of the safest modes of transportation available. They can experience malfunctions due to the sensitivity of their sophisticated automatic controls, but the elevator will not free fall. WHAT TO DO IN AN EMERGENCY Push elevator emergency phone button. Phone will be automatically answered by 24-hour elevator phone monitoring service. An elevator repair person will be dispatched. They have 24-hour access to the building. Give the building name and address Plaza Tower One 6400 S. Fiddler s Green Circle Greenwood Village, CO Also give the elevator car number and which floor you are on. There is a sign in each elevator car that designates the elevator cab number. Remain calm while waiting for emergency personnel response. Help will arrive as soon as possible. If you have a cell phone, during normal building hours, call the Management Office at (303) or call your office and tell them to notify the management office of your entrapment. Answer the phone in elevator if it rings. Someone will contact you by phone or from outside the elevator door. Do not try to force open an elevator door. Do not attempt to exit from the hatch at the top of the elevator cab unless instructed to do so. Should a medical emergency take place while in a malfunctioning elevator, have the elevator phone monitoring service contact the police/ fire department and they will respond immediately. CAUTION: Jumping or bouncing in an elevator will cause the elevator to stop and become immobilized. This is a built-in safety factor for your protection. 23

25 Hazardous Material Contamination In the event that you encounter a hazardous material contamination such as oil or gas take the following actions. Remain Calm. Call 911. Give the operator your name, location of the contamination or odor and from where you are calling. Indicate the nature of the contamination if known. Notify the building Management Office at (303) Provide assistance to anyone who cannot leave the area on his or her own. Move away from the contaminant. If outside, move upwind. Do not eat, smoke, drink or apply cosmetics anywhere in the vicinity of the spill/fumes. Gas and Noxious Odors Should a tenant smell an unusual odor or gas/ammonia fumes leave the immediate area and contact the Building Management Office at (303) Do not attempt to locate the source of the odor. A building engineer will investigate and contact the fire department if necessary. 1. Do not switch on electrical equipment or light switches. 2. Do not strike a match. 3. Do evacuate the immediate area using the stairwells. Natural gas odor can be recognized by the rotten egg/dead fish smell. Ammonia leaks have a tendency to cause watery eyes and a burning sensation in the sinuses. 24

26 Civil Disorder Civil disorder refers to any disturbance outside but near enough to the building to impact normal operations. Labor disruptions (strikes) and rioting are the most common forms of civil disorder. Labor unions are permitted to picket on public property (e.g. sidewalk), but cannot disrupt traffic in or out of the building. If such disruptions occur, notify the Management Office and we will contact the police. In no case should anyone get into arguments with or counter-demonstrate against the picketers. In the event of any kind of a riot in the vicinity of the building, tenants will be notified. If deemed serious or instructed by the police, the building will be locked down and only authorized tenants will be permitted to enter. The police will determine when it is safe enough to be able to exit the building. Tenants should not become spectators (picketers want an audience and rioters want a target) stay away from windows and pull down the blinds to protect against flying glass. Building management will make a special effort to keep tenants informed of the status of the strike or other civil disorder. 25
