(200 kgs) of R407C refrigerant (halocarbon) was released Refrigerant) due to capillary tubing found broken away from the discharge line of Minor (<1M3).261 the compressor. 23-Sep-07 oil Sheen observed at the starboard side of the Northern River dive Minor (<1M3).0005 vessel. The sheen measured approximately 50m X 1m (broken). It is estimated that less than 0.5 liters of oil, from the cement pump, washed off the deck of the vessel into the sea. 19-Sep-07 gas A gas leak to atmosphere was found on a 18" suction valve around a None N/A threaded bull plug. Leak is being monitored and maintenance are investigating. 25-Aug-07 Hydraulic Oil 73 L of Hydraulic oil leaked to the sea. After further investigation, Minor (<1M3) the Alma platform was shut-in. 15-Aug-07 OIW Produced water exceedence from South Venture. OIW reading of None N/A 122 ppm for Aug. 15. 2 16-Jul-07 Radarsat observed a possible pollution incident. Upon None investigation, it was determined that because of the false fire eye alarm the platform fire pumps and deluge system were activated. After depressuring was complete and the fire pumps stopped, there were no indications of a spill from any of the systems, however, it is likely that the drains tank overflowed oil into the sea (the amount is extremely difficult to estimate as the drains tank indicated 100% full); or that the deluge water may have affected the surface tension of the sea and lead to a signal to the satellite radar system suggesting oil may have been present. 07-Jul-07 Newly installed PSV on gas contactor leaked gas to the atmosphere Atmosphere on start up of system. No threat to personnel. Depressured unit and changed out PSV. The leak was on a 1/4" bleed port and lasted for about 15 minutes. 25-May-07 Engine leak on Alma generator liters of oil contained on None deck outside enclosure. Location of leak was close to the generator fan causing oil to be blown into the enclosure door. Oil leaked out via rubber seal in the door. Leak detection verified to be working. Oil level in pump not enough to activate detection system. 15-Mar-07 During pre-start-up activities at the North Triumph platform, while re- Minor (<1M3) establishing flare system after shutdown, the operator noticed misting at the flare tip when opening up the flare line. A sheen was observed with the dimensions: 30 feet X 1000 feet and was bluishsilver in colour. This translates to an expected volume (using the oil sheen calculator) of 260 ml to the sea (later determined it was closer to 3/4 Litre). Wednesday, November 07, 2007 Page 2 of 10

3 14-Mar-07 Thebaud control room operator had a gas detector ring in for the Venture Platform at 12:30 AM. The gas detector was reading 16% and upon investigation a second gas head was reading 7%. The control room operator called the Venture OIM and he investigated and found the vent drum pump seal leaking and shut down production. The vent drum pump seal was replaced and production was restarted with no issues. 09-Feb-07 Contained release after equipment damage. The compression module firewater pump "B" cooler failed. Water ingress to gear box resulted in seal failure and subsequent loss of gear box oil (contained). 07-Feb-07 oil in water The platform open drains system became blocked washing grout Minor (<1M3) material from the jacket grouting work into the open drain system on the cellar deck east. The blockage was noticed when water from the water polishing unit was being drained to the tank to allow for filter media change. Some water from the open drains system began to over flow the tank stand pipe. This resulted in a water stream with a small amount of oil being released from the platform. A sheen was observed on the water in the vicinity of the platform. Sea state at the time was approximately 2-3 meters resulting in the sheen being broken up. 07-Feb-07 Exceedence - deck drains plugged / frozen causing overflow. Measured at 25 mg/l 01-Feb-07 oil in water Approximately 16.9 mg/l of oil in water concentrate spilled into the Minor (<1M3) sea while discharging the deck drainage water via the overboard drain treatment system. 4 19-Jan-07 Contained release - hydraulic tubing connector parted on T6 master, approx 75 litres of oil contained on the deck (nothing went to the sea). 11-Jan-07 Ocean-Rite AW Hydraulic While performing ROV pipeline surveillance operations, the Oil 22 hydraulic components of the ROV leaked approximately 8 liters of Minor (<1M3) biodegradable ocean-rite AW hydraulic oil 22 to the sea. 31-Dec-06 Hydraulic Fluid A hydraulically operated fail safe valve on a subsurface pipeline Minor (<1M3) 0.37 went closed and shortly afterwards the hydraulic pumps tripped off on low supply. It appears there is a failure in the supply line to the subsurface pipeline valve. Estimated volume of spill is approximately 370 liters (to be confirmed). (SSIV-3136E) 30-Oct-06 Contained release. Diesel Generator 3241 developed leak on Oil Plug. Unit trip on Low oil pressure. Approx 10 litres of oil contained within the enclosure. 29-Oct-06 Hydraulic Oil Oil Spill Incident - Hydraulic oil overflow from South Venture SCSSV Minor (<1M3).09 returns tank. Approx 90 liters spilled to sea. 28-Oct-06 Contained release on P-3242 generator controlled start up, two gas detectors rang in 14 and 6% respectively. 5 04-Oct-06 Dow Therm SR Maersk Gabarus was alongside Thebaud platform unloading glycol- Minor (<1M3) 0.3 based compression engine coolant. The activity required the hose to be connected for over 3 hours. The transfer hose was caught in the thruster and parted, spilling 300 L of coolant to the sea. 24-Aug-06 oil Exceeding the normal operating practice of 15 mg/l of oil in deck Minor (<1M3) drainage water (OWTG) 39 cubic meters of slops were received from the "Sable Sea" on Aug. 23. the slops were a combination of deck water from various platforms. On Aug. 24 Thebaud started to process and discharge the Sable water through A then B filter, discharge was stopped. On Aug. 27 and then again Aug. 29 they tried again and discovered it was leaking and halted discharge. 02-Aug-06 Hydraulic Oil 28 L of hydraulic oil leaked from the starboard Controllable Pitch Minor (<1M3) Propeller (CPP). 7 L released inside the Thebaud 500m zone as well as 21 L released while the vessel was en route to Thebaud. (56 L in total spilled to the sea). Vessel sent to shore base in NFLD. For repair. 05-Jul-06 Oil Exceeded the normal operating practice of 15mg/L of oil in deck Minor (<1M3) 0.01 drainage water when Thebaud started to process and discharge deck drainage water via the crudesorb unit. The reading after the first hour (1715 hrs) was 31.7 mg/l through the A filter. The unit was switched to series operation (through A filter then through B filter). The next reading at 1815 hrs was 30.8 mg/l and discharge was stopped. Less than 10 L spilled to the sea. Exxon to monitor on an ongoing basis. 28-Jun-06 oil A leak was discovered during morning platform rounds, a sheen was Minor (<1M3) noticed on the water. Upon further investigation, it was discovered that the vent drum pump packing was leaking. The pump was immediately shutdown and isolated. Estimated <2 litres condensate spilled to the sea. 6 27-Jun-06 hydraulic oil While cleaning with the ROV, the pilot noticed hydraulic fluid Minor (<1M3) 0.02 coming from the hose supplying the cleaning tool. Approximately 20 L of hydraulic oil was spilled to the sea (the ROV uses Ocean-Rite AW Hydraulic Oil 22 which is a refined white mineral oil.) Immediately he shut down the marine growth removal brush motor which operates on auxiliary hydraulic servo valve. Recovery operations for the ROV took place right away. 17-Jun-06 gear oil While the Hebron Sea was discharging cargo at Thebaud Platform Minor (<1M3) on June 17th, a trace amount of oil on the water surface on the starboard side was observed (est. 100 ml). 05-May-06 Diesel Diesel leak through from catch tray drain line to ocean. This drip Minor (<1M3) pan has a valve assembly to allow for draining of water and it appears as though the valve may have been frozen over winter. As a result this freezing split the valve assembly. Estimated maximum of 47 ml to the sea. 01-Apr-06 Hydrocarbon Condensate Packing on condensate valve leaking on deck. Approx. 50 L dripped Minor (<1M3).05 to the sea. Dissipated quickly with no sheen visible. 31-Mar-06 Loss of small amount (less than 1 liter) of hydraulic fluid while tying in on Alma 3. No fluid to sea. 08-Feb-06 Gas A small leak was discovered on the HP Separator Flow Control Minor (<1M3) very small amount Valve (PDV 7102) on South Venture. The leak was occurring around valve packing. Wednesday, November 07, 2007 Page 6 of 10

7 18-Jan-06 Hydraulic Oil The return line (hose) blew off on top of the hydraulic unit at the Minor (<1M3).0002 motor shed, 0.2 litres of hydraulic oil spilled to the sea. 12-Jan-06 MEG Concentration Leak in 3" Mono-ethylene Glycol (MEG) pipeline between Alma and Significant (>10M3) Thebaud Platforms. The Thebaud to Alma MEG 3" pipeline experienced a failure at an elbow at the base of the Thebaud jacket. The line is leaking MEG at an undetermined rate. 09-Jan-06 Produced Water While shutting in the Venture platform prior to rig move a light sheen Minor (<1M3).0006 measuring approx. 30m X 30m was discovered. The source was the produced water overboard line as the process level control and the safety system level controls on the degassing drum were both contaminated with sand and would not function, allowing the produced water overboard valve to remain open 0.6L produced water to the sea. 19-Dec-05 Synthetic Based Oil While transferring OBM at the Galaxy II, it is estimated that a Minor (<1M3).0002 maximum amount of 200 ml of synthetic based oil may have reached the sea from the Hebron Sea as part of the 20 litres inadvertent overboard release of mixed slops from the deck of the vessel. The slops constituents include oil based drilling mud (approx 1%) and water (balance) 02-Dec-05 hydraulic oil The actuator on the condensate level control valve (LV-7113 B) Minor (<1M3).005 leaked hydraulic oil on to the deck of the South Venture platform, a small amount of oil (5 liters) spilled from the deck to the sea. A sheen measuring 3 metres by 400 metres was observed adjacent to the South Venture platform. The sheen dissipated quickly due to sea state. Wednesday, November 07, 2007 Page 7 of 10

8 27-Nov-05 Hydrocarbon Unexplained showing on the water during cargo operations. During Minor (<1M3) offloading of the Sable Sea, along side the Thebaud facility, it was noticed a light sheen on the water around the vessel. The sheen was silvery in appearance and approx 15 ft wide and 80 ft long. The vessel was contacted and they confirmed that they were not discharging anything from the boat. At the same time, the operation personnel on Thebaud investigated the process on board. It was determined that Thebaud's overboard discharges were not the cause of the sheen. No source was found from Thebaud. When the vessel moved away from Thebaud the sheen disappeared. The sheen was estimated as 7 ml of hydrocarbon from an unknown source. 11-Nov-05 Condensate A sheen was reported, the result of skimming the crude sorb filter D- Minor (<1M3) While skimming the filter about 20 liters of produced water was bled off the top of D-3141 through a hose and inadvertently released 4 ml of condensate over the side to the sea. 29-Aug-05 Hydrocarbon Well Services Supervisor noticed a small sheen underneath the Minor (<1M3) 2.4e-005 cellar deck. The vent drum pump packing on the cellar deck of Venture Platform had failed and process fluids had filled the berm with some overflow to the sea. Based on the sheen size, it was estimated 24 ml of hydrocarbon had overfilled the berm along with an undeterminable quantity of produced water. 05-Aug-05 Hydraulic oil While restarting South Venture 3 well (via opening the hydraulic Minor (<1M3) 5.5e-005 master valve), the hydraulic reservoir tank low level alarm initiated. Further investigation by the control room operator noted that the tank level had dripped 4% over the previous 4 days. The intervention maintenance team that was on the Alma platform was sent to South Venture to investigate. Upon arrival they found SSV 7004 hydraulic supply fitting loose and hydraulic oil leaking to the sea (approx. 55 liters). Wednesday, November 07, 2007 Page 8 of 10

9 11-Jul-05 Condensate Condensate carryover from degasser drum in produced water during Minor (<1M3).0001 flow rate testing on group separator with Gel (J572). Degasser drum PSV lifted allowing +95 ml of condensate to be discharged overboard with produced water. 28-Jun-05 Hydrocarbon A worker picked up a bucket that was filled with produced water, the Minor (<1M3) 1.2e-005 bucket had grease on the handle and it inadvertently tipped over resulting in approximately 12 ml of hydrocarbon spilling into the sea. 15-Jun-05 Hydraulic oil While conducting the Sable Island annual pipe line survey on the Minor (<1M3) export line, the ROV low oil hydraulic alarm sounded at approx. 7:00 a.m. The ROV was lifted to the docking Snubber and the vacuum poppet on the Servo chest sucked in, thus spilling approx. 4 liters of hydraulic oil into the sea. 10-May-05 Hydraulic oil The control room operator noticed the hydraulic tank level had Minor (<1M3).0975 dropped from 44% to 22% in 12 hours. Received notification on May 16/05 that there was 97.5 liters of hydraulic oil spilled to the sea. 04-May-05 Oil While completing a deck clean up, a garbage bag containing mixed Minor (<1M3).0001 waste including oily rags was lifted from it's container. The bag had a hole in it and oil leaked onto the deck then into the sea. Estimated volume of 100 ml was released into the sea. 10-Apr-05 Hydraulic fluid During a planned intervention to the Alma platform, hydraulic fluid Minor (<1M3).065 was found to be leaking from the internals of the actuator assembly on the production separator condensate level control valve LV 6102B. Approximately 65 L of hydraulic fluid escaped from the actuator through the grating to the sea. Wednesday, November 07, 2007 Page 9 of 10

10 21-Mar-05 Condensate The PSD was triggered from a pressure increase in the Venture Minor (<1M3).0005 degassing drum. As the pressure in the degassing drum continued to build up after the PSD, it was concluded that the shutdown valve from the production separator was passing, causing the pressure to build up. The production separator was blown down to prevent the continued build up of pressure. The PCV4123 valves failed closed, while the repairs were being carried out less than 0.5 liters of condensate spilled into the sea. 10-Mar-05 SBM Galaxy II rig inadvertently discharged not more than 19 liters of Minor (<1M3).019 Synthetic Based Mud into the ocean Wednesday, November 07, 2007 Page 10 of 10
