3 - Issue Description - With the Main Circuit Breaker, manufactured by Eaton, hereafter referred to as Cutler-Hammer RD Circuit Breaker, the following potential safety issue exists: Safety Issue Cutler-Hammer RD Circuit Breaker Toggle Handle Flag Error. Flag color reversal may mislead the service personnel that the breaker is Open when it is Closed. Eaton has identified an issue that requires your immediate attention. A red and green label, applied internally, to the toggle handle of the Cutler Hammer RD Series C breaker was misoriented resulting in an improper indication of the circuit breaker s status. ATTENTION: This condition poses a safety hazard that could result in death, serious personal injury or property damage. Verify the flag in the flag slot is the correct color. To do this, verify a green flag is showing when the circuit breaker handle is in the open (OFF) position and a red flag is showing when the circuit breaker handle is in the closed (ON) position. 4 correct circuit breaker. If the colors are reversed, the breaker is incorrectly assembled and the following corrective action steps need to be taken. 5 - Issue Correction - Eaton has issued a new overlay label to be placed on the Circuit Breaker which covers the existing flag slots and displays the correct colors in relationship to the position of the toggle handle. The potential safety issue described above has been corrected in the Cutler-Hammer RD Series C breaker manufactured after April 22, ALL Cutler Hammer circuit breakers which have been identified as having the incorrect flag showing must be retrofitted with the correct overlay label kit (Kit # ). Kit # will include Publication 2100-SB001A-EN-P that will provide instructions on how to install the correct overlay label. 6 - Requested Customer Action - Each notified customer should take the following actions: Confirm that they have the affected products. Immediately contact their local Rockwell Automation Sales and Support office to place a no charge order for the appropriate type and quantity of kits using the PSA number CPAG as reference. Follow the instructions in Publication 2100-SB001A-EN-P included in kit # to install the correct overly label.
