2 09 May 2016 Introduction to the Report The following report contains a site profile and summary of non-conformities identified during an onsite assessment commissioned by the Alliance for Worker Safety (Alliance) and conducted by a third-party Qualified Assessment Firm (QAF). The assessment was conducted against the Alliance for Worker Safety Assessment Protocols (APs) and Fire Safety and Structural Integrity Standard, which is harmonized with the factory assessment guidelines developed by University of Engineering and Technology (BUET) for the National Tripartite Plan of Action (NTPA). The goal of the Alliance process is to provide clear and practical technical requirements by which i Ready Made Garment (RMG) Factories producing for Alliance members may be consistently and fairly evaluated for fire, structural, and electrical safety in a non-duplicative manner. Each assessment will prompt action plans that will be used by RMG factories to systematically and sustainably improve safety conditions for garment workers. Beyond tracking and reporting on action steps taken in a transparent manner, the Alliance organization and its members will seek to further support factory improvements through technical assistance, training, implementation support for functional Worker Committees, and in some cases financial assistance and wage support for workers if factories are closed for remediation. The contents of the report do not constitute a guarantee of compliance with the applicable laws, the Alliance Standard or the absolute or continued safety against fire, electrical and/or structural integrity issues that may lead to injury or loss of life. The report is designed to provide a non-exhaustive summary of risk issues, based on a limited sampling and duration of time onsite by the named QAF. Prepare as-built drawings for new fire safety features of the building. 25 Aug 2016 Alliance Standard reference 8.20 Fire Protection Construction Are openings and penetrations through rated walls and/or assemblies protected? Wall openings were found due to sprinkler system work of in several cases. Photograph: openings were found in walls. Provide fire-resistive rated opening or penetration protection for rated walls and assemblies in accordance with Alliance Standard Sections 4.6 and 4.7. 6 09 May 2016 Is each floor separated with a fire-resistive rated construction barrier? Some unprotected penetrations were found in the Factory Building. Electrical conduit penetrations from the ground floor to the top floor are not properly protected. Another opening due to transport of goods from one floor to another floor was also not properly protected. Photograph: Penetrations found in several cases. Provide fire-resistive rated construction barriers between floors in accordance with Alliance Standard Section 4.5. Provide fire-resistive rated penetration protection for rated walls and assemblies in accordance with Alliance Standard Section 4.7. Consult a qualified fire protection engineer to design the required penetration systems. 13 Aug 2016 Reference Alliance Standards Part 4 Section 4.5 Separation Are separations between hazards provided with fire-resistive rated construction barriers. Compressor room on the 6th floor adjacent to the production floor is not properly separated with fire resistive rated barrier. Photograph: Compressor room is not separated with fire rated door. Provide fire-resistive rated construction barriers (with associated opening protection) between hazard types in accordance with Alliance Standard Sections 3.4 and 4.5. Consult a qualified fire protection engineer to design the required rated construction barrier. 7 09 May 2016 Apply to appropriate authority in an expeditious manner for issuance of the Certificates of Occupancy for each building and ancillary structure according to building use. 02 Jul 2016 Are certificates of occupancy provided for each building or ancillary structure? Fire Protection Systems Does the building have a fire pump? Factory has approved design drawings of standpipe and sprinkler system, but fire pump is not installed in the factory building. Visual Assessment: According to standard, factory needs to install Fire pump but Fire pump was not available. Install a dedicated fire pump for the facility in accordance with NFPA 20 to supply the demands of the connected fire protection systems along with a stored source of water sufficient to meet the demands in accordance with NFPA 22. Fire pump installation is to be tested for final acceptance in presence of Alliance and a final inspection of the installation shall be conducted by the Alliance prior to final acceptance of the installation by the Alliance as per clause Acceptance testing of the installation shall be in accordance with NFPA 20, 22, and 25 testing requirements. Documentation of all testing shall be submitted to the Alliance for review prior to final acceptance by the Alliance. The pump is to be connected to an alternative power source such as a generator. The generator is to be configured with an ATS (auto starter). Alliance Standard Part 5 Fire Protection Systems Does the building have a Standpipe System? Hose connections are provided by overhead water tank, but standpipe System is not installed throughout the factory. Factory has approved design drawings of standpipe and sprinkler system, but standpipe system is not installed in the factory building. Alliance Standard Part 5 Section Trouble or alarm notifications were not indicated on the fire alarm control panel. Addressable Fire alarm control panel is not installed. Visual Assessment: According to Visual Assessment of this factory on 09 May, Install an automatic fire alarm system for the facility which is centrally monitored and interfaced with sprinkler system and provide audible and visible device. System shall comply with the Alliance Standard and NFPA 72. Consult a qualified fire protection engineer and/or authorized fire alarm company to design and install the system. Alliance Standard Part 13 Section Maintenance of Fire Protection Equipment Are notification and initiation devices for the fire alarm system installed at required locations based on occupancy type? Notification and initiation devices for the fire alarm system are installed but are not connected to centralized fire alarm control panel. Visual Assessment: According to Visual Assessment of this factory on 09 May, Install initiating devices and notification appliances as required by the Alliance Standard and NFPA 72. This includes electrical supervision of all valves controlling fire protection systems (sprinklers, fire pumps, water supplies, etc.). Connect devices to an automatic fire alarm and detection system for the facility. All fire alarm installations or modifications shall be documented with shop drawings and submitted for review by the Alliance prior to commencement of installation. 9 09 May 2016 Pull stations at egress points, smoke detectors in air handling equipment, visual and audible devices spaced appropriately based on occupancy type. Reference NFPA 72 Is the building protected by an automatic sprinkler system? Non-Compliance Level: 1 Occupied floor height is 76 ft, so sprinkler system is required in accordance with the Alliance Standard. Factory has approved design drawings from the Alliance for the sprinkler system. Installation of sprinkler system is ongoing. Photograph: installation of sprinkler system is under process. Complete installation of automatic sprinkler system throughout the building in accordance with design drawings and Alliance Standard. 04 Feb 2017 Reference Alliance Standards Part 3 Section Existing Buildings, Part 5 Section 5.3 Automatic Sprinkler Systems and Section 6.13 Travel Distance Fire extinguishers are inspected, tested, and maintained as required. Non-compliant fire extinguisher inspection, testing, and maintenance reports were found. Photograph: Factory provided documentation, but it was not compliant with all aspects of NFPA 10. Establish an inspection, testing, and maintenance program for all fire extinguishers and prepare proper documentation. Program must comply with NFPA 10. NFPA 10 Chapter 7 Is the fire alarm and detection system monitored by a central station monitoring service or directly connected to the Fire Service and Civil Defense? Factory has a conventional detection system, but an addressable detection system is not provided. The Alliance for Worker Safety Page 9/13

10 09 May 2016 Visual Assessment: According to Visual Assessment of this factory on 09 May, Install a new automatic fire alarm and detection system. Once installed, arrange for direct connection of the fire alarm and detection system to a central station monitoring service or the Fire Service and Civil Defence as per Alliance Standard Section Until that time, a person trained to contact the Fire Service and Civil Defence in the event of fire alarm activation shall be provided. An annunciator shall be located in a constantly attended location (such as a fire control room) to alert this person. Alliance Standard Part 5 Section Monitoring Are inspection, maintenance, and testing procedures of the sprinkler system documented and up to date? Low Non-Compliance Level: 1 Factory has approved design drawings from the Alliance for the sprinkler system. Installation of sprinkler system is ongoing. Inspection, maintenance, and testing procedures for the sprinkler system are not occurring yet. Document Review: According to Visual Assessment and document review of this factory on 09 May, After installing the system, establish an inspection, maintenance and testing program for the sprinkler system. Program needs to comply with the requirements of NFPA Feb 2017 Reference NFPA 25 Chapter 5 Sprinkler Systems Table Means of Egress Means of egress are free from impediments, obstructions, and stored materials. Means of egress were found obstructed in several cases during assessment. Photograph: Storage beside staircases. Remove all impediments, obstructions, and stored materials from the means of egress. Keep all elements of the means of egress (exit path, aisles, stairs, corridors, etc.) continuously free and clear of all obstructions in accordance with Alliance Standard Section The Alliance for Worker Safety Page 10/13

11 09 May Jun 2016 Alliance Standard Part 6 Section Impediments to means of egress and Section Reliability Emergency power for means of egress illumination is verified at least once per year. If battery operated lights are used, these lights are tested on a monthly basis. Functional testing of battery powered lights is provided for a minimum 90 min once per year. Emergency power is provided for means of egress illumination, but it is not verified at least once per year. IPS operated lights are used but lights are not tested on monthly basis. Document Review: Factory does not maintain any document to verify or testing emergency power for means of egress illumination. Develop a testing and maintenance program that ensures the emergency power for all egress lighting is verified at least once per year. If batteryoperated lights are used, these lights shall be tested on a monthly basis. Functional testing of battery powered lights shall be provided for a minimum 30 min once per year. Alliance Standards Part 10 Section Illumination of Exit Signs and Means Of Escape Lighting Emergency power for exit signs is tested at least once per year. If battery operated, these lights are tested on a monthly basis. Functional testing of battery powered signs is provided for a minimum 90 min once per year. Emergency power is provided for exit sign illumination, but it is not verified at least once per year. Document Review: Factory does not maintain any document to verify or test emergency power for exit sign illumination. Develop a testing and maintenance program that ensures the emergency power for exit signs is verified at least once per year. If battery-operated signs are used, these signs shall be tested on a monthly basis. Functional testing of battery powered signs shall be provided for a minimum 90 min once per year. 12 09 May 2016 Alliance Standard Part 10 Section Illumination of Exit Signs and Means Of Escape. Every door in a stair enclosure serving more than 5 stories is provided with reentry unless it meets the requirements of Alliance Standards Part 6 Section Re-entry is not provided for the main factory building. Visual Assessment: According to visual assessment. Provide re-entry to floor levels from the stairwells in accordance with Alliance Standard Section Aug 2016 Alliance Standards Part 6 Section 6.8 Doors and Gates Occupant loads are posted for every assembly and production floor in a conspicuous space near the main point of egress. Occupant loads were not posted anywhere at the assembly points or production floor areas of this factory. Visual Assessment: According to Visual Assessment of this factory on 09 May, Post the occupant load for all assembly and production floor areas in a conspicuous space near the main exit or exit access doorway for the space in accordance with Alliance Standard Section Jul 2016 Alliance Standards Part 6 Section Posting of Occupant Load Fire Safety Programs Training programs are implemented and documented in accordance with the Alliance Safety Training Curriculum.
