1 Biological Safety Cabinet Failure Response Procedure Biosafety Officer Steven Cole T248 Basic Science Building Winnipeg, Manitoba Canada R3E 0W3 Tel: (204) This procedure is intended to describe the steps which should be taken to protect worker safety in the event of a biological safety cabinet failure. Signs of Biological Safety Cabinet Failure If any of the following signs of biological safety cabinet failure are observed work should be stopped as quickly and safely as possible. 1. Power failure lights within the cabinet will go out and the blower motor will stop. Laminar flow within the cabinet has failed. 2. No airflow if you can t feel the downward airflow in the cabinet the air curtain has failed. 3. Alarm sounding or visible on cabinet modern biological safety cabinets are commonly equipped with audible or visual alarm systems which warn the operator when the cabinet malfunctions. 4. Unusual noises if unusual noises such as squeaking, squealing, loud humming, knocking, or buzzing occur they could indicate failure or imminent failure of the cabinet s mechanical and/or control systems. 5. Unusual smells unusual smells such as ozone or smoke could indicate a fire within the cabinet. Response Procedure If any of the signs of cabinet failure described above are observed when starting or attempting to start the biological safety cabinet do not start work. If any of the signs of cabinet failure described above are observed while work is occurring in the cabinet, follow the steps below. 1. Stop working. If safe to do so follow steps If conditions are immediately dangerous to life and health evacuate. 2. Move slowly and smoothly to prevent aerosols from spreading. 3. Cap, cover or otherwise package any vessels which contain biological agents. 4. Back out of the biological safety cabinet and close the sash (if it closes) 5. Remove and discard your gloves as biohazardous waste. 6. Turn off power to the cabinet. 7. Advise other workers in the area of the cabinet failure. 8. Complete, print and post the attached warning sign on the sash of the cabinet. 9. 2 10. Don the appropriate personal protective equipment (gloves, lab coat or back fastening gown, safety glasses and a fit tested N95 respirator). 11. Open the sash of the cabinet slowly and carefully to prevent aerosols from spreading. 12. Package and decontaminate in order tools/equipment, sample containers, waste and the cabinet working surface and interior. 13. Complete an incident report with your supervisor and forward it to the Biological Safety Officer. 14. Contact ConTest to service your cabinet before next use. 4 Centrifuge Containment Failure Response Procedure Biosafety Officer Steven Cole T248 Basic Science Building Winnipeg, Manitoba Canada R3E 0W3 Tel: (204) This procedure is intended to describe the steps which should be taken to protect worker safety in the event of centrifuge containment failure. Centrifuge containment systems seal the centrifuge bottles and rotor to prevent the release of infectious aerosols. Signs of Centrifuge Containment Failure If any of the following signs of centrifuge containment failure are observed work should be stopped as quickly and safely as possible. 1. Centrifuge becomes unbalanced samples should be carefully balanced before being loaded into the centrifuge. If the samples become unbalanced it may be due to a leak and the centrifuge will begin to knock and/or shake. 2. Centrifuge makes unusual noises knocking, humming, and shaking could be as a result of a load being or becoming unbalanced. 3. Explosion explosion inside the centrifuge may indicate a catastrophic failure of the rotor or bottles. Response Procedure If any of the signs of centrifuge containment failure described above are observed when starting or attempting to start the centrifuge do not start work. If any of the signs of centrifuge failure described above are observed while the centrifuge is in operation, follow the steps below. 1. Stop working. If safe to do so follow steps If conditions are immediately dangerous to life and health evacuate. 2. Turn off power to the centrifuge and do not open. 3. Advise other workers in the area of the potential failure. 4. Complete, print and post the attached warning sign visibly and conspicuously on the centrifuge. 5. Vacate the area and wait 30 minutes for aerosols to settle. 6. Don the appropriate personal protective equipment (gloves, lab coat or back fastening gown, safety glasses and a fit tested N95 respirator). 7. Open the door of the centrifuge slowly and carefully to prevent aerosols from spreading. 8. Assess the inside of the centrifuge to determine if centrifuge containment has failed. Signs of failure could include liquids splattered inside the unit, presence of debris, cracked or broken rotor. Centrifuge Containment Failure Response Procedure December 2015 SCC

5 a. If the rotor appears sound and the inside of the centrifuge appears uncontaminated remove the rotor and place in a certified biological safety cabinet to open and inspect the contents. b. If the rotor is not sound or the inside of the centrifuge appears contaminated the rotor is to be packaged to prevent leaks and transferred to a certified biosafety cabinet for inspection and decontamination. c. If the inside of the centrifuge is contaminated it must be cleaned and disinfected using an effective disinfectant. 9. Complete an incident report with your supervisor and forward it to the Biological Safety Officer. 10. Contact your service provider to inspect and certify that the centrifuge is safe for continued operation before next use. Centrifuge Containment Failure Response Procedure December 2015 SCC
