1 CHRISTIAN BROTHERS RISK MAGEMENT SERVICES Self Inspection Checklist Building: Date: Inspected By: Time Began: Time Ended: PARKING LOTS Are gates operable? Condition of striping? Condition of parking lot concrete/asphalt? Warning signs (stop, yield, slow, etc.)? Condition of parking stones? Is there adequate lighting? Condition of sidewalks? GROUNDS Is the fencing intact? Are pot holes repaired? Are trees and shrubs trimmed? Exposures from neighboring property? BUILDING EXTERIOR Exterior walls (tuck pointing)? Condition of roofs? Condition of windows?
2 Condition of signs? ALARM SYSTEMS Is the fire alarm system tested? Is the smoke detection system tested? Is the burglar or intrusion alarm tested? Are emergency phone numbers posted and readily available to all persons on the premises? BOILERS & INTERL HEATING SYSTEMS Is the storage of flammable and combustible materials prohibited in the boiler room or in and around the internal heating system? Is the boiler log being kept up to date to make sure the necessary tests, maintenance and services are performed? Are all boiler and heating systems thoroughly and periodically inspected? Is the boiler inspection certificate posted? Are pipes in good condition? ENGINES, PUMPS, COMPRESSORS, AND FANS Is the machinery securely installed and properly supported? Are emergency stop buttons identified on the equipment? Are guards in place on the machinery to minimize the hazard of being struck by, striking against, caught on, in, or between?
3 ELEVATORS Is a record of the contracted inspections maintained in a central location? Is there a method of emergency communication available on the elevator and does it work? Is a sign posted on the elevator that reads "Do Not Use in the event of a fire? FIRE PREVENTION AND PROTECTION Are automatic sprinklers blocked by storage? Are fire extinguishers in good condition? Are fire extinguishers and hoses clearly marked, readily accessible and properly mounted? Are fire doors unobstructed and protected from obstruction? Are fusible links in place on the fire doors? Are the sprinkler control valves locked in an open position? Have portable heaters been removed? CHEMICAL SUBSTANCES Is an inventory of all chemical substances available? Are all chemicals properly labeled, dated, and stored? Have Material Safety Data Sheets (MSDS) been obtained on all chemicals?
4 ELECTRICAL Are all disconnecting switches and circuit breakers labeled to indicate their use or equipment served? Are electrical switches, receptacles, junction boxes, etc. provided with tight fitting covers or plates? Are all unused openings (including conduit knock outs), electrical enclosed and fittings closed with appropriate covers, plugs or plates? Is exposed wiring or cords with frayed or deteriorated insulation repaired or replaced properly? Is the use of flexible cords in place of permanent wiring prohibited? Are multiple plug adapters limited? Do extension cords and electrical equipment have a grounding conductor? EXITS AND MEANS OF EGRESS Are doors or other passageways, which are neither exits nor access to an exit, located where they may be mistaken for exits? If so, are they appropriately marked Not An Exit, To Basement, or Store Room, etc. Are all exits visible and unobstructed? Are the directions to exits, when not immediately apparent, marked with visible signs? Are all exits marked with a readily visible sign that is properly illuminated? Is panic hardware operable? Is emergency lighting working?
5 RECEIVING Are grates or similar type covers over floor openings such as floor drains, of such design that floor traffic, or rolling equipment will not be affected by the grate spacing? Are forklifts in good working conditions? Are hand carts in good condition? Are dock boards (bridge plates) used when loading or unloading operations are taking place between vehicles and docks? Are trucks and trailers secured from movement during loading and unloading operations? Housekeeping? PROCESSING Are aisles and passage ways kept clear? Are material handling carts in good condition? Are carts being used? Sorting areas well organized? Ergonomic concerns? STORAGE Basement/attic storage housekeeping? Combustible material storage? Flammable liquid storage? Garage/storage sheds? Are all areas kept clean and dry and arranged in an orderly fashion? Are aisles and passageways kept clear?
6 Are floor openings guarded by a cover, a guardrail, or equivalent on all sides (except at entrance to stairways of ladders)? Are toe boards installed around the edges at permanent floor openings (where persons may pass below the opening)? HAND TOOLS AND EQUIPMENT Are all machines or operations that expose individuals to rotating parts, pinch points, flying chips, particles, or sparks, adequately guarded? Are power tools equipped with safety guards? Are portable electric tools and equipment guarded or double insulated? Do portable electric tools have appropriate grounding plugs? Are lawn and garden and snow removal equipment properly maintained and stored? SECURITY Are doors/windows equipped with locks? Is a safe used for monies and valuables? Are buildings inspected after use, prior to locking? WALKING AND STANDING SURFACES Are aisles and walkways marked as appropriate? Are aisles and passageways kept clear? Are holes in the floor or other walking surface repaired properly, covered, or otherwise made safe?
7 Are changes of direction or elevations readily identifiable? Are standard guardrails provided whenever aisle or walkway surfaces are elevated more than 30" above any adjacent floor or the ground? Condition of tile floor? Condition of carpet floors? Condition of floor mats/runners? Are steps on stairs and stairways designed or provided with a surface that renders them slip resistant? Are stairway handrails anchored tight? Are stair treads in good condition? Are portable wood ladders and metal ladders adequate for their purpose and in good condition? VEHICLE SAFETY Are vehicles equipped with a first aid kit and fire extinguisher? Is there an occupant restraint system working in all vehicles? Condition of vehicles? CONTRACTED SERVICE Last Inspection Next Inspection Alarm System Back flow valves for city water Boiler inspection Elevator inspection Fire extinguishers Heat/smoke detection alarms Sprinklers These general guidelines are merely a tool to assist in your safety survey efforts. But, like any tool, it has certain limitations. These guidelines must be used with good judgment since they can never cover all the possibilities. These guidelines will be periodically reviewed and every attempt will be made to keep them up-to-date. By the same token, you should review these periodically, and as circumstances dictate, adapt them to meet your particular needs. Remember, there is no substitute for an inquisitive state of mind, and some degree of good judgement and common sense.
